Dor musculoesquelĂ©tica na SĂndrome PĂłs-Covid-19: Uma revisĂŁo integrativa
A SĂndrome PĂłs-Covid-19 Ă© caracterizada pela persistĂŞncia e/ou aparecimento de novos sintomas e anormalidades que persistem alĂ©m de 12 semanas do inĂcio da Covid-19 aguda e nĂŁo sĂŁo atribuĂveis a diagnĂłsticos alternativos. Dentre os sintomas descritos na literatura a respeito da SĂndrome PĂłs-Covid-19, a dor musculoesquelĂ©tica está entre os mais frequentes. O presente estudo teve como objetivo revisar a literatura e identificar a forma de apresentação e fatores preditores para a dor musculoesquelĂ©tica na SĂndrome PĂłs-Covid-19. Uma pesquisa bibliográfica abrangente foi realizada nas seguintes bases de dados: Medline (via PubMed), Embase, PEDro, Lilacs, Scopus, Web of Science e Cinahl no perĂodo de 22 de abril a 14 de junho de 2022. Dos 2.044 artigos encontrados, 16 foram incluĂdos, compreendendo um total de 13.089 pacientes. Os resultados mostraram que mialgia (27,12%, 11 estudos), artralgia (28,88%, 9 estudos), artromialgia (43,07%, dois estudos) e dor musculoesquelĂ©tica (15.3%, 1 estudo) foram os sintomas álgicos musculoesquelĂ©ticos presentes na SĂndrome PĂłs-Covid-19. O sexo feminino foi um fator de risco para a presença de artralgia e mialgia, enquanto a idade entre 40 e 59 anos foi associada Ă persistĂŞncia de artromialgia. Elevado Ăndice de massa corporal (IMC) foi um fator preditor para a presença da mialgia. A severidade da infecção e a necessidade de hospitalização nĂŁo foram preditoras para a presença de mialgia e artralgia na SĂndrome PĂłs-Covid-19. Beneficial effects of treadmill training in experimental diabetic nerve regeneration
OBJECTIVES: We investigated the effects of treadmill training (10 weeks) on hindlimb motor function and nerve morphometric parameters in diabetic rats submitted to sciatic nerve crush. MATERIALS AND METHOD: Wistar rats (n = 64) were divided into the following groups: non-diabetic; trained non-diabetic; non-diabetic with sciatic nerve crush; trained non-diabetic with sciatic nerve crush; diabetic; trained diabetic; diabetic with sciatic nerve crush or trained diabetic with sciatic nerve crush. Diabetes was induced by streptozotocin injection (50 mg/kg, iv). Hindlimb motor function was evaluated weekly by assessing sciatic functional indices, and the proximal and distal portions of the sciatic nerve were used for morphometric analysis. RESULTS: At 13 weeks post-injury, the distal nerve portion of all injured groups and the proximal nerve portion of the diabetic with sciatic nerve crush group presented altered morphometric parameters such as decreased myelinated fiber diameter (~7.4 + 0.3μm vs ~4.8 + 0.2μm), axonal diameter (~5 + 0.2μm vs ~3.5 + 0.1μm) and myelin sheath thickness (~1.2 + 0.07μm vs ~0.65 + 0.07μm) and an increase in the percentage of area occupied by endoneurium (~28 + 3% vs ~60 + 3%). In addition, in the non-diabetic with sciatic nerve crush group the proximal nerve portion showed a decreased myelinated fiber diameter (7.4+0.3μm vs 5.8 + 0.3μm) and myelin sheath thickness (1.29 + 0.08μm vs 0.92 + 0.08μm). The non-diabetic with sciatic nerve crush, trained non-diabetic with sciatic nerve crush, diabetic with sciatic nerve crush and trained diabetic with sciatic nerve crush groups showed normal sciatic functional index from the 4th,4th,9th and 7th week post-injury, respectively. Morphometric alterations in the proximal nerve portion of the diabetic with sciatic nerve crush and non-diabetic with sciatic nerve crush groups were either prevented or reverted to values similar to the non-diabetic group by treadmill training. CONCLUSION: Diabetic condition promoted delay in sciatic nerve regeneration. Treadmill training is able to accelerate hindlimb motor function recovery in diabetic injured rats and prevent or revert morphometric alterations in proximal nerve portions in non-diabetic and diabetic injured rats
Treadmill training in diabetic rats: effects on motor and sensitive behavior, tyrosine hydroxylase immunoreactivity in the substantia nigra, calcitonin gene-related peptide in the dorsal horn of the spinal cord and sural nerve morphology
