    Lifetime Adherence to Physical Activity Recommendations and Fall Occurrence in Community-dwelling Older Adults: a Retrospective Cohort Study

    Falling is a major health concern for community-dwelling older adults. Regular physical activity has been proposed to prevent falls. The aim of this study was to assess whether the achievement of the 2004 UK Department of Health physical activity recommendations over a lifetime had a protective effect against falling in older people. 313 community-dwelling older adults completed a questionnaire about lifetime physical activity and fall occurrence. There were significantly fewer falls in those who had led an active lifestyle compared to those who had not (χ2Yates=4.568, p=0.033), with a lower relative risk of fall occurrence for the active respondents (RR=0.671) compared to the inactive (RR=1.210). Of those who were sufficiently active in their early adulthood, the decade where there was the biggest decrease in remaining active enough was in the 60s.     Generation of Hyperentangled Photons Pairs

    We experimentally demonstrate the first quantum system entangled in every degree of freedom (hyperentangled). Using pairs of photons produced in spontaneous parametric downconversion, we verify entanglement by observing a Bell-type inequality violation in each degree of freedom: polarization, spatial mode and time-energy. We also produce and characterize maximally hyperentangled states and novel states simultaneously exhibiting both quantum and classical correlations. Finally, we report the tomography of a 2x2x3x3 system (36-dimensional Hilbert space), which we believe is the first reported photonic entangled system of this size to be so characterized.Comment: 5 pages, 3 figures, 1 table, published versio

    Improving dialysis adherence for high risk patients using automated messaging: Proof of concept

    AbstractComorbidities and socioeconomic barriers often limit patient adherence and self-management with hemodialysis. Missed sessions, often associated with communication barriers, can result in emergency dialysis and avoidable hospitalizations. This proof of concept study explored using a novel digital-messaging platform, EpxDialysis, to improve patient-to-dialysis center communication via widely available text messaging and telephone technology. A randomized controlled trial was conducted through Washington University-affiliated hemodialysis centers involving ESRD patients with poor attendance, defined as missing 2–6 sessions over the preceding 12 weeks. A cross-over study design evaluated appointment adherence between intervention and control groups. Comparing nonadherence rates eight weeks prior to enrollment, median appointment adherence after using the system increased by 75%, and median number of unintended hospitalization days fell by 31%. A conservative cost-benefit analysis of EpxDialysis demonstrates a 1:36 savings ratio from appointment adherence. EpxDialysis is a low-risk, cost-effective, intervention for increasing hemodialysis adherence in high-risk patients, especially at centers caring for vulnerable and low-income patients.</jats:p

    Mathematical modelling of ethanol metabolism in normal subjects and chronic alcohol misusers

    The time course of ethanol disappearance from the blood has been examined in normal males and females and in alcohol misusers. Blood alcohol estimations were made over a period of 3 hr, following an oral dose of ethanol (0.8 g/kg body weight) administered in the form of whisky. Attempts were made to fit the data to zero order, first order and mixed zero + first order kinetics. In the majority (75%) of normal females the blood ethanol concentration was still increasing at 30 min. This was only seen in 50% of normal males and in 50% of non-dependent alcohol misusers, but not in dependent alcohol misusers. In all of the normal females the disappearance of ethanol could be adequately described by zero order kinetics. However, in the normal male group only 20% could be described by zero order kinetics, 10% fitted first order kinetics and the remainder required a mixed model of zero + first order. The rate constant for the zero order component of the control male group was identical to zero order rate constant obtained for the female control group. In the female alcohol misuser group, 40% of the curves could not be described by zero order kinetics and fitted best to a mixed model. The zero order component of the entire group was significantly increased (by 35%) compared to that obtained for the female control group. In the male dependent and non-dependent alcohol misuser groups, all blood alcohol concentration curves fitted best to mixed zero and first order kinetics. However, no significant differences were noted in the values of the kinetic parameters when compared with the male control group. It is suggested that the zero order component of the blood alcohol concentration curves is due to the action of liver alcohol dehydrogenase and the first order component represents redistribution to the tissues.     Laser induced electron diffraction: a tool for molecular orbital imaging

    We explore the laser-induced ionization dynamics of N2 and CO2 molecules subjected to a few-cycle, linearly polarized, 800\,nm laser pulse using effective two-dimensional single active electron time-dependent quantum simulations. We show that the electron recollision process taking place after an initial tunnel ionization stage results in quantum interference patterns in the energy resolved photo-electron signals. If the molecule is initially aligned perpendicular to the field polarization, the position and relative heights of the associated fringes can be related to the molecular geometrical and orbital structure, using a simple inversion algorithm which takes into account the symmetry of the initial molecular orbital from which the ionized electron is produced. We show that it is possible to extract inter-atomic distances in the molecule from an averaged photon-electron signal with an accuracy of a few percents
