Are tones aligned with articulatory events? Evidence from Italian and French
International audienceTonal alignment work has suggested that the temporal location of tonal targets relative to segmental "anchors" might be governed by principles of synchrony and stability (Arvaniti et al 1998, Ladd et al. 1999, inter alia). However, a number of discrepancies have emerged in the cross-linguistic study of alignment. For instance, despite some regularities in the alignment of L targets (Caspers and van Heuven 1993; Prieto et al. 1995), the alignment of H targets appears to be quite controversial. In fact, it is sometimes difficult to find definite segmental landmarks to which such targets might be aligned. Also, most of the alignment proposals so far inherently assume that if some anchors for tonal alignment do exist they must be acoustic in nature. A plausible alternative would be to assume that such anchors are primarily articulatory, which would explain why in some cases the underlying regularities would be masked. Hence, we adopt a new experimental paradigm for alignment research in which articulatory measures are performed simultaneously with acoustic measures. In order to test the constant alignment hypothesis, a preliminary study (D'Imperio et al. 2003) was conducted in which various latency measures, both acoustically and articulatorily based, were analyzed. Specifically, the kinematics of OPTOTRAK markers attached to the speaker's upper and lower lip was tracked over time during the production of the corpus sentences. The melodic target considered is the H tone of LH nuclear rises in Neapolitan Italian. In this variety, yes/no question LH rises are systematically later than (narrow focus) statement LH rises (D'Imperio 2000, 2001, 2002; D'Imperio and House 1997). In order to test the hypothesis of constant anchoring of H targets, the materials were produced with two different rates of speech, i.e. normal and fast. Summarizing the results, H targets of nuclear rises in Neapolitan statements and questions appear to be more closely phased with the articulatory dimension of between-lip distance than with two of the most commonly employed acoustic segmental landmarks for tonal alignment (i.e., onset and offset of stressed vowel). Statement H tones are phased with maximum between-lip distance within the stressed syllable. Note that this location does not correspond to any identifiable segmental boundary, acoustic event or phonological unit, and does not overlap with RMS peak amplitude. In fact, RMS peaks were generally much earlier than articulatory peaks, hence further away from H peaks. This calls for the collection and analysis of more articulatory data (especially jaw and tongue movements) to shed light on tonal alignment issues.In a second study, a French corpus was collected on the basis of the alignment contrast found by Welby (2003, in press). Welby's results show that listeners use the alignment of the initial rise (LHi) in French Accentual Phrases as a cue to speech segmentation. Specifically, listeners exploit the presence of an early rise to demarcate the beginning of a content word. In the present study, a corpus was built with a set of utterances displaying this specific alignment contrast. The kinematics of 10 pellets (8 on the face and tongue, 2 references) was tracked over time using an electromagnetometer (EMA, Carstens). The phasing of several articulatory events relative to the L and H part of the early rise were examined. The preliminary results seem to point to some kind of fine alignment specification for the L and H target. Specifically, we hypothesize that tonal target commands of Neapolitan as well as French rises are phased with commands of the supralaryngeal articulator involved to produce the segments to which the tone is associated. Regarding the word segmentation issue for French, it is important to study alignment in a diachronic perspective since we know of case of speech segmentation errors that can lead to lexical reinterpretation and change (l'abondance "abundance" > la bondance, from Welby 2003). We also take these results to suggest that not all rises align in the same way with the associated syllable. Though the role of articulatory constraints is important, the exact phasing properties of prosodic events are language-specific. Since prosody has recently become the realm of investigation of the Task Dynamics program (Byrd and Saltzman 2003), our alignment work will be cast under such a perspective
Barriers to access prevention of mother-to-child transmission for HIV positive women in a well-resourced setting in Vietnam
<p>Abstract</p> <p>Background</p> <p>According to Vietnamese policy, HIV-infected women should have access at least to HIV testing and Nevirapine prophylaxis, or where available, to adequate counselling, HIV infection staging, ARV prophylaxis, and infant formula. Many studies in high HIV prevalence settings have reported low coverage of PMTCT services, but there have been few reports from low HIV prevalence settings, such as Asian countries. We investigated the access of HIV-infected pregnant women to PMTCT services in the well-resourced setting of the capital city, Hanoi.</p> <p>Methods</p> <p>Fifty-two HIV positive women enrolled in a self-help group in Hanoi were consulted, through in-depth interviews and bi-weekly meetings, about their experiences in accessing PMTCT services.</p> <p>Results</p> <p>Only 44% and 20% of the women had received minimal and comprehensive PMTCT services, respectively. Nine women did not receive any services. Twenty-two women received no counselling. The women reported being limited by lack of knowledge and information due to poor counselling, gaps in PMTCT services, and fear of stigma and discrimination. HIV testing was done too late for optimal interventions and poor quality of care by health staff was frequently mentioned.</p> <p>Conclusion</p> <p>In a setting where PMTCT is available, HIV-infected women and children did not receive adequate care because of barriers to accessing those services. The results suggest key improvements would be improving quality of counselling and making PMTCT guidelines available to health services. Women should receive early HIV testing with adequate counselling, safe care and prophylaxis in a positive atmosphere towards HIV-infected women.</p