O objetivo desta tese foi analisar os efeitos do exercĂcio fĂsico sobre o comportamento motor, a expressĂŁo de tirosina hidroxilase na subtsância nigra, a sensibilidade mecânica e nociceptiva, a morfologia do nervo sural e a expressĂŁo de peptĂdeo relacionado ao gene da calcitonina no corno dorsal da medula espinal de ratos adultos machos e diabĂ©ticos induzidos por estreptozotocina. Para isso foram realizados trĂŞs experimentos. No primeiro, foi realizado protocolo de oito semanas de treinamento em esteira em animais nĂŁo diabĂ©ticos e diabĂ©ticos. Foi analisado o comportamento motor, pelo uso do teste do rotarod e campo aberto e a expressĂŁo de TH na substância nigra e área tegmental ventral desses animais. Os animais diabĂ©ticos sedentários apresentaram dĂ©ficits motores e diminuição na reatividade Ă tirosina hidroxilase (TH) na substância nigra e área tegmental ventral. As alterações motoras e reatividade Ă TH na substância nigra foram prevenidas nos animais diabĂ©ticos treinados. No segundo experimento os animais diabĂ©ticos sedentários apresentaram diminuição no limiar sensorial mecânico, avaliado pelos filamentos de von Frey, bem como diminuição da área e diâmetro das fibras mielinizadas, aumento da densidade de fibras e área ocupada pelo tecido conjuntivo, diminuição da espessura da bainha de mielina e aumento da relação g no nervo sural. Essas alterações nĂŁo foram vistas nos animais diabĂ©ticos treinados. Ainda, o nervo sural dos animais diabĂ©ticos apresentou maior porcentagem de fibras mielinizadas pequenas e menor porcentagem de fibras mielinizadas de pequeno diâmetro quando comparado aos animais controles e diabĂ©ticos treinados. No terceiro experimento demonstramos que os animais diabĂ©ticos induzidos por estreptozotocina apresentaram diminuição da sensibilidade nociceptiva, avaliada pelo teste do tail flick, assim como diminuição da expressĂŁo do peptĂdeo relacionado ao gene da calcitonina no corno dorsal da medula espinal. Por outro lado, os animais diabĂ©ticos treinados nĂŁo mostraram alterações na sensibilidade mecânica e na expressĂŁo do mesmo peptĂdeo. Estes dados demonstram que o exercĂcio fĂsico em esteira em ratos diabĂ©ticos Ă© capaz de prevenir alterações no comportamento motor e sensorial, assim como as alterações morfolĂłgicas no nervo sural, na expressĂŁo de tirosina hidroxilase na substância nigra e do peptĂdeo relacionado ao gene da calcitonina no corno dorsal da medula espinal.The aim of this thesis was to study the effects of treadmill training on the motor and sensitive behavior, the tyrosine hydroxylase immunoreactivity in the substantia nigra, the mechanical and nociceptive sensitivity, the sural nerve morphology and the calcitonin gene-related peptide immunoreactivity in the dorsal horn of the spinal cord from diabetic rats induced by streptozotocin. For this, we made three experiments. In the first, an eight weeks treadmill training protocol was made in diabetic and non diabetic animals. Was analyzed the motor behavior using rotarod and open field tests and the immunoreactivity to tyrosine hydroxylase (TH) in the substantia nigra and ventral tegmental area of these animals. Sedentary diabetic animals showed motor deficits and a decrease in the TH immunoreactivity in the substantia nigra and in the ventral tegmental area. Treadmill training in diabetic animals prevented the motor deficits and the decrease in the tyrosine hydroxylase reactivity. In the second experiment, sedentary diabetic animals showed a reduction in the mechanical sensitivity threshold, analyzed using von Frey filaments. Also, diabetic animals showed a decreased in the area and in diameter of the myelinated fibers, an increased in the fibers density and in the area occupied by connective tissue, a decreased myelin sheat thickness and an increased g ratio in the sural nerve. These alterations were not seen in trained diabetic animals. In addition, the sural nerve of the sedentary diabetic animals had a higher percentage of small myelinated fibers and a lower percentage of large diameter myelinated fibres than the control and trained diabetic animals. Moreover, in the third experiment we showed that the diabetic animals induced by streptozotocin had a decrease in the nociceptive sensitivity, analyzed using the tail flick test. These animals showed a decrease in the calcitonin gene-related peptide immunoreactivity in the dorsal horn of the spinal cord. Furthermore, these decreased in the nociceptive sensitivity and in the peptide immunoreactivity were not seen in the trained diabetic rats These data showed that treadmill training in diabetic rats is able to prevent alterations in the motor and sensitive behavior. Also, exercise is able to prevent the morphological alterations in the sural nerve, the decrease in the TH immunoreactivity in the substantia nigra and the decrease in the calcitonin gene-related peptide immunoreactivity in the dorsal horn of the spinal cord
InfluĂŞncia do exercĂcio fĂsico sobre a morfologia dos neurĂ´nios do gânglio da raiz dorsal L5 de ratos machos adultos diabĂ©ticos
