    End-point estimates for iterated commutators of multilinear singular integrals

    Iterated commutators of multilinear Calderon-Zygmund operators and pointwise multiplication with functions in BMOBMO are studied in products of Lebesgue spaces. Both strong type and weak end-point estimates are obtained, including weighted results involving the vectors weights of the multilinear Calderon-Zygmund theory recently introduced in the literature. Some better than expected estimates for certain multilinear operators are presented too.Comment: A typo in the original manuscript lead to overlook a gap in one of our arguments which has been fixed. The new arguments are provided in the proof of Theorem 3.1 in Section 3.     Optimizing Sepsis Management Through Enhanced Protocol Compliance in the Emergency Department

    Problem: This quality improvement (QI) project aims to optimize early sepsis management and sepsis bundle compliance among Emergency Department (ED) registered nurses in order to reduce the risk of sepsis-related deaths and associated hospital length of stay. Context: Clinical Nurse Leader (CNL) students conducted a microsystem assessment on the ED at Hospital A, located within the greater San Francisco Bay Area. This microsystem, equipped with 115 nurses and 44 rooms, consists of an acuity of care for urgent to life-threatening medical conditions, including sepsis. Intervention: Following time constraints, and limitations, implementation of an intervention was not feasible; however, CNL students provided recommendations so stakeholders may further investigate and pursue. The recommended interventions include: increasing sepsis bundle training frequency, refining intravenous placement skills through technologically driven training and readily available resources, badge buddy sepsis cards, and revision of the current charting system. Measures: CNL students collected data to assess the nurses\u27 knowledge of the sepsis bundle and Hospital A’s sepsis policy. The data also served to identify frequency of training, debriefings, and any barriers nurses experienced while implementing the bundle. Due to time constraints, a post-intervention survey was not conducted. Results: 41 of the 115 ED registered nurses responded. Pre-intervention questionnaire responses revealed that 42% of ED nursing staff recommend protocol revision, 24.4% claim to not have attended any sepsis training with another 4.9% reporting rarely attending, and 46.3% reported there is no debriefing, or follow-up training, when sepsis bundle compliance is failed to be met. Additionally, nurses identified various barriers, prolonging sepsis bundle compliance. Conclusions: Analysis of the pre-intervention questionnaire responses from Hospital A’s ED registered nurses revealed knowledge gaps and barriers to timely sepsis management and bundle compliance. CNL students identified evidence-based recommendations, which stakeholders may implement to yield anticipated outcomes.     Cocoa Yield Development of Different Sites, Varieties, Production Systems and Years, in Alto Beni, Bolivia

    One of the most essential limiting factors of cocoa (Theobroma cacao L.) productivity worldwide is pests and diseases. Each of the major production regions has its specific pests and diseases. Reported yield losses range from minor to almost 100 per cent. In Alto Beni, located in the Amazonian watershed of the department La Paz, Bolivia, the Research Institute of Organic Agriculture (FiBL) and its local partners are addressing several problems of cocoa producers using a participatory technology development approach. Problems were identified in a participatory way and are, in order of priority, i) to reduce the incidence of pests and diseases, mainly the cocoa mirid (Monalonion dissimulatum) and Frosty Pod Rot (Moniliophthora roreri); ii) to evaluate the productivity of different cocoa varieties (local selections, introduced clones), and iii) to document the management practices and plantation layouts of high yielding cocoa farmers. In order to develop novel biological pest control measures, both the knowledge of cocoa yield development in the course of the harvest period, as well as the dynamics of pests and diseases are of great interest. Data from three different research activities of the mentioned project are analysed for yield development, the appearance, and the incidence of pests and diseases, where available. The research data are from: a) On-farm trials in multiple locations which were established in 2004. The performance of 16 cocoa varieties has been assessed for 3 years (2010–2012). b) Four high yielding cocoa farmers’ fields (2012 only). c) A long-term field experiment assessing the sustainability of five cocoa production systems (2011 and 2012). The trial investigates the influence of monocultures and different agroforestry systems under organic and conventional management on the yield development, among other agronomic, economic and environmental parameters
