Recent progress in translational research on neurovascular and neurodegenerative disorders
The already established and widely used intravenous application of recombinant tissue plasminogen activator as a re-opening strategy for acute vessel occlusion in ischemic stroke was recently added by mechanical thrombectomy, representing a fundamental progress in evidence-based medicine to improve the patientâs outcome. This has been paralleled by a swift increase in our understanding of pathomechanisms underlying many neurovascular diseases and most prevalent forms of dementia. Taken together, these current advances offer the potential to overcome almost two decades of marginally successful translational research on stroke and dementia, thereby spurring the entire field of translational neuroscience. Moreover, they may also pave the way for the renaissance of classical neuroprotective paradigms.
This review reports and summarizes some of the most interesting and promising recent achievements in neurovascular and dementia research. It highlights sessions from the 9th International Symposium on Neuroprotection and Neurorepair that have been discussed from April 19th to 22nd in Leipzig, Germany. To acknowledge the emerging culture of interdisciplinary collaboration and research, special emphasis is given on translational stories ranging from fundamental research on neurode- and -regeneration to late stage translational or early stage clinical investigations
Suture-based vs. pure plug-based vascular closure devices for VA-ECMO decannulationâA retrospective observational study
BackgroundVeno-arterial extracorporeal membrane oxygenation (VA-ECMO) is a valuable treatment option for patients in cardiogenic shock, but complications during decannulation may worsen the overall outcome. Therefore, the aim of this study was to compare the efficacy and safety of suture-based to pure plug-based vascular closure devices for VA-ECMO decannulation.MethodsIn this retrospective study, the procedural outcome of 33 patients with suture-based Perclose ProGlide closure devices was compared to 38 patients with MANTA plug-based closure devices.ResultsRate of technically correct placement of closure devices was 88% in the suture-based group and 97% in the plug-based group (p = 0.27). There was a significant reduction of severe bleeding events during VA-ECMO decannulation in plug-based versus suture-based systems (3% vs. 21%, p = 0.04). Ischemic complications occurred in 6% with suture-based and 5% with plug-based device (p = 1.00). Pseudoaneurysm formation was detected in 3% in both groups (p = 1.00). No switch to vascular surgery due to bleeding after decannulation was necessary in both groups.ConclusionBased on our retrospective analysis, we propose that plug-based vascular closure should be the preferred option for VA-ECMO decannulation. This hypothesis should be further tested in a randomized trial
Antibody-mediated procoagulant platelets in SARS-CoV-2-vaccination associated immune thrombotic thrombocytopenia
The COVID-19 pandemic has resulted in significant morbidity and mortality worldwide. To prevent severe infection, mass COVID-19 vaccination campaigns with several vaccine types are currently underway. We report pathological and immunological findings in 8 patients who developed vaccine-induced immune thrombotic thrombocytopenia (VITT) after administration of SARS-CoV-2 vaccine ChAdOx1 nCoV-19. We analyzed patient material using enzyme immune assays, flow cytometry and heparin-induced platelet aggregation assay and performed autopsies on two fatal cases. Eight patients (5 female, 3 male) with a median age of 41.5 years (range, 24 to 53) were referred to us with suspected thrombotic complications 6 to 20 days after ChAdOx1 nCoV-19 vaccination. All patients had thrombocytopenia at admission. Patients had a median platelet count of 46.5 x109/L (range, 8 to 92). Three had a fatal outcome and 5 were successfully treated. Autopsies showed arterial and venous thromboses in various organs and the occlusion of glomerular capillaries by hyaline thrombi. Sera from VITT patients contain high titer antibodies against platelet factor 4 (PF4) (OD 2.59±0.64). PF4 antibodies in VITT patients induced significant increase in procoagulant markers (P-selectin and phosphatidylserine externalization) compared to healthy volunteers and healthy vaccinated volunteers. The generation of procoagulant platelets was PF4 and heparin dependent. We demonstrate the contribution of antibody-mediated platelet activation in the pathogenesis of VITT

Aircraft type influence on contrail properties
The investigation of the impact of aircraft parameters on contrail properties helps to better understand the climate impact from aviation. Yet, in observations, it is a challenge to separate aircraft and meteorological influences on contrail formation. During the CONCERT campaign in November 2008, contrails from 3 Airbus passenger aircraft of type A319-100, A340-300 and A380-800 were probed at cruise under similar meteorological conditions with in-situ instruments on board the DLR research aircraft Falcon
Aircraft emissions at cruise and plume processes
The detection of aircraft emissions at cruise altitudes helps to understand and assess the effects of aviation on atmospheric composition and climate. Since the early 1990s, aircraft emissions of carbon dioxide, water vapor, nitrogen and sulfur oxides, aerosol and soot and their processing in the atmosphere as well as contrail formation have been measured in-situ with the instrumented DLR research aircraft Falcon. Scientific results from a series of aircraft missions are summarized and explained, uncertainties are discussed and suggestions are made on how to move forward
Has the aircraft type an impact on the microphysical parameters of young contrails?
Modeling studies suggest an impact of the aircraft type on the microphysical and optical properties of young contrails. However, up to now, a low intercomparability of meteorological conditions has prevented a study on the aircraft effect on contrail properties using in-situ observations.
Therefore, we analyze contrail observations from 3 different aircraft, a large A380, a medium sized A340 and a smaller A319, performed during the CONCERT2008 (CONtrail and Cirrus ExpeRimenT) campaign under similar meteorological conditions in terms of relative humidity over ice (RHi) and wind shear. The young contrails were probed on 19 November 2008 over Northern Germany with instruments onboard the DLR research aircraft Falcon. 1-min old contrails were sampled at an altitude of 10.6 km near ice saturation at temperatures of 217 to 218 K above a natural cirrus cloud layer. We find large differences in the 3 observed contrail optical depth distributions, while the aircraft impact is not as pronounced in the detected particle size spectra.
In addition, we simulate the contrail properties from the 3 aircraft with two independent models, the Eulerian particle tracking EULAG-LCM model and the contrail and cirrus prediction tool CoCiP. The models generally confirm the observed trend, an increase in contrail optical depth with increasing aircraft size (or weight). However, the models show smaller contrail optical depths for all aircraft types than observations. These discrepancies are reduced assuming higher values for the ambient relative humidity
Detection of young contrails â selected results from the CONCERT (CONtrail and Cirrus ExpeRimenT) campaign
Large uncertainties remain in estimating the climate impact from contrails. In particular it is unknown, whether the aircraft type has an influence on contrail properties. Therefore, microphysical and radiative properties of contrails were detected with the DLR research aircraft Falcon during the CONCERT campaign in October/ November 2008. During 12 mission flights over Western Europe 22 contrails from 11 different aircraft were probed and the ice particle number
density, size, extinction, contrail dimension as well as trace gas fields were measured. Here we focus on the 14 minutes sampling of the contrail of an A319. The 1 to 3 min old contrail was detected in the vortex regime. It was observed at an altitude of 10.6km and a temperature of 216K in ice subsaturated air (82%<RHI<98%). Particle concentration, extinction, and ice water content decrease within the sampling period due to contrail ageing and dilution. A vertical contrail depth of 122m has been estimated from the measurements and agrees with vortex descent simulations. Micro- and macro-physical contrail observations allow for the quantification of the contrail optical depth, playing a crucial role for the estimate of contrail radiative forcing
