Health-related quality of life in French adolescents and adults: norms for the DUKE Health Profile
<p>Abstract</p> <p>Background</p> <p>The continual monitoring of population health-related quality of life (HRQoL) with validated instruments helps public health agencies assess, protect, and promote population health. This study aimed to determine norms for the French adolescent and adult general population for the Duke Health Profile (DUKE) questionnaire in a large representative community sample.</p> <p>Methods</p> <p>We randomly selected 17,733 French people aged 12 to 75 years old in 2 steps, by households and individuals, from the National Health Barometer 2005, a periodic population study by the French National Institute for Prevention and Health Education. Quality of life and other data were collected by computer-assisted telephone interview.</p> <p>Results</p> <p>Normative data for the French population were analyzed by age, gender and self-reported chronic disease. Globally, function scores (best HRQoL=100) for physical, mental, social, and general health, as well as perceived health and self-esteem, were 72.3 (SEM 0.2), 74.6 (0.2), 66.8 (0.1), 71.3 (0.1), 71.3 (0.3), 76.5 (0.1), respectively. Dysfunction scores (worst HRQoL=100) for anxiety, depression, pain and disability domains were 30.9 (0.1), 27.6 (0.2), 34.3 (0.3), 3.1 (0.1), respectively.</p> <p>Conclusion</p> <p>The French norms for adolescents and adults for the DUKE could be used as a reference for other studies assessing HRQoL, for specific illnesses, in France and for international comparisons.</p
Differential effects of glucagon-like peptide-1 receptor agonists on heart rate
Abstract
While glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are known to increase heart rate (HR), it is insufficiently recognized that the extent varies greatly between the various agonists and is affected by the assessment methods employed. Here we review published data from 24-h time-averaged HR monitoring in healthy individuals and subjects with type 2 diabetes mellitus (T2DM) treated with either short-acting GLP-1 RAs, lixisenatide or exenatide, or long-acting GLP-1 RAs, exenatide LAR, liraglutide, albiglutide, or dulaglutide (N\ua0=\ua01112; active-treatment arms). HR effects observed in two independent head-to-head trials of lixisenatide and liraglutide (N\ua0=\ua0202; active-treatment arms) are also reviewed. Short-acting GLP-1 RAs, exenatide and lixisenatide, are associated with a transient (1\u201312\ua0h) mean placebo- and baseline-adjusted 24-h HR increase of 1\u20133\ua0beats per minute (bpm). Conversely, long-acting GLP-1 RAs are associated with more pronounced increases in mean 24-h HR; the highest seen with liraglutide and albiglutide at 6\u201310\ua0bpm compared with dulaglutide and exenatide LAR at 3\u20134\ua0bpm. For both liraglutide and dulaglutide, HR increases were recorded during both the day and at night. In two head-to-head comparisons, a small, transient mean increase in HR from baseline was observed with lixisenatide; liraglutide induced a substantially greater increase that remained significantly elevated over 24\ua0h. The underlying mechanism for increased HR remains to be elucidated; however, it could be related to a direct effect at the sinus node and/or stimulation of the sympathetic nervous system, with this effect related to the duration of action of the respective GLP-1 RAs. In conclusion, this review indicates that the effects on HR differ within the class of GLP-1 RAs: short-acting GLP-1 RAs are associated with a modest and transient HR increase before returning to baseline levels, while some long-acting GLP-1 RAs are associated with a more pronounced and sustained increase during the day and night. Findings from recently completed trials indicate that a GLP-1 RA-induced increase in HR, regardless of magnitude, does not present an increased cardiovascular risk for subjects with T2DM, although a pronounced increase in HR may be associated with adverse clinical outcomes in those with advanced heart failure
Multidifferential study of identified charged hadron distributions in -tagged jets in proton-proton collisions at 13 TeV
Jet fragmentation functions are measured for the first time in proton-proton
collisions for charged pions, kaons, and protons within jets recoiling against
a boson. The charged-hadron distributions are studied longitudinally and
transversely to the jet direction for jets with transverse momentum 20 GeV and in the pseudorapidity range . The
data sample was collected with the LHCb experiment at a center-of-mass energy
of 13 TeV, corresponding to an integrated luminosity of 1.64 fb. Triple
differential distributions as a function of the hadron longitudinal momentum
fraction, hadron transverse momentum, and jet transverse momentum are also
measured for the first time. This helps constrain transverse-momentum-dependent