O objetivo deste estudo foi avaliar os efeitos do treinamento fĂsico em esteira sobre a morfologia dos neurĂ´nios do gânglio da raiz dorsal (GRD) L5 de ratos diabĂ©ticos induzidos pela estreptozotocina. Ratos machos foram divididos em 4 grupos: grupo controle (C; n = 5; ratos nĂŁodiabĂ©ticos), controle treinado (TC; n = 5; ratos nĂŁo-diabĂ©ticos que realizaram exercĂcio fĂsico), diabĂ©tico (D; n = 5; ratos diabĂ©ticos que nĂŁo realizaram exercĂcio fĂsico) e diabĂ©tico treinado (TD; n = 5; ratos nĂŁo-diabĂ©ticos que realizaram treinamento fĂsico). O treinamento em esteira foi realizado a partir de um protocolo de treinamento para ratos diabĂ©ticos, o qual durou 10 semanas (5dias/semana, duas vezes/dia). A glicemia foi avaliada a cada duas semanas. EntĂŁo, os animais foram sacrificados e o GRD L5 foi removido. A análise histolĂłgica e morfomĂ©trica consistiu de uma avaliação dos volumes nucleares e dos somas das cĂ©lulas B e A. Os dados da glicemia foram analisados por ANOVA de medidas repetidas,enquanto que os dados dos volumes nucleares e dos somas das cĂ©lulas B e A foram analisados por ANOVA de duas vias, com teste post-hoc de Tukey. Os resultados mostraram que os ratos diabĂ©ticos, treinados e nĂŁo-treinados apresentaram valores glicĂŞmicos aumentados quando comparados aos ratos nĂŁo-diabĂ©ticos. O diabetes reduziu o volume do nĂşcleo e do soma das cĂ©lulas B e A. No volume nuclear das cĂ©lulas A do GRD houve uma interação entre diabetes e exercĂcio fĂsico, e o teste post-hoc mostrou que os ratos diabĂ©ticos tiveram o volume nuclear menor que os ratos controles (3630,68 ± 240,36; 2796,24 ± 508,17; 2269,36 ± 221,97; 2915,84 ± 209,67 ÎĽm3 para grupo controle, controle treinado, diabĂ©tico, e diabĂ©tico treinado respectivamente, P = 0.03). O exercĂcio aumentou o volume nuclear das cĂ©lulas A dos ratos diabĂ©ticos. Em conclusĂŁo, diabetes causou alterações patolĂłgicas nas cĂ©lulas do GRD, e algumas das quais foram modificadas pelo exercĂcio fĂsico, como evidenciado pela melhora dos padrões morfolĂłgicos das cĂ©lulas do GRD encontradas no presente estudo. AlĂ©m disso, o exercĂcio fĂsico foi capaz de aumentar o volume nuclear das cĂ©lulas A dos ratos diabĂ©ticos

Exercise alleviates hypoalgesia and increases the level of calcitonin gene-related peptide in the dorsal horn of the spinal cord of diabetic rats
OBJECTIVE: The aim of this study was to evaluate the effects of treadmill training on nociceptive sensitivity and immunoreactivity to calcitonin gene-related peptide in the dorsal horn of the spinal cord of diabetic rats. METHODS: Male Wistar rats were divided into three groups: control, diabetic and trained diabetic. Treadmill training was performed for 8 weeks. The blood glucose concentrations and body weight were evaluated 48 h after diabetes induction and every 30 days thereafter. The nociceptive sensitivity was evaluated using the tail-flick apparatus. The animals were then transcardially perfused, and the spinal cords were post-fixed, cryoprotected and sectioned in a cryostat. Immunohistochemistry for calcitonin gene-related peptide analysis was performed on the dorsal horn of the spinal cord. RESULTS: The nociceptive sensitivity analysis revealed that, compared with the control and trained diabetic animals, the latency to tail deflection on the apparatus was longer for the diabetic animals. Optical densitometry demonstrated decreased calcitonin gene-related peptide immunoreactivity in the dorsal horn of the spinal cord in diabetic animals, which was reversed by treadmill training. CONCLUSION: We concluded that treadmill training can alleviate nociceptive hypoalgesia and reverse decreased calcitonin gene-related peptide immunoreactivity in the dorsal horn of the spinal cord of diabetic animals without pharmacological treatment

Treadmill training induces plasticity in spinal motoneurons and sciatic nerve after sensorimotor restriction during early postnatal period : new insights into the clinical approach for children with cerebral palsy
The aim of the present study was to investigate whether locomotor stimulation training could have beneficial effects on the morphometric alterations of spinal cord and sciatic nerve consequent to sensorimotor restriction (SR). Male Wistar rats were exposed to SR from postnatal day 2 (P2) to P28. Control and experimental rats underwent locomotor stimulation training in a treadmill for three weeks (from P31 to P52). The cross-sectional area (CSA) of spinal motoneurons innervating hind limb muscles was determined. Both fiber and axonal CSA of myelinated fibers were also assessed. The growth-related increase in CSA of motoneurons in the SR group was less than controls. After SR, the mean motoneuron soma size was reduced with an increase in the proportion of motoneurons with a soma size of between 0 and 800 μm2. The changes in soma size of motoneurons were accompanied by a reduction in the mean fiber and axon CSA of sciatic nerve. The soma size of motoneurons was reestablished at the end of the training period reaching controls level. Our results suggest that SR during early postnatal life retards the growth-related increase in the cell body size of motoneurons in spinal cord and the development of sciatic nerve. Additionally, three weeks of locomotor stimulation using a treadmill seems to have a beneficial effect on motoneurons’ soma size