Children's Attention-Deficit/Hyperactivity Disorder Symptoms Predict Lower Diet Quality but Not Vice Versa:Results from Bidirectional Analyses in a Population-Based Cohort
BACKGROUND: As an adjuvant for medication, dietary changes focused on specific nutrients have been proposed to prevent or reduce attention-deficit/hyperactivity disorder (ADHD) symptoms. However, whether an overall healthy dietary pattern is associated with ADHD symptom severity during childhood remains unclear. Furthermore, it is not clear what the direction of this association is. OBJECTIVES: We aimed to examine the association between dietary patterns and ADHD symptoms in school-aged children. In addition, we aimed to identify the temporal direction of this association-that is, whether dietary patterns predict ADHD symptoms or vice versa. METHODS: We analyzed data from 3680 children participating in the Generation R Study, a prospective cohort in Rotterdam, Netherlands. ADHD symptoms were assessed with parent-report questionnaires at ages 6 and 10 y using the Child Behavior Checklist. Dietary intake was assessed at the age of 8 y with a validated food-frequency questionnaire. We computed a diet quality score reflecting adherence to dietary guidelines. We examined bidirectional associations of diet quality with ADHD symptom scores using multivariable linear regression analysis and cross-lagged modeling. RESULTS: Linear regressions showed that more ADHD symptoms at age 6 y were associated with a lower diet quality score at age 8 y (SD score = -0.08; 95% CI: -0.11, -0.05) but that diet quality at age 8 y was not associated with ADHD symptoms at age 10 y. Cross-lagged models confirmed a unidirectional relation from ADHD symptoms to diet quality but not vice versa. Associations did not differ by overweight status or between boys and girls. CONCLUSION: Our study suggests that children with more ADHD symptoms may be at higher risk of an unhealthy diet but that overall diet quality does not affect ADHD risk
Intravitreal Sirolimus for the Treatment of Noninfectious Uveitis: Evolution through Preclinical and Clinical Studies
In recent decades, the treatment paradigm for noninfectious intermediate uveitis, posterior uveitis, and panuveitis, a group of intraocular inflammatory diseases, has included systemic and local (periocular or intraocular) corticosteroids, biologics, and other steroid-sparing immunomodulatory therapy agents. Recently, an intravitreal formulation of sirolimus, an immunosuppressant that inhibits the mammalian target of rapamycin, a key regulator of cell growth in the immune system, was developed. On the basis of this mechanism and the local method of delivery, it was hypothesized that intravitreal sirolimus can improve ocular inflammation in patients with noninfectious intermediate uveitis, posterior uveitis, and panuveitis, with minimal systemic exposure and systemic adverse events (AEs). This review summarizes the pharmacokinetics, efficacy, and safety results of intravitreal sirolimus from 3 preclinical studies and 4 phase 1–3 clinical studies. Preclinical studies in rabbits showed that 22 to 220 μg intravitreal sirolimus results in sustained release of sirolimus in the vitreous for 2 months or more, with systemic concentrations below the threshold for systemic immunosuppression (approximately 8 ng/ml). Subsequently, 2 phase 1 studies (n = 50 and n = 30) established that intravitreal sirolimus improves ocular inflammation in humans. Further investigation in phase 2 and 3 studies (n = 24 and n = 347, respectively) suggested that 440 μg has the best benefit-to-risk profile. In the phase 3 study, the proportion of patients who showed complete resolution of ocular inflammation at month 5 was significantly higher in the 440-μg group than in the 44-μg group (22.8% vs. 10.3%; P = 0.025, Fisher exact test). In addition, 47 of 69 patients (68.1%) who were treated with systemic corticosteroids at baseline discontinued corticosteroid use at month 5. No sirolimus-related systemic AEs were reported in phase 1–3 studies. Collectively, these preclinical and clinical study data of intravitreal sirolimus support the therapeutic rationale of treating noninfectious uveitis with a local mammalian target of rapamycin inhibitor and suggest that 440 μg intravitreal sirolimus has the potential to be an effective and well-tolerated anti-inflammatory and corticosteroid-sparing treatment for noninfectious intermediate uveitis, posterior uveitis, and panuveitis
Evolution of Online Health-Related Information Seeking in France From 2010 to 2017: Results From Nationally Representative Surveys