fragmentation functions. Differences in the shapes and magnitudes of the
measured distributions for the different hadron species provide insights into
the hadronization process for jets predominantly initiated by light quarks.Comment: All figures and tables, along with machine-readable versions and any

Study of the decay
The decay is studied
in proton-proton collisions at a center-of-mass energy of TeV
using data corresponding to an integrated luminosity of 5
collected by the LHCb experiment. In the system, the
state observed at the BaBar and Belle experiments is
resolved into two narrower states, and ,
whose masses and widths are measured to be where the first uncertainties are statistical and the second
systematic. The results are consistent with a previous LHCb measurement using a
prompt sample. Evidence of a new
state is found with a local significance of , whose mass and width
are measured to be and , respectively. In addition, evidence of a new decay mode
is found with a significance of
. The relative branching fraction of with respect to the
decay is measured to be , where the first
uncertainty is statistical, the second systematic and the third originates from

sample of proton-proton collision data corresponding to 3.0 fb of
integrated luminosity recorded by the LHCb experiment during 2011 and 2012. The
tau lepton is identified in the decay mode
. The measured values are

, where the first uncertainty is
statistical and the second is systematic. The correlation between these
measurements is . Results are consistent with the current average
of these quantities and are at a combined 1.9 standard deviations from the

Etude des facteurs influençant l'activité anti-Xa chez les sujets âgés (> 65 ans) traités par énoxaparine en prévention médicale d'une thrombose veineuse profonde
L'AFSSAPS a ajouté en 2000 une contre-indication pour l'utilisation des héparines de bas poids moléculaire (HBPM) à doses curatives en cas d'insuffisance rénale sévère. Des précautions d'emploi sont recommandées en cas d'utilisation des HBPM à doses préventives en cas d'insuffisance rénale. Les accidents hémorragiques graves observés étaient entre autre liés à l'administration des HBPM à des patients âgés (> 75 ans), en cas d'insuffisance rénale ou dans le cadre de traitements à doses curatives prolongés. Toutefois, à ce jour, aucune étude clinique n'a trouvé de relation entre l'activité anti-Xa plasmatique (meilleur reflet biologique actuel du risque hémorragique des HBPM) et la clairance de la créatinine chez les sujets âgés hospitalisés traités par HBPM en prévention d'une thrombose veineuse profonde ou d'une embolie pulmonaire (TVP/EP). La seule étude disponible allant actuellement en ce sens est limitée du fait du faible nombre de patients évalués et de leur fonction rénale encore relativement préservée. Dans notre travail; une étude prospective d'évaluation épidémiologique, monocentrique, sans bénéfice individuel direct ; nous recherchons les possibles facteurs influençant l'activité anti-Xa plasmatique mesurée chez les sujets âgés (>65 ans) hospitalisés en service de court séjour gériatrique et recevant 4000 unités d'énoxaparine en prévention d'une TVP/EP. Les facteurs étudiés sont l'âge, le poids, la clairance de la créatinine évaluée selon deux méthodes (Cockcroft et MDRD) et l'état nutritionnel. L'activité anti-Xa plasmatique est le critère de jugement de l'étude. L'analyse préliminaire des résultats des 15 premiers patients de notre étude encore en cours qui doit en comporter au total 123 laisse supposer qu'il pourrait y avoir un lien entre l'activité anti-Xa plasmatique d'une part, l'âge et l'IMC d'autre part. Qualité de vie, bénéfices et risques des traitements antagonistes de la vitamine K dans la fibrillation atriale chez les sujets âgés
Les antagonistes de la vitamine K (AVK) sont indiqués dans la fibrillation atriale (FA) chez les sujets âgés, mais leur utilisation pose des problèmes dans cette population qui est exposée à un risque hémorragique accru et où ils sont sous-utilisés. Afin d'améliorer l'adhérence aux guidelines, il est nécessaire d'améliorer le rapport bénéfice-risque des AVK chez les patients âgés. La Qualité de Vie liée à la santé (QDV) a toute sa place dans l'évaluation du bénéfice et des inconvénients des AVK chez les patients âgés et peut être un des déterminants du choix de traitement. Une meilleure perception de la santé pourrait également contribuer à une meilleure adhérence au traitement AVK chez ces patients. Nous avons montré dans une méta-analyse de la littérature que l'utilisation d'une posologie très faible d'AVK (INR <= 1,6), comparée à une posologie ajustée (INR 2-3), ne diminuait pas le risque d'hémorragie alors qu'elle augmentait le risque de thrombose dans la fibrillation atriale, toutefois ce travail n'a pu étudier séparément les patients très âgés. Nous nous sommes attachés parallèlement à déterminer