Eenvoudige elleboogluxaties in Nederland: Wat doen Nederlandse chirurgen?
Abstract:
Er is weinig evidence voor de optimale behandeling van eenvoudige elleboogluxaties. Hoewel er aanwijzingen zijn dat eenvoudige elleboogluxaties niet te lang moeten worden geïmmobiliseerd en functioneel kunnen worden nabehandeld. Om een overzicht te krijgen over hoe eenvoudige elleboogluxaties in Nederland worden behandeld, verrichtten wij een enquête onder leden van de Nederlandse Vereniging van Traumatologie.
Alle leden ontvingen een e-mail met het verzoek om een digitale vragenlijst over de behandeling van elleboogluxaties in te vullen. Het response percentage was 17% (n=90). Vijfendertig (39%) chirurgen verbinden geen consequenties aan stabiliteitsonderzoek van de elleboog na repositie. Drieënzestig procent van deze chirurgen behandelt de patiënten met een gipsverband gedurende gemiddeld 3.4 weken. Bij 55 (61%) van de 90 respondenten beïnvloed het stabiliteitsonderzoek wel de vorm van behandeling. Bij een stabiel gewricht wordt in ongeveer 64% van de gevallen een functionele behandeling voorgeschreven en bij een instabiel gewricht behandelen 24% van de respondenten functioneel met een functionele fixateur. Background: Randomised controlled trials and observational comparative studies show little proof for the optimal form of treatment for simple elbow dislocations. However, there is some evidence that functional treatment in simple elbow dislocations may lead to better functional outcome. To get an overview of how simple elbow dislocations are treated in the Netherlands we performed a survey among Dutch surgeons.
Methods: All members of the Dutch Society of Traumatology received an e-mail with the request to fill out an electronic questionnaire.
Results: The response rate was 17% (n=90) . Thirty vive (39%) of them stated that stability testing after reposition of the joint did not influence subsequent form of treatment. Sixty three percent of them treated patients with a above the elbow cast for a mean of 3.4 weeks. Fifty five (61%) responders used stability testing as a guide for treatment. joint is stable after reposition. In the case of a stabile joint 64% of these surgeons treat patients with functional treatment. When there is joint instability 24% prefer functional treatment with a hinged external fixator.
Conclusion: The results of this inquiry among Dutch surgeons showed that the majority of simple elbow dislocations are treated with immobilisation and that functional treatment is not widespread
Exercise-Induced splanchnic hypoperfusion results in gut dysfunction in healthy men
Background Splanchnic hypoperfusion is common in various pathophysiological conditions and often considered to lead to gut dysfunction. While it is known that physiological situations such as physical exercise also result in splanchnic hypoperfusion, the consequences of flow redistribution at the expense of abdominal organs remained to be determined. This study focuses on the effects of splanchnic hypoperfusion on the gut, and the relationship between hypoperfusion, intestinal injury and permeability during physical exercise in healthy men. Methods and Findings Healthy men cycled for 60 minutes at 70% of maximum workload capacity. Splanchnic hypoperfusion was assessed using gastric tonometry. Blood, sampled every 10 minutes, was analyzed for enterocyte damage parameters (intestinal fatty acid binding protein (I-FABP) and ileal bile acid binding protein (I-BABP)). Changes in intestinal permeability were assessed using sugar probes. Furthermore, liver and renal parameters were assessed. Splanchnic perfusion rapidly decreased during exercise, reflected by increased gapg-apCO2 from −0.85±0.15 to 0.85±0.42 kPa (p < 0.001). Hypoperfusion increased plasma I-FABP (615±118 vs. 309±46 pg/ml, p < 0.001) and I-BABP (14.30±2.20 vs. 5.06±1.27 ng/ml, p < 0.001), and hypoperfusion correlated significantly with this small intestinal damage (rS = 0.59; p < 0.001). Last of all, plasma analysis revealed an increase in small intestinal permeability after exercise (p < 0.001), which correlated with intestinal injury (rS = 0.50; p < 0.001). Liver parameters, but not renal parameters were elevated. Conclusions Exercise-induced splanchnic hypoperfusion results in quantifiable small intestinal injury. Importantly, the extent of intestinal injury correlates with transiently increased small intestinal permeability, indicating gut barrier dysfunction in healthy individuals. These physiological observations increase our knowledge of splanchnic hypoperfusion sequelae, and may help to understand and prevent these phenomena in patients
Exploiting the social and semantic web for guided web archiving
The constantly growing amount of Web content and the success of the Social Web lead to increasing needs for Web archiving. These needs go beyond the pure preservation of Web pages. Web archives are turning into "community memories" that aim at building a better understanding of the public view on, e.g., celebrities, court decisions, and other events. In this paper we present the ARCOMEM architecture that uses semantic information such as entities, topics, and events complemented with information from the social Web to guide a novel Web crawler. The resulting archives are automatically enriched with semantic meta-information to ease the access and allow retrieval based on conditions that involve high-level concepts. Comprehensive study on Escherichia coli genomic expression : does position really matter?
