Histopathologic examination was composedof band-like nests of basaloid cells showing peripheralpalization, abortive hair papilla and horn cysts ina fibrocellular stroma. A 82-year-old woman applied for a10-year old groin mass that recently slowly growing.     Evaluation of microsatellite instability in colorectal adenomas and carcinomas by immunohistochemistry and a comparison of histopathological features

    Objective: Approximately 15% of sporadic colorectal carcinomas (CRCs) develop along the microsatellite instability (MSI) pathway. In this study, we compared the MLH1, MSH2, Ki-67, and p53 immunostaining properties with histopathological features of colorectal adenomas and CRCs. Methods: A total of 102 cases were selected, including 50 adenomatous polyps, 25 adenocarcinomas, 10 adenocarcinomas with mucinous component, 14 mucinous adenocarcinomas, and three signet-ring cell carcinomas. The tissues were stained for MLH1, MSH2, p53, and Ki-67 primary antibodies. Results: Negative staining was observed for MLH1 in 25% and MSH2 in 3.8% of all CRC cases. Compared with adenocarcinoma not otherwise specified (NOS), mucinous adenocarcinomas showed weaker staining for MLH1, which was statistically significant. There was also a statistically significant difference between adenocarcinoma NOS and signet-ring cell carcinomas in terms of negative staining for MLH1. A total of 69.2% of the MLH1-negative cases were high-grade. There was a statistically significant relationship between the histological grade and MLH1 negativity. A positive correlation was found between the grade of dysplasia and p53 staining intensity and Ki-67 proliferation index. No negative staining was observed for MLH1 and MSH2 in any of the adenomatous polyps. Conclusion: For the histopathological examination of CRCs, in the presence of mucinous and poorly differentiated morphology, tumor-infiltrating lymphocytes and Crohn-like inflammatory response, immunohistochemical staining for MLH1, and MSH2 antibodies may be useful in the detection of tumors showing MSI

    Gastrointestinal Stromal Tumors with Unusual Localization: Report of Three Cases with a Brief Literature Review

    The most common tumors derived from the mesenchyme of the gastrointestinal system are stromal tumors. These tumors are typically seen in the stomach and small intestine and less frequently in the colon, rectum and esophagus and are very rarely located outside the gastrointestinal system. Cure is provided with complete surgical resection with resection borders free of tumor. Tumor size, mitotic index, localization, CD117 and CD34 negativity in immunohistochemical studies, mucosal ulceration and presence of necrosis help to predict recurrence of the illness and patient survival. In high-risk gastrointestinal stromal tumors (GISTs) there is an increased rate of recurrence and shortened survival despite complete surgical resection. Thus patients with a high-risk GIST should be given adjuvant therapy with imatinib mesylate. Sunitinib maleate is another FDA-approved agent only for cases who cannot tolerate imatinib or who are resistant to it.     The course of acute appendicitis disease i̇n the COVID-19 period

    The COVID 19 outbreak has changed the course of diseases requiring emergency surgery. Our aim in this study is to reveal whether the COVID 19 pandemic complicates acute appendicitis (AA) disease or not. We retrospectively investigated the patients who were diagnosed with AA in our city (Rize, Turkey). Our study includes two patient groups over 16 years of age who were operated on for AA. Group 2 includes patients operated between 11 March 2020 and 31 December 2020, while Group 1 includes patients who were operated on during the same period in 2019. There were 298 patients (57%) in Group 1 and 226 patients (43%) in Group 2. In Group 2, midline or paramedian incisions were used significantly more than the classical Mc Burney incision: (%10 vs %20, p<0.003). Postoperative complications were significantly higher in Group 2: (%5 vs %12, p<0.004). White blood cells and CRP were significantly different in Group 2 in the admission blood tests (p<0.005, p<0.001, respectively). Operation time and hospitalization time were significantly different in Group 2 (p<0.001, p<0.014, respectively). During the COVID-19 pandemic, there was a serious decrease in the number of patients admitted to the emergency services. Because people were afraid of being infected, they preferred to stay home and with the increase in complaints, they applied to the hospital. AA disease has also become more complicated, as people delayed their admission to the hospital. Serious changes happened in the course of the disease

    Effects of COVID-19 pandemic on colorectal cancer surgery

    BACKGROUND: The coronavirus disease-19 (COVID-19) pandemic has changed the course of diseases that require emergency surgery. OBJECTIVE: To evaluate the effect of the COVID-19 pandemic on colorectal cancer disease stage. DESIGN AND SETTING: Retrospective analysis in the city of Rize, Turkey. METHODS: This was a comparative analysis on two groups of patients with various symptoms who underwent surgical colorectal cancer treatment. Group 1 comprised patients operated between March 11, 2019, and December 31, 2019; while group 2 comprised patients at the same time of the year during the COVID-19 pandemic. RESULTS: Groups 1 and 2 included 56 and 48 patients, respectively. The rate of presentation to the emergency service was higher in Group 2 (P < 0.02). The stage of the pathological lymph nodes and the rate of liver metastasis was higher in Group 2 (P < 0.004 and P < 0.041, respectively). The disease stage was found to be more advanced in Group 2 (P < 0.005). The rate of postoperative complications was higher in Group 2 (P < 0.014). CONCLUSION: The presentation of patients with suspicious findings to the hospital was delayed, due both to the fear of catching COVID-19 and to the pandemic precautions that were proposed and implemented by healthcare authorities worldwide. Among the patients who presented to the hospital with emergency complaints and in whom colorectal cancer was detected, their disease was at a more advanced stage and thus a higher number of emergency oncological surgical procedures were performed on those patients

    Retrospective evaluation of patients with thyroidectomy in our region with thyroid

    This study presents to evaluate patients with malignant or operated for benign reasons in terms of the Eastern Black Sea region of Turkey which is an endemic region of Goiter. Within the scope of the study, 460 patients over the age of 18 who applied to the RecepTayip Erdoğan University Training and Research Hospital General Surgery Outpatient Clinic between January 2013 and July 2016 and underwent total thyroidectomy or hemithyroidectomy for benign or malign reasons were retrospectively analyzed. Age, gender, TSH values of the patients, preoperative (preop.) Thyroid or neck ultrasonography (USG) reports, thyroid fine-needle aspiration biopsy (FNAB) results, and pathological/morphological examination results were evaluated. 352 of 460 patients were women (76.5%) and the mean age of all patients was calculated as 52.2 (± 11.39). 70 cases (18%) non-diagnostic cytology, 104 cases (26.8%) benign cytology, 59 cases (15.2%) atypia of uncertain significance, 61 cases (15.7%) suspicion of follicular neoplasia/neoplasia, 78 cases (20.1%) were suspected of malignancy and 16 cases (4.1%) were detected as malignant cytology. While malignancy was diagnosed histopathologically in 248 of 352 female patients who were operated on (70.5%), malignancy was detected in 81 of 108 male patients (75%).