quel (quels) instrument(s) pourrait(ent) être le(s) mieux à même d'évaluer la QDV chez les patients âgés, ayant une FA et/ou sous AVK. Nous avons comparé l'utilisation du Duke Health Profile et du SF-36 chez des patients âgés de plus de 65 ans et comparé la QDV de patients âgés ayant une FA à des témoins appariés avec ces deux questionnaires. Le SF-36 est un outil utilisable chez des patients âgés chez lesquels il semble présenter de meilleures qualités psychométriques, toutefois c'est un questionnaire plus court, mieux rempli et qui semble être intéressant chez les patients âgés ayant une FA pour son approche plus développée de la dimension mentale. Ce travail est poursuivi, avec en particulier l'utilisation d'outils peut-être mieux ciblés d'une part sur la personne âgée (comme le Whoqol-old élaboré par l'OMS), d'autre part sur les anticoagulants oraux.NANCY1-SCD Medecine (545472101) / SudocSudocFranceF
Corrélation entre la pression artérielle périphérique et la vitesse d'onde de pouls chez des sujets de plus de 80 ans institutionnalisés
Rationnel : La vitesse d'onde de pouls (VOP) est une méthode non invasive d'estimation de la rigidité artérielle. Les recommandations internationales établissent que la VOP est un marqueur puissant du risque cardiovasculaire (CV). La corrélation entre la pression artérielle périphérique et la VOP et leurs influences respectives sur la mortalité ont été peu étudiés chez les sujets âgés. Notre objectif était d'analyser cette corrélation chez les sujets institutionnalisés de plus de 80 ans. Méthodes : PARTAGE (valeur prédictive de la pression artérielle et de la rigidité artérielle chez institutionnalisé la population très âgée) est une étude de cohorte suivant pendant 2 ans 1130 sujets de plus de 80 ans institutionnalisés dans 72 centres en France et en Italie. La corrélation entre pression artérielle systolique et diastolique (PAS et la PAD) et la VOP (mesuré avec un tonomètre PulsePen ®) a été étudié chez 1071 sujets ayant des données de VOP. Résultats : La corrélation entre la PA et la VOP dans notre étude est significative mais faible. Les coefficients de corrélation sont de 0,24 pour la corrélation PAS clinique-VOP, 0,26 pour la corrélation PP-VOP, et 0,30 pour la corrélation PA automesure-VOP. La corrélation est systématiquement plus élevée chez les femmes mais sans atteindre la significativité. Le niveau de corrélation est inversement proportionnel à l'âge : les corrélations les plus fortes sont retrouvées dans la population la plus jeune. Le traitement antihypertenseur n'a pas d'impact sur la corrélation. Conclusion : La faiblesse de la corrélation montre que la PA et la VOP expriment différents phénomènes physiopathologiques de la rigidité artérielle. L'analyse longitudinale de l'étude PARTAGE, mise en perspective avec ceux trouvés dans nos travaux, pourraient permettre de proposer la VOP comme une méthode complémentaire, voire alternative, à la mesure de la PA dans l'évaluation du risque CV dans la population des sujets très âgésBackground: Carotid-femoral pulse wave velocity (PWV) provides a comprehensive non-invasive assessment of arterial stiffness. PWV is now established as a strong marker of cardiovascular disease. The correlation between peripheral blood pressure and PWV and their respective influences on mortality have been poorly studied in the elderly. Our objective was to analyze this correlation in nursing home residents over 80 years of age. Results could ultimately be helpful in implementing strategies for diagnosis and long-term follow-up of the very elderly population. Methods: The PARTAGE (Predictive value of blood pressure and ARTerial stiffness in institutionalized very AGEd population) study is a 2-year cohort study of 1130 subjects living in 72 nursing homes in France and Italy. The correlation between baseline systolic and diastolic blood pressure (SBP and DBP) and baseline PWV (measured with a PulsePen® tonometer) was studied in 1071 subjects with available PWV measurements. Results: Correlations between peripheral blood pressure and PWV were significant but weak: r=0.24 for self-measured SBP, r=0.30 for casual SBP, r=0.11 for self measured DBP, r=0.14 for casual DBP and r=0.26 for casual pulse pressure (PP). A trend for a weaker correlation was observed in the higher age group for self measured SBP and in the lower ADL group for self measured SBP and DBP. The correlations were systematically higher in women compared to men (but did not reach statistical significance) and lower with advanced age group. The correlation was not impacted by antihypertensive. Conclusion: These findings suggest that SBP, DBP and PWV provide different information in the very elderly. The prospective, longitudinal, long term PARTAGE study results will allow further insight; provide additional in-depth information regarding the respective prognostic value of these two measurement methods.
