    The missing complexity in seismically imaged normal faults: What are the implications for geometry and production response?

    The impact of geometric uncertainty on across-fault flow behaviour at the scale of individual intra-reservoir faults is investigated in this study. A high resolution digital elevation model (DEM) of a faulted outcrop is used to construct an outcrop-scale geocellular grid capturing high-resolution fault geometries (5 m scale). Seismic forward modelling of this grid allows generation of a 3D synthetic seismic cube, which reveals the corresponding seismically resolvable fault geometries (12.5 m scale). Construction of a second geocellular model, based upon the seismically resolvable fault geometries, allows comparison with the original outcrop geometries. Running fluid flow simulations across both models enables us to assess quantitatively the impact of outcrop resolution versus seismic resolution fault geometries upon across-fault flow. The results suggest that seismically resolvable fault geometries significantly underestimate the area of across-fault juxtaposition relative to realistic fault geometries. In turn this leads to overestimates in the sealing ability of faults, and inaccurate calculation of fault plane properties such as transmissibility multipliers (TMs)

    Understanding regional scale structural uncertainty: The onshore Gulf of Corinth Rift as a hydrocarbon exploration analogue

    A major challenge when exploring for hydrocarbons in frontier areas is a lack of data coverage. Data may be restricted to regional scale 2D seismic lines, from which assumptions of the 3D geometric configuration are drawn. Understanding the limitations and uncertainties when extrapolating 2D data into 3D space is crucial when assessing the requirements for acquiring additional data such as 3D seismic or exploration wells, and of assigning geologically reasonable uncertainty ranges. The Onshore Gulf of Corinth Rift provides an excellent analogue for rift-scale structural uncertainty in the context of hydrocarbon exploration. Here we use seismic forward modelling to explore this area of uncertainty. Synthetic seismic sections have been generated across the rift based upon fault geometries mapped in the field. Comparison of these sections with the mapped geometries allows quantification of uncertainties encountered when extrapolating 2D data into three dimensions. We demonstrate through examples how potential column heights may be both severely over- and under-estimation due to trap integrity, spill point depth and fault seal ambiguities directly related to fault geometric uncertainty. In addition, fault geometries and linkages also control the location of hanging wall syn-rift reservoirs. Hence, gross reservoir volumes and sediment facies distributions are also significantly influenced by how fault geometries are extrapolated along-strike from 2D to 3D

    How to increase earthquake and home fire preparedness: the fix-it intervention

    Published, evaluated community intervention studies concerning natural hazard preparedness are rare. Most lack a rigorous methodology, thereby hampering the development of evidence-based interventions. This paper describes the rationale and methodology of a cross-cultural, longitudinal intervention study on earthquake and home fire preparedness, termed fix-it. The aim is to evaluate whether and how the intervention brings about behaviour change in the targeted communities in two coastal cities with high seismic risk: Seattle, USA and Izmir, Turkey. Participants are adult residents of these cities. The intervention group attends a 6-h workshop, which focuses on securing items in the household. The control group does not attend the workshop. All participants complete baseline and post-intervention, as well as 3- and 12-month follow-up assessments. The primary outcome measure is an observational measure of nine preparedness items for earthquake and fire in participants’ homes. This is evaluated alongside participants’ self-reports concerning their preparedness levels. Secondary outcomes are changes in levels of self-efficacy, perceived outcome, trust, corruption, empowerment, anxiety and social cohesion. Results from the first of the studies, conducted in Seattle in September 2015, indicate that while the fix-it intervention is effective, in the longer term, multi-hazard preparedness is increased by the mere act of going into people’s homes to observe their preparedness levels along with assessing self-reported preparedness and sociopsychological orientation towards natural hazards. This protocol and study aim to augment the empirical literature on natural hazard preparedness, informing national and international policy on delivery of evidence-based community interventions to promote multi-hazard preparedness in households

    A pneumococcal MerR-like regulator and S-nitrosoglutathione reductase are required for systemic virulence

All rights reserved.A transcriptional regulator, NmlR(sp), has been identified in Streptococcus pneumoniae that is required for defense against nitric oxide (NO) stress. The nmlR(sp) gene is cotranscribed with adhC, which encodes an alcohol dehydrogenase that is able to reduce S-nitrosoglutathione (GSNO) with NADH as reductant. nmlR(sp) and adhC mutants exhibited a reduced level of NADH-GSNO oxidoreductase activity and were more susceptible to killing by NO than were wild-type cells. Comparison of the virulence of wild-type and mutant strains by use of a mouse model system showed that NmlR(sp) and AdhC do not play a key role in the adherence of pneumococci to the nasopharynx in vivo. An intraperitoneal challenge experiment revealed that both NmlR(sp) and AdhC were required for survival in blood.     Appropriate disclosure of a diagnosis of dementia : identifying the key behaviours of 'best practice'

