    Severe hepatic and pulmonary involvement in Rendu-Osler-Weber syndrome

    We report the case of a young woman with hereditary hemorrhagic telangiectasia (HHT) with severe liver involvement and pulmonary shunting. The medical imaging in this patient illustrates the severe shunting that can occur in these patients who often are asymptomatic. By showing this case, we want to highlight the role of liver transplantation in HHT with hepatic involvement

    International audienceLeonardo (1968-present), published by MIT Press, is the leading international peer-reviewed publication on the relationship between art, science and technology, making it an ideal dataset to analyze the emergence of such complex collaborations over time. To identify and analyze both the visible and latent interaction patterns, the research employs different granularities of data (article texts, images, publication dates, authors, their places of affiliation and disciplines) as part of a multimodal approach. Using a convolutional neural network, we examined the features of the images to analyze the modes of representing (and actually doing) art, science or engineering. We paired these features with information extracted using text mining to examine the relationships between the visual and the textual over time

    Local control of hepatocellular carcinoma and colorectal liver metastases after surgical microwave ablation without concomitant hepatectomy

    Purpose Microwave ablation (MWA) is an accepted technique in the multimodal treatment of hepatocellular carcinoma (HCC) and colorectal liver metastases (CRLM). Study endpoints were to evaluate the local efficacy of surgical MWA in selected patients with oligonodular disease without the combination of liver resection to allow a clear interpretation of the follow-up imaging and compare it to the results on percutaneous MWA available in the literature. Methods Consecutive MWA-only procedures performed between May 2013 and May 2018 for HCC and CRLM with free-hand ultrasound guidance were identified. MWA systems with 2450 MHz were used. Incomplete ablation (IA) was defined as residual disease within 1 cm of the ablation site at the first post-ablation imaging and local recurrence (LR) as the presence of disease after at least one tumor-free imaging. Results A total of 70 tumors in 47 patients were treated with 46 laparoscopic and 1 open procedures. Each patient had no more than 3 tumors, and median size of the lesions was 15 mm (IQR: 10-22). After a median follow-up of 26 months (IQR: 12-40), IA rate was 8.6% and LR rate was 29.4%. Multivariable analysis showed that vascular proximity (OR = 3.4; 95% CI = 1.26-9.22; p=0.016) was the only significant predictor of the combined outcome IA or LR. Discussion In the present study, after mostly laparoscopic MWA, LR was higher than the rates available in the literature for percutaneous MWA of HCC but lower than in the limited studies analyzing isolated percutaneous MWA of liver metastases. Future developments may help establish the role of each therapeutic modality per tumor, in order to improve the outcomes

    Global Disease Burden Estimates of Respiratory Syncytial Virus–Associated Acute Respiratory Infection in Older Adults in 2015::A Systematic Review and Meta-Analysis

    Respiratory syncytial virus associated acute respiratory infection (RSV-ARI)constitutes a substantial disease burden in older adults≥65 years. We aimed to identify all studies worldwide investigating the disease burden ofRSV-ARIin this population. We estimated thecommunityincidence, hospitalisationrate and in-hospital case fatality ratio (hCFR) of RSV-ARI in older adults stratified by industrialized anddeveloping regions, with data from a systematic review ofstudies published between January 1996 and April 2018, and from 8 unpublished population-based studies. We applied these rate estimates to population estimates for 2015, to calculate the global and regional burdenin older adults with RSV-ARIin community and in hospital duringthat year. We estimated thenumber ofin-hospital RSV-ARIdeaths by combining hCFR with hospital admission estimates from hospital-based studies. In 2015, there were about 1.5million(95% CI 0.3-6.9) episodes of RSV-ARIin older adults in41industrialised countries (data missing in developing countries), and of these 214,000 (~14.5%; 95% CI 100,000-459,000) were admitted to hospitals. The global number of hospital admissionsforRSV-ARI in older adults was estimated at 336,000 (UR 186,000-614,000).We further estimated about 14,000 (UR 5,000-50,000) in-hospital deaths related to RSV-ARIglobally.The hospital admission rate and hCFR were higher for those ≥65 years than those aged 50-64 years. The disease burden of RSV-ARIamong older adults is substantialwith limited data from developing countries; appropriate prevention and management strategiesare needed to reduce this burden

    Stratospheric aerosol radiative forcing simulated by the chemistry climate model EMAC using Aerosol CCI satellite data

    This paper presents decadal simulations of stratospheric and tropospheric aerosol and its radiative effects by the chemistry general circulation model EMAC constrained with satellite observations in the framework of the ESA Aerosol CCI project such as GOMOS (Global Ozone Monitoring by Occultation of Stars) and (A)ATSR ((Advanced) Along Track Scanning Radiometer) on the ENVISAT (European Environmental Satellite), IASI (Infrared Atmospheric Sounding Interferometer) on MetOp (Meteorological Operational Satellite), and, additionally, OSIRIS (Optical Spectrograph and InfraRed Imaging System). In contrast to most other studies, the extinctions and optical depths from the model are compared to the observations at the original wavelengths of the satellite instruments covering the range from the UV (ultraviolet) to terrestrial IR (infrared). This avoids conversion artifacts and provides additional constraints for model aerosol and interpretation of the observations. MIPAS (Michelson Interferometer for Passive Atmospheric Sounding) SO2 limb measurements are used to identify plumes of more than 200 volcanic eruptions. These three-dimensional SO2 plumes are added to the model SO2 at the eruption times. The interannual variability in aerosol extinction in the lower stratosphere, and of stratospheric aerosol radiative forcing at the tropopause, is dominated by the volcanoes. To explain the seasonal cycle of the GOMOS and OSIRIS observations, desert dust simulated by a new approach and transported to the lowermost stratosphere by the Asian summer monsoon and tropical convection turns out to be essential. This also applies to the radiative heating by aerosol in the lowermost stratosphere. The existence of wet dust aerosol in the lowermost stratosphere is indicated by the patterns of the wavelength dependence of extinction in observations and simulations. Additional comparison with (A)ATSR total aerosol optical depth at different wavelengths and IASI dust optical depth demonstrates that the model is able to represent stratospheric as well as tropospheric aerosol consistently.SCOPUS: ar.jinfo:eu-repo/semantics/publishe

Genetic diversity (diversity within species) underlies species’ adaptation and survival, ecosystem resilience, and societal innovation. The low priority given to genetic diversity has largely been due to knowledge gaps in key areas, including the importance of genetic diversity and the trends in genetic diversity change; the perceived high expense and low availability and the scattered nature of genetic data; and complicated concepts and information that are inaccessible to policymakers. However, numerous recent advances in knowledge, technology, databases, practice, and capacity have now set the stage for better integration of genetic diversity in policy instruments and conservation efforts. We review these developments and explore how they can support improved consideration of genetic diversity in global conservation policy commitments and enable countries to monitor, report on, and take action to maintain or restore genetic diversity
