    Stages of syphilis in South China – a multilevel analysis of early diagnosis

    Abstract Background Early diagnosis of syphilis and timely treatment can effectively reduce ongoing syphilis transmission and morbidity. We examined the factors associated with the early diagnosis of syphilis to inform syphilis screening strategic planning. Methods In an observational study, we analyzed reported syphilis cases in Guangdong Province, China (from 2014 to mid-2015) accessed from the national case-based surveillance system. We categorized primary and secondary syphilis cases as early diagnosis and categorized latent and tertiary syphilis as delayed diagnosis. Univariate analyses and multivariable logistic regressions were performed to identify the factors associated with early diagnosis. We also examined the factors associated with early diagnosis at the individual and city levels in multilevel logistic regression models with cases nested by city (n = 21), adjusted for age at diagnosis and gender. Results Among 83,944 diagnosed syphilis cases, 22% were early diagnoses. The city-level early diagnosis rate ranged from 7 to 46%, consistent with substantial geographic variation as shown in the multilevel model. Early diagnosis was associated with cases presenting to specialist clinics for screening, being male and attaining higher education level. Cases received syphilis testing in institutions and hospitals, and diagnosed in hospitals were less likely to be in early diagnosis. At the city-level, cases living in a city equipped with more hospitals per capita were less likely to be early diagnosis. Conclusions To enhance early diagnosis of syphilis, city-specific syphilis screening strategies with a mix of passive and client/provider-initiated testing might be a useful approach

    Distribution of reported syphilis cases in South China: spatiotemporal analysis.

    There was a varied spatial distribution of reported syphilis cases across cities in South China. This study aims to identify and describe spatiotemporal clusters of primary and secondary syphilis (P/S) cases in this region. Reported syphilis cases in Guangdong Province, China, from January 2014 to June 2015 were collected from the national centralized reporting system. Spatiotemporal clusters of P/S were identified and cross-validated by calculating local Moran's I, performing hotspot analysis (Getis-Ord Gi*), and constructing a discrete Poisson model in SaTScan. Reported cases within and outside the clusters were compared by bivariable and multivariable logistic regression. Out of 17,691 reported P/S cases, 11% were in the identified spatiotemporal clusters. The monthly P/S notification rate (per 100,000 persons) ranged between 0.6 and 1. The identified clusters were located in 14, out of 126, counties in eight, out of 21, cities. Cases of older age, living in rural area and taking self-initiated syphilis test were more likely to be in the clusters. Some areas bore a greater burden of P/S in Guangdong Province. Routine spatiotemporal analysis of P/S cases may be useful for enhancing syphilis control programs by strategic location-based service planning

    Controls on subaerial erosion rates in Antarctica

    Erosion rates offer insight on landscape development and the relative importance of chemical and physical processes of weathering. Minimal chemical weathering makes Antarctica an ideal location in which to compare the physical weathering of carbonate rocks to other lithologies. Here we report the first cosmogenic nuclide-derived erosion rates for carbonate rocks in Antarctica. Carbonate samples collected in the southernmost Ellsworth Mountains reflect a 36Cl erosion rate of 0.22 ± 0.02 mm/ka. This erosion rate is consistent with other reported Antarctic erosion rates, but is lower than 36Cl erosion rates derived from other arid regions in the world. These results are integrated with a continent-wide reanalysis of 28 erosion rate studies (>200 measurements), which comprise numerous rock types and other cosmogenic nuclides. By combining cosmogenic nuclide-derived erosion rates across studies, the larger trends provide insight into factors (e.g. lithology, glacial history, and availability of abrasive material) affecting subaerial erosion rates in Antarctica. Statistical analysis of the compiled data set shows differences based on lithology, with sandstone having the largest range of erosion rates. The compiled data also reveals higher erosion rates in areas with a large potential sediment supply, like the Dry Valleys. Samples collected from boulders yield lower erosion rates than those collected from bedrock, likely due to a combination of physical processes that affect boulders and bedrock differently, and glacial history, which can affect the apparent cosmogenic-nuclide derived erosion rate

    Abstract Human immunodeficiency virus (HIV) and sexually transmitted diseases (STDs) cause substantial morbidity and mortality both in African and China. However, there is limited data available on the prevalence of HIV/STDs and the uptake of testing experience ever during in China among African migrants. A venue-based survey was conducted at a tertiary hospital in Guangzhou to investigate the prevalence of HIV/STDs through laboratory testing and identify the associated factors. A total of 200 eligible participants completed the survey and bring into the analysis from April to October 2019, and the temporary visitors were excluded. The prevalence rates of HIV, syphilis, NG, CT, and HBsAg among the participants were 1.0%, 2.5%, 1.0%, 1.5%, and 5.5%, respectively. The overall reported rate of HIV/STD testing was 37.0%, with rates of 23.0% for HIV, 16.5% for syphilis, 12.5% for NG, 6.5% for genital herpes, 5.0% for condyloma acuminata, and 2.5% for CT. HIV/STD testing was associated with living environment in Guangzhou, having medical insurance in China, and utilizing health services in China in the past year. HIV/STDs are prevalent among Africans in Guangzhou, and the epidemic is likely to spread due to a significant proportion of unprotected sexual behaviors and low rates of HIV/STD testing. Urgent interventions, including targeted health education, promotion of health service utilization, and active surveillance of HIV/STDs, are needed to reduce the risk of HIV/STD transmission