    Background: Despite growing evidence that many people with dementia want to know their diagnosis, there is wide variation in attitudes of professionals towards disclosure. The disclosure of the diagnosis of dementia is increasingly recognised as being a process rather than a one-off behaviour. However, the different behaviours that contribute to this process have not been comprehensively defined. No intervention studies to improve diagnostic disclosure in dementia have been reported to date. As part of a larger study to develop an intervention to promote appropriate disclosure, we sought to identify important disclosure behaviours and explore whether supplementing a literature review with other methods would result in the identification of new behaviours. Methods: To identify a comprehensive list of behaviours in disclosure we conducted a literature review, interviewed people with dementia and informal carers, and used a consensus process involving health and social care professionals. Content analysis of the full list of behaviours was carried out. Results: Interviews were conducted with four people with dementia and six informal carers. Eight health and social care professionals took part in the consensus panel. From the interviews, consensus panel and literature review 220 behaviours were elicited, with 109 behaviours over-lapping. The interviews and consensus panel elicited 27 behaviours supplementary to the review. Those from the interviews appeared to be self-evident but highlighted deficiencies in current practice and from the panel focused largely on balancing the needs of people with dementia and family members. Behaviours were grouped into eight categories: preparing for disclosure; integrating family members; exploring the patient's perspective; disclosing the diagnosis; responding to patient reactions; focusing on quality of life and well-being; planning for the future; and communicating effectively. Conclusion: This exercise has highlighted the complexity of the process of disclosing a diagnosis of dementia in an appropriate manner. It confirms that many of the behaviours identified in the literature (often based on professional opinion rather than empirical evidence) also resonate with people with dementia and informal carers. The presence of contradictory behaviours emphasises the need to tailor the process of disclosure to individual patients and carers. Our combined methods may be relevant to other efforts to identify and define complex clinical practices for further study.This project is funded by UK Medical Research Council, Grant reference number G0300999

    Laterally Confined Volcanic Successions (LCVS); recording rift-jumps during the formation of magma-rich margins

    Seaward Dipping Reflectors (SDRs) are a characteristic feature of magma-rich margins, and represent the generation of large volumes of flood basalts at the point of continental breakup. A number of recent studies provide new insights into the emplacement and tilting of SDRs and conclude that the majority of SDRs are contained within new magmatic crust that has a close affinity to oceanic crust. However, the process by which these initial magmatic systems evolve into a fully established spreading centre remains poorly understood. Several characteristic features of magma-rich margins may be explained by the occurrence of rift-jumps during SDR emplacement, yet the cause and prevalence of such rift-jumps remain unknown. Here we constrain the 3D geometry of the continent–ocean transition in the Orange Basin, offshore South Africa. This allows us to test if, where and why such rift jumps occur. Our results demonstrate an order of along-strike segmentation previously unobserved in these settings. We demonstrate that the SDR belt is disrupted by the occurrence of a volcanic-stratigraphic package, defined as the Laterally Confined Volcanic Succession (LCVS), not previously identified on a rifted margin. We interpret this as a magmatic spreading centre that was abandoned by a subsequent rift-jump. Identification of LCVSs is important for two reasons. First, we argue that the LCVS formed via the same process as SDRs, and hence provides a unique example of SDR geometry prior to their separation onto conjugate plates. Second, as we can map out the 3D geometry of the LCVS and SDRs, we propose that rift-jumps during magma-rich margin formation may be fundamental to the establishment of a laterally continuous incipient spreading centre

    A comparison of customised and prefabricated insoles to reduce risk factors for neuropathic diabetic foot ulceration: a participant-blinded randomised controlled trial.

    UNLABELLED: BACKGROUND: Neuropathic diabetic foot ulceration may be prevented if the mechanical stress transmitted to the plantar tissues is reduced. Insole therapy is one practical method commonly used to reduce plantar loads and ulceration risk. The type of insole best suited to achieve this is unknown. This trial compared custom-made functional insoles with prefabricated insoles to reduce risk factors for ulceration of neuropathic diabetic feet. METHOD: A participant-blinded randomised controlled trial recruited 119 neuropathic participants with diabetes who were randomly allocated to custom-made functional or prefabricated insoles. Data were collected at issue and six month follow-up using the F-scan in-shoe pressure measurement system. Primary outcomes were: peak pressure, forefoot pressure time integral, total contact area, forefoot rate of load, duration of load as a percentage of stance. Secondary outcomes were patient perceived foot health (Bristol Foot Score), quality of life (Audit of Diabetes Dependent Quality of Life). We also assessed cost of supply and fitting. Analysis was by intention-to-treat. RESULTS: There were no differences between insoles in peak pressure, or three of the other four kinetic measures. The custom-made functional insole was slightly more effective than the prefabricated insole in reducing forefoot pressure time integral at issue (27% vs. 22%), remained more effective at six month follow-up (30% vs. 24%, p=0.001), but was more expensive (UK £656 vs. £554, p<0.001). Full compliance (minimum wear 7 hours a day 7 days per week) was reported by 40% of participants and 76% of participants reported a minimum wear of 5 hours a day 5 days per week. There was no difference in patient perception between insoles. CONCLUSION: The custom-made insoles are more expensive than prefabricated insoles evaluated in this trial and no better in reducing peak pressure. We recommend that where clinically appropriate, the more cost effective prefabricated insole should be considered for use by patients with diabetes and neuropathy.