As a biorefinery platform host, Escherichia coli has been used extensively to produce metabolites of commercial interest. Integration of foreign DNA onto the bacterial genome allows for stable expression overcoming the need for plasmid expression and its associated instability. Despite the development of numerous tools and genome editing technologies, the question of where to incorporate a synthetic pathway remains unanswered. To address this issue, we studied the genomic expression in E. coli and linked it not only to 26 rationally selected genomic locations, but also to the gene direction in relation to the DNA replication fork, to the carbon and nitrogen source, to DNA folding and supercoiling, and to metabolic burden. To enable these experiments, we have designed a fluorescent expression cassette to eliminate specific local effects on gene expression. Overall it can be concluded that although the expression range obtained by changing the genomic location of a pathway is small compared to the range typically seen in promoter-RBS libraries, the effect of culture medium, environmental stress and metabolic burden can be substantial. The characterization of multiple effects on genomic expression, and the associated libraries of well-characterized strains, will only stimulate and improve the creation of stable production hosts fit for industrial settings
D2.1 Report on analysis of the requirements for MONOCLE sensors including projection of cost-savings and stakeholder feedback. Deliverable report of project H2020 MONOCLE (grant 776480)
Requirements for MONOCLE sensors were analysed at the start of sensor development, particularly with regard to projected cost-savings in monitoring and specific stakeholder feedback. The main inputs from stakeholders were obtained from the MONOCLE water quality monitoring survey (D9.1) and are used here to define sensor-specific development priorities, particularly with respect to purpose, performance, cost and interoperability. This document guides both the initial development of new sensors and evolution of existing prototypes to higher technological readiness levels
Peyer\u27s patch M cells derived from Lgr5\u3csup\u3e+\u3c/sup\u3e stem cells require SpiB and are induced by RankL in cultured miniguts
Peyer\u27s patches consist of domains of specialized intestinal epithelium overlying gut-associated lymphoid tissue (GALT). Luminal antigens reach the GALT by translocation through epithelial gatekeeper cells, the so-calledMcells. We recently demonstrated that all epithelial cells required for the digestive functions of the intestine are generated from Lgr5-expressing stem cells. Here, we show thatMcells also derive from these crypt-based Lgr5 stem cells. The Ets family transcription factor SpiB, known to control effector functions of bone marrow-derived immune cells, is specifically expressed inMcells. In SpiB-/-mice, Mcells are entirely absent, which occurs in a cell-autonomous fashion. It has been shown that Tnfsf11 (RankL) can induceMcell development in vivo. We show that in intestinal organoid ( minigut ) cultures, stimulation with RankL induces SpiB expression within 24 h and expression of otherMcell markers subsequently. We conclude that RankL-induced expression of SpiB is essential for Lgr5 stem cell-derived epithelial precursors to develop intoMcells. © 2012, American Society for Microbiology