    A sentiment analysis approach for travel-related Chinese online review content

    Using technology for sentiment analysis in the travel industry can extract valuable insights from customer reviews. It can assist businesses in gaining a deeper understanding of their consumers’ emotional tendencies and enhance their services’ caliber. However, travel-related online reviews are rife with colloquialisms, sparse feature dimensions, metaphors, and sarcasm. As a result, traditional semantic representations of word vectors are inaccurate, and single neural network models do not take into account multiple associative features. To address the above issues, we introduce a dual-channel algorithm that integrates convolutional neural networks (CNN) and bi-directional long and short-term memory (BiLSTM) with an attention mechanism (DC-CBLA). First, the model utilizes the pre-trained BERT, a transformer-based model, to extract a dynamic vector representation for each word that corresponds to the current contextual representation. This process enhances the accuracy of the vector semantic representation. Then, BiLSTM is used to capture the global contextual sequence features of the travel text, while CNN is used to capture the richer local semantic information. A hybrid feature network combining CNN and BiLSTM can improve the model’s representation ability. Additionally, the BiLSTM output is feature-weighted using the attention mechanism to enhance the learning of its fundamental features and lessen the influence of noise features on the outcomes. Finally, the Softmax function is used to classify the dual-channel fused features. We conducted an experimental evaluation of two data sets: tourist attractions and tourist hotels. The accuracy of the DC-CBLA model is 95.23% and 89.46%, and that of the F1-score is 97.05% and 93.86%, respectively. The experimental results demonstrate that our proposed DC-CBLA model outperforms other baseline models

    The improvement of rural living standards in developing countries and the continuous upgrading of the rural industrial economy have prompted the diversification of rural areas and residential forms. Thus, an integrated rural sewage treatment process has gradually become the mainstream technology for rural sewage treatment. Numerous studies have reported the effects of ecological wastewater treatment. Meanwhile, the relevant process technologies, evaluations, and operating models of the integrated rural sewage treatment process have yet to be thoroughly summarized. This review aims to fill these gaps. First, the applicability of artificial wetland, soil infiltration, stabilization pond, and integrated rural sewage treatment process technology in rural sewage treatment are outlined and compared. Second, the process flow, technical characteristics, and economic indicators of typical integrated sewage treatment processes (i.e., Anoxic/Oxic (A/O) process, Membrane Bio-Reactor (MBR) process, biological contact oxidation process, Sequencing Batch Reactor Activated Sludge (SBR) process) are introduced. The engineering application effects of the integrated rural sewage treatment process in different countries are also described. Third, the practical and effective evaluation methods of the integrated rural sewage treatment process are introduced. Bearing in mind the current operation and maintenance management modes of the integrated rural sewage treatment process in developed and developing countries, combined with the national conditions of developing countries, the prospect section provides development proposals for further optimization and improvement of the integrated rural sewage treatment process in developing countries

    HAMP as a Potential Diagnostic, PD-(L)1 Immunotherapy Sensitivity and Prognostic Biomarker in Hepatocellular Carcinoma

    Hepatocellular carcinoma (HCC) remains a global medical problem. Programmed cell death protein 1 (PD-1) is a powerful weapon against many cancers, but it is not sensitive to some patients with HCC. We obtained datasets from the Gene Expression Omnibus (GEO) database on HCC patients and PD-1 immunotherapy to select seven intersecting DEGs. Through Lasso regression, two intersecting genes were acquired as predictors of HCC and PD-1 treatment prognosis, including HAMP and FOS. Logistic regression was performed to build a prediction model. HAMP had a better ability to diagnose HCC and predict PD1 treatment sensitivity. Further, we adapted the support vector machine (SVM) technique using HAMP to predict triple-classified outcomes after PD1 treatment in HCC patients, which had an excellent classification ability. We also performed external validation using TCGA data, which showed that HAMP was elevated in the early stage of HCC. HAMP was positively correlated with the infiltration of 18 major immune cells and the expression of 2 important immune checkpoints, PDCD1 and CTLA4. We discovered a biomarker that can be used for the early diagnosis, prognosis and PD1 immunotherapy efficacy prediction of HCC for the first time and developed a diagnostic model, prognostic model and prediction model of PD1 treatment sensitivity and treatment outcome for HCC patients accordingly
