    Marine Plastic Drift from the Mekong River to Southeast Asia

    Southeast Asia is the world’s most polluted area in terms of marine plastics. The Mekong River is one of the largest rivers in the area, and ranked as somewhere between the 8th- and 11th-biggest contributor to plastics in the world’s oceans. Here, we investigate how microplastics drift from the Mekong river to Southeast Asia, and which coastlines are most exposed. We identify potential factors (wind drift, rivers, vertical mixing and sinking rates) that affect plastic drift in the region using the OpenDrift model with realistic wind and ocean currents for simulations between three months (summer and winter) and 15 months. We find that the seasonal drift is influenced by the monsoon systems and that most of the plastics strand in the Philippines and Indonesia. In addition, the role of wind drift is significant in strong winds. Vertical mixing and sinking rates are unknowns that affect the relative importance of wind drift (near the surface) and ocean currents. Simulations with different terminal velocities show that, unsurprisingly, the higher the terminal velocities are, the closer they deposit to the source. In light of the large uncertainties in sinking rates, we find that the plastic distribution has large uncertainties, but is clearly seasonal and influenced by wind, vertical mixing, river discharge and sinking rates. The Philippines and Indonesia are found to have the coastlines that are most exposed to plastic pollution from the Mekong river. This study shows that simulations of marine plastic drift are very variable, depending on many factors and assumptions. However, it provides more detailed information on marine plastic pollution in Southeast Asia, and hopefully helps authorities take more practical actions.publishedVersio

    Establishment of a Percutaneous Coronary Intervention Registry in Vietnam: Rationale and Methodology

    Copyright: © 2020 The Author(s). Background: In lower- and middle-income countries across Asia there has been a rapid expansion and uptake of percutaneous coronary intervention (PCI). However, there has been limited routine collection of related data, particularly around quality, safety and cost. The aim of this study was to assess the viability of implementing routine collection of PCI data in a registry at a leading hospital in Hanoi, Vietnam. Method: A Vietnamese data collection form and collection strategy were developed in collaboration with the Vietnam National Heart Institute. Information on patient characteristics, treatments, and outcomes was collected through direct interviews using a standardised form and medical record abstraction, while PCI data was read and coded into paper forms by interventional cardiologists. Viability of the registry was determined by four main factors: 1) being able to collect a representative sample; 2) quality of data obtained; 3) costs and time taken for data collection by hospital staff; and 4) level of support from key stakeholders in the institute. Results: Between September 2017 and May 2018, 1,022 patients undergoing PCI were recruited from a total of 1,041 procedures conducted during that time frame. The estimated mean time to collect information from patients before discharge was 60 minutes. Of the collected data fields, 98% were successfully completed. Most hospital staff surveyed indicated support for the continuation of the activity following the implementation of the pilot study. Conclusions: The proposed methodology for establishing a PCI registry in a large hospital in Vietnam produced high quality data and was considered worthwhile by hospital staff. The model has the potential opportunity for replication in other cardiac catheterisation sites, leading to a national PCI registry in Vietnam

    Antibiotic use and prescription and its effects on Enterobacteriaceae in the gut in children with mild respiratory infections in Ho Chi Minh City, Vietnam. A prospective observational outpatient study.

    BACKGROUND AND OBJECTIVES: Treatment guidelines do not recommend antibiotic use for acute respiratory infections (ARI), except for streptococcal pharyngitis/tonsillitis and pneumonia. However, antibiotics are prescribed frequently for children with ARI, often in absence of evidence for bacterial infection. The objectives of this study were 1) to assess the appropriateness of antibiotic prescriptions for mild ARI in paediatric outpatients in relation to available guidelines and detected pathogens, 2) to assess antibiotic use on presentation using questionnaires and detection in urine 3) to assess the carriage rates and proportions of resistant intestinal Enterobacteriaceae before, during and after consultation. MATERIALS AND METHODS: Patients were prospectively enrolled in Children's Hospital 1, Ho Chi Minh City, Vietnam and diagnoses, prescribed therapy and outcome were recorded on first visit and on follow-up after 7 days. Respiratory bacterial and viral pathogens were detected using molecular assays. Antibiotic use before presentation was assessed using questionnaires and urine HPLC. The impact of antibiotic usage on intestinal Enterobacteriaceae was assessed with semi-quantitative culture on agar with and without antibiotics on presentation and after 7 and 28 days. RESULTS: A total of 563 patients were enrolled between February 2009 and February 2010. Antibiotics were prescribed for all except 2 of 563 patients. The majority were 2nd and 3rd generation oral cephalosporins and amoxicillin with or without clavulanic acid. Respiratory viruses were detected in respiratory specimens of 72.5% of patients. Antibiotic use was considered inappropriate in 90.1% and 67.5%, based on guidelines and detected pathogens, respectively. On presentation parents reported antibiotic use for 22% of patients, 41% of parents did not know and 37% denied antibiotic use. Among these three groups, six commonly used antibiotics were detected with HPLC in patients' urine in 49%, 40% and 14%, respectively. Temporary selection of 3rd generation cephalosporin resistant intestinal Enterobacteriaceae during antibiotic use was observed, with co-selection of resistance to aminoglycosides and fluoroquinolones. CONCLUSIONS: We report overuse and overprescription of antibiotics for uncomplicated ARI with selection of resistant intestinal Enterobacteriaceae, posing a risk for community transmission and persistence in a setting of a highly granular healthcare system and unrestricted access to antibiotics through private pharmacies.     Kaluza-Klein Structure Associated With Fat Brane

    It is known that the imposition of orbifold boundary conditions on background scalar field can give rise to a non-trivial vacuum expectation value (VEV) along extra dimensions, which in turn generates fat branes and associated unconventional Kaluza-Klein (KK) towers of fermions. We study the structure of these KK towers in the limit of one large extra dimension and show that normalizable (bound) states of massless and massive fermions can exist at both orbifold fixed points. Closer look however indicates that orbifold boundary conditions act to suppress at least half of bound KK modes, while periodic boundary conditions tend to drive the high-lying modes to the conventional structure. By investigating the scattering of fermions on branes, we analytically compute masses and wavefunctions of KK spectra in the presence of these boundary conditions up to one-loop level.     Threat to Cedar, Cedrela odorata, Plantations in Vietnam by the Weevil, Aclees sp.

    The recent decline and death of young cedar, Cedrela odorata L. (Sapindales: Meliaceae), plantations in Vietnam is caused by Aclees sp. (Coleoptera: Curculionidae), a wood-boring brown weevil. A field study was undertaken in three-year-old plantations in two districts in Thanh Hoa province in August 2008. Trees were heavily impacted by the weevil, Aclees; the infestation level (P) ranged from 80 to 100% and the average damage index (R) ranged from 1.8 to 2.8. Observations over one year enabled the life history to be determined. Eggs were laid (February to March, September to November) inside the bark from the base of the trunk up to 60 cm in height. Larvae formed extensive feeding tunnels in the inner bark and sap wood. Pupation occurred in feeding tunnels or pupal chambers in the sapwood. Adults emerged twice a year, February to March and August to October. It is concluded that Aclees is a threat to C. odorata plantations in tropical regions of the world, and quarantine measures should be implemented to reduce the risk of spread