BACKGROUND: Given the rapid ongoing progression of the internet and increase in health information available from disparate online sources, it is important to understand how these changes impact online health information-seeking behavior of the population and the way of managing one's health. OBJECTIVE: This paper aims at describing the evolution of internet use as a source of health information between 2010 and 2017, as well as the characteristics of online health information seekers, topics of interest, sources of information, and trust in retrieved information and potential impact on behavior. METHODS: Data from the French nationally representative surveys Health Barometers were used (N=4141 in 2010, 4811 in 2014, and 6255 in 2017). Evolutions over time were assessed using chi-square tests. Associations with sociodemographic characteristics and health status were evaluated using logistic regression models. RESULTS: The use of the internet as a source of health information rose between 2010 and 2014 (from 37.3% to 67.9%, P<.001) but decreased significantly in 2017 (60.3%, P<.001). Overall, the profile of health information seekers compared with nonseekers did not change over time. They were more likely to be women, to be younger, to have a higher educational level, to have a higher household income, and to be executives. Between 2014 and 2017, the proportion of those who did not pay attention to the source of information significantly increased to reach 39.7% (P<.001). In 2017 as in 2014, general health-related websites remained the first source of information (38.6%) while institutional websites were the third source (8.1%). Most information seekers trusted the information found online in 2010 (more than 80%), with a slight decrease between 2014 and 2017 (P=.048). Among individual characteristics, trust in the information was the main determinant of the way of managing one's health (odds ratio 4.06, 95% CI 3.26-5.06). CONCLUSIONS: After a rapid growth in the internet use for seeking health information in the 2010 to 2014 period, a decrease was recorded in 2017, in parallel with a decrease in trust in the quality and reliability of information found online. These findings underline the need for public health authorities to increase citizens' eHealth literacy and to provide alternative trustworthy sources combining the popularity and accessibility of general health information websites
CHARACTERIZATION OF PAST METALLURGICAL RESIDUES USING GEOPHYSICAL IMAGING: A CASE STUDY OF DUFERCO SITE (BELGIUM)
Ancient metallurgical industries produced large amounts of residues which were typically
deposited in heaps or tailing ponds. They were derived from mineral processing and metallurgical treatments that were not as efficient as the extraction processes used nowadays. On one hand, metallurgical wastes could represent a potential source of pollution, being an environmental and sanitary threat even centuries after the end of industrial activities. On the other hand, these residues may still contain valuable ferrous materials, non-ferrous metals and other elements considered as critical raw materials. In this regard, remediation strategies can be improved by integrating the valorization of
metallurgical residues and potential resource recovery. To this purpose, it is crucial to have in-depth information about the composition of the metallurgical waste, and spatial information to identify and quantify volumes of these residues. Geophysical methods represent suitable non-invasive technologies for subsurface site characterization, imaging lateral and vertical variations in the physical properties of geological environments including anthropogenic deposits. Complemented with ground truth data from excavations, geophysical imagery can be quantitatively interpreted in terms of material(s) composition and zonation, volume(s) estimation, etc. In this contribution, we present the results of an integrated geophysical investigation carried out in a slag heap of the former iron and steel factory of Duferco, located in Belgium. First, we carried out a geophysical survey in the field with a 3D Electrical Resistivity Tomography (ERT) and Induced Polarization (IP) acquisition. Based on these results, we designed a targeted sampling, i.e., excavation and collection of samples at different locations and depths. In the laboratory, we measured ERT and IP in the samples and compared them with elemental chemical analyses. We investigated correlations between the laboratory data and identified different types of slags or ‘categories’, i.e., slags richer in Fe, Ca and Si. Then we used a probabilistic approach to classify or predict the categories in the whole domain of field acquisition (where no samples are available). To this
aim we used the ERT and IP field data co-located with the samples and the elevation at which these samples were taken. Overall, the combination of geophysical measurements in the field, targeted sampling, geophysical measurements in the laboratory and correlations with chemical analyses, may be promising to quantify the metallic content or materials of interest with a discriminating geophysical signature.NWE-Regenerati
CRLX101, a Nanoparticle–Drug Conjugate Containing Camptothecin, Improves Rectal Cancer Chemoradiotherapy by Inhibiting DNA Repair and HIF1α
Novel agents are needed to improve chemoradiotherapy for locally advanced rectal cancer. In this study, we assessed the ability of CRLX101, an investigational nanoparticle-drug conjugate containing the payload camptothecin (CPT), to improve therapeutic responses as compared to standard chemotherapy. CRLX101 was evaluated as a radiosensitizer in colorectal cancer cell lines and murine xenograft models. CRLX101 was as potent as CPT in vitro in its ability to radiosensitize cancer cells. Evaluations in vivo demonstrated that the addition of CRLX101 to standard chemoradiotherapy significantly increased therapeutic efficacy by inhibiting DNA repair and HIF-1α pathway activation in tumor cells. Notably, CRLX101 was more effective than oxaliplatin at enhancing the efficacy of chemoradiotherapy, with CRLX101 and 5-fluorouracil (5-FU) producing the highest therapeutic efficacy. Gastrointestinal toxicity was also significantly lower for CRLX101 compared to CPT when combined with radiotherapy. Our results offer a preclinical proof of concept for CRLX101 as a modality to improve the outcome of neoadjuvant chemoradiotherapy for rectal cancer treatment, in support of ongoing clinical evaluation of this agent (LCC1315 {"type":"clinical-trial","attrs":{"text":"NCT02010567","term_id":"NCT02010567"}}NCT02010567)
