    An Inverse Method for Cracks Characterization from Ultrasonic Bscan Images

    Concern has been expressed about the capabilities of performing non destructive evaluation (NDE) of flaws located near to the outer surface in nuclear pressurized water reactor (PWR) vessels. The ultrasonic examination of PWR is accomplished from the inside with ultrasonic focused transducers working in the pulse echo mode. By recording the echoes as a function of time, the Ascan representation may be obtained. Many ultrasonic flaw detectors used for NDE are based on the simple Ascan concept involving measuring a time interval called “time of flight”. By combining the Ascan concept with synchronized transducer scanning, one can produce Bscan images that are two dimensional descriptions of the flaw interaction with the ultrasonic field

    Predictive Value of POSSUM and ACPGBI Scoring in Mortality and Morbidity of Colorectal Resection: A Case–Control Study

The aim of this study was to compare mortality and morbidity of colorectal resections performed in a tertiary referral center with mortality and morbidity as predicted with physiological and operative score for enumeration of mortality and morbidity (POSSUM), Portsmouth POSSUM (P-POSSUM), and colorectal POSSUM (CR-POSSUM). The second aim of this study was to analyze the accuracy of different POSSUM scores in surgery performed for malignancy, inflammatory bowel diseases, and diverticulitis. POSSUM scoring was also evaluated in colorectal resection in acute vs. elective setting. In procedures performed for malignancy, the Association of Coloproctology of Great Britain and Ireland (ACPGBI) score was assessed in the same way for comparison. METHODS: POSSUM, P-POSSUM, and CR-POSSUM predictor equations for mortality were applied in a retrospective case-control study to 734 patients who had undergone colorectal resection. The total group was assessed first. Second, the predictive value of outcome after surgery was assessed for malignancy (n = 386), inflammatory bowel diseases (n = 113), diverticulitis (n = 91), and other indications, e.g., trauma, endometriosis, volvulus, or ischemia (n = 144). Third, all subgroups were assessed in relation to the setting in which surgery was performed: acute or elective. In patients with malignancy, the ACPGBI score was calculated as well. In all groups, receiver operating characteristic (ROC) curves were constructed. RESULTS: POSSUM, P-POSSUM, and CR-POSSUM have a significant predictive value for outcome after colorectal surgery. Within the total population as well as in all four subgroups, there is no difference in the area under the curve between the POSSUM, P-POSSUM, and CR-POSSUM scores. In the subgroup analysis, smallest areas under the ROC curve are seen in operations performed for malignancy, which is significantly worse than for diverticulitis and in operations performed for other indications. For elective procedures, P-POSSUM and CR-POSSUM predict outcome significantly worse in patients operated for carcinoma than in patients with diverticulitis. In acute surgical interventions, CR-POSSUM predicts mortality better in diverticulitis than in patients operated for other indications. The ACPGBI score has a larger area under the curve than any of the POSSUM scores. Morbidity as predicted by POSSUM is most accurate in procedures for diverticulitis and worst when the indication is malignancy. CONCLUSION: The POSSUM scores predict outcome significantly better than can be expected by chance alone. Regarding the indication for surgery, each POSSUM score predicts outcome in patients operated for diverticulitis or other indications more accurately than for malignancy. The ACPGBI score is found to be superior to the various POSSUM scores in patients who have (elective) resection of colorectal malignancy

    Micronutrient Deficits Are Still Public Health Issues among Women and Young Children in Vietnam

    Background: The 2000 Vietnamese National Nutrition Survey showed that the population’s dietary intake had improved since 1987. However, inequalities were found in food consumption between socioeconomic groups. As no national data exist on the prevalence of micronutrient deficiencies, a survey was conducted in 2010 to assess the micronutrient status of randomly selected 1526 women of reproductive age and 586 children aged 6–75 mo. Principal Findings: In women, according to international thresholds, prevalence of zinc deficiency (ZnD, 67.262.6%) and vitamin B12 deficiency (11.761.7%) represented public health problems, whereas prevalence of anemia (11.661.0%) and iron deficiency (ID, 13.761.1%) were considered low, and folate (,3%) and vitamin A (VAD,,2%) deficiencies were considered negligible. However, many women had marginal folate (25.1%) and vitamin A status (13.6%). Moreover, overweight (BMI$23 kg/m 2 for Asian population) or underweight occurred in 20 % of women respectively highlighting the double burden of malnutrition. In children, a similar pattern was observed for ZnD (51.963.5%), anemia (9.161.4%) and ID (12.961.5%) whereas prevalence of marginal vitamin A status was also high (47.362.2%). There was a significant effect of age on anemia and ID prevalence, with the youngest age group (6–17 mo) having the highest risk for anemia, ID, ZnD and marginal vitamin A status as compared to other groups.     Mathematical surprises and Dirac's formalism in quantum mechanics

    By a series of simple examples, we illustrate how the lack of mathematical concern can readily lead to surprising mathematical contradictions in wave mechanics. The basic mathematical notions allowing for a precise formulation of the theory are then summarized and it is shown how they lead to an elucidation and deeper understanding of the aforementioned problems. After stressing the equivalence between wave mechanics and the other formulations of quantum mechanics, i.e. matrix mechanics and Dirac's abstract Hilbert space formulation, we devote the second part of our paper to the latter approach: we discuss the problems and shortcomings of this formalism as well as those of the bra and ket notation introduced by Dirac in this context.     Guidelines for diagnosis and management of the cobalamin-related remethylation disorders cblC, cblD, cblE, cblF, cblG, cblJ and MTHFR deficiency

    BACKGROUND: Remethylation defects are rare inherited disorders in which impaired remethylation of homocysteine to methionine leads to accumulation of homocysteine and perturbation of numerous methylation reactions. OBJECTIVE: To summarise clinical and biochemical characteristics of these severe disorders and to provide guidelines on diagnosis and management. DATA SOURCES: Review, evaluation and discussion of the medical literature (Medline, Cochrane databases) by a panel of experts on these rare diseases following the GRADE approach. KEY RECOMMENDATIONS: We strongly recommend measuring plasma total homocysteine in any patient presenting with the combination of neurological and/or visual and/or haematological symptoms, subacute spinal cord degeneration, atypical haemolytic uraemic syndrome or unexplained vascular thrombosis. We strongly recommend to initiate treatment with parenteral hydroxocobalamin without delay in any suspected remethylation disorder; it significantly improves survival and incidence of severe complications. We strongly recommend betaine treatment in individuals with MTHFR deficiency; it improves the outcome and prevents disease when given early

    Automatic Diagnostic of Resistance Spot Welds Using Adaptive Ultrasonic Phased Array Method

    This paper presents the results of a collaborative research program related to an efficient inspection procedure dedicated to characterizing and sizing resistance spot welds. This method allows to perform a real-time diagnostic of the resistance spot weld according to the following categories: good/stick/small/excessive indentation. The method is based on the combination of a highly resolved ultrasonic cross-section image thanks to real-time Adaptive phased array Total Focusing Method, with a dedicated decision making process. A specific phased array-probes with a flexible membrane with water coupling is placed over the spot weld, and the ATFM technique automatically measures the exact, uneven shape of the part and provides an accurate TFM image of the cross-section of the weld (see Figure 1). This image is automatically processed and analyzed. These algorithms have been implemented in portable phased-array ultrasonic system (Gekko, by M2M), and experimental tests over a large series of good and bad samples have been carried out (by Renault), to evaluate the performances of this method. The principle and implementation of the method, as well as examples of images and diagnostics associated to various samples will be discussed in this paper.</p
