    Luxação de próteses totais da anca: causas e tratamento.

    A luxação de próteses totais da anca (PTA) continua a ser uma das complicações mais comuns e temidas. Pode ser um evento único, recidivante, precoce ou tardio. O risco de nova luxação aumenta significativamente após o primeiro episódio. Os fatores de risco são diversos: cirurgia prévia da anca, cirurgia de revisão da PTA, via de abordagem e técnica cirúrgica, experiência do cirurgião, má orientação dos componentes, conflito dos componentes ou da estrutura óssea, má colaboração do paciente. Quando ocorre luxação é fundamental um estudo sistematizado e rigoroso para avaliar a sua causa e programar a abordagem terapêutica. Normalmente a redução fechada é eficaz. Quando a redução por meios fechados não é possível, quando existe instabilidade, componentes malposicionados ou inadequada tensão dos tecidos moles, poderá recorrer-se à cirurgia. Existem várias alternativas cirúrgicas: colocação de rebordo acetabular antiluxante, reposicionamento dos componentes, resseção de osteófitos, aumento do tamanho da cabeça femoral, uso de acetábulo restritivo ou prótese bipolar. Quando estes procedimentos falham, a artroplastia tipo Girdlestone poderá estar indicada

    V Roteiro de Saúde Mental “Dignidade em Saúde Mental”, 10 a 14 de Outubro de 2016, Ponta Delgada, Açores (Poster).Enquadramento: A esquizofrenia é uma perturbação cerebral complexa e hereditária, que engloba custos significativos em termos de cuidados de saúde e implica consequências psicossociais severas ao doente e seus cuidadores (tensão familiar, diminuição do bem-estar, da autonomia e da expectativa de vida). As coortes das ilhas portuguesas tem sido um alvo de particular interesse no estudo da esquizofrenia, em parte devido a existência de uma grande prevalência de casos dentro da mesma família (aproximadamente 69%), uma taxa supe-rior ao verificado noutras populações mundiais. Considerando a heterogeneidade desta doença, a atual compreensão da esquizofrenia indica a integração dos estudos genéticos e da neurobio-logia como vias preferenciais para investigação, como forma de descrever os seus aspetos endofenotípicos e estudar sistemática e empiricamente a sua patofisiologia. As recentes aplicações de medidas neurocomportamentais focam-se particularmente na relação entre diversas variáveis da cognição e da cognição social, bem como do seu impacto funcional nestes doentes. Objetivos: Este projeto tem por objetivo caracterizar os aspetos neurocognitivos e da cognição social em 3 grupos de participantes com diferentes graus de risco genético pa-ra a esquizofrenia. Pretende-se investigar eventuais diferenças funcionais nestas 3 amostras e se o funcionamento social e neurocognitivo apresenta áreas deficitárias que possam constitui um fator de vulnerabilidade à doença e de agravamento da capacidade funcional dos indivíduos afetados. Metodologia: Três amostras serão constituídas no presente estudo: doentes diagnosticados com esquizofrenia (n= 50), familiares em primeiro grau de doentes (n=60) e controlos saudáveis (n = 200). Nos Açores, diversas famílias atualmente seguidas por um consórcio internacional de genómica (Genomic Psychiatric Cohort) serão convidadas a participar neste estudo, juntamente de participantes da população geral. O método passa pela a administração de questionários e/ou entrevistas e diversos testes baseados no desempenho, nomeadamente uma bateria neurocognitiva computadorizada e uma avaliação funcional baseada na simulação de tarefas quotidianas. Conclusões: O presente projeto visa proporcionar um contributo empírico ao caracterizar a população afetada e com risco genético elevado, visando a um estudo aprofundado dos fatores neurocognitivos e da cognição social que podem estar afetados pela esquizofrenia, propondo alvos de intervenção terapêutica potencialmente eficazes e novas abordagens psicossociais e preventivas dirigidas aos doentes e seus familiares.     The purpose of this research was to study the ratio of sweet water guava and water in making probiotic drink by using Lactobacillus casei subsp. casei R-68 isolated from dadih. The research was carried out experimentally using a Completely Randomized Design (CRD) with tree treatments and six replications. The treatments were J1S1 (ratio of sweet water guava and water 1:0), J2S2 (ratio of sweet water guava and water 3:1), J3S3 (ratio of sweet water guava and water 2:1), J4S4(ratio of sweet water guava and water 1:1), J5S5, (ratio of sweet water guava and water 1:2) and J6S6 (ratio of sweet water guava and water 1:3). The data obtained were analyzed statistically using ANOVA and DNMRT at level 5%. The results show that the best ratio of sweet water guava and water significantly influenced (P<0,05) the pH, total lactic acid, total lactic acid bacteria, total solid, hedonic test at taste as well as descriptive test of colour. Based on the results of this research, it is concluded that best ratio of sweet water guava and water to produce probiotic drink was 1 : 3 (J6S6) which pH 4.17, total lactic acid 0.73%, total lactic of acid bacteria 8.77 log CFU/ml, total solid 12.95% and hedonic test of colour 3.55, aroma 3.50 and taste 3.58 as well as rather prefered by panelists

    The purpose of this study was to determine the shelf life of instant noodles made from local corn flour with or without the addition of tapioca. This study was conducted experimentally using the Accelerated Storage Studies Method by keeping the products for 28 days at temperatures of 35ºC, 45ºC, 55ºC, the observed parameter organoleptic quality and TBA values. The results showed the self life of instant noodles made with or without the addition of tapioca were 39,552 days and 49,869 days, respectively

    Cannulation of the internal jugular vein is often necessary for the management of critically ill patients. Despite being a very common procedure and performed more and more safely, several complications still occur. Horner's Syndrome (HS) is one of those complications described before the use of ultrasound as a method of guidance. HS is caused by functional interruption of sympathetic nerve supply to the eye, leading to a classic triad of ipsilateral ptosis, miosis, and anhidrosis. We present the case of a patient, in need of emergent surgery to control the hemorrhagic focus after delivery, with a transient HS secondary to internal jugular vein cannulation under real-time ultrasound guidance.info:eu-repo/semantics/publishedVersio

    Coupling biochemistry and mechanics in cell adhesion: a model for inhomogeneous stress fiber contraction

    Biochemistry and mechanics are closely coupled in cell adhesion. At sites of cell-matrix adhesion, mechanical force triggers signaling through the Rho-pathway, which leads to structural reinforcement and increased contractility in the actin cytoskeleton. The resulting force acts back to the sites of adhesion, resulting in a positive feedback loop for mature adhesion. Here we model this biochemical-mechanical feedback loop for the special case when the actin cytoskeleton is organized in stress fibers, which are contractile bundles of actin filaments. Activation of myosin II molecular motors through the Rho-pathway is described by a system of reaction-diffusion equations, which are coupled into a viscoelastic model for a contractile actin bundle. We find strong spatial gradients in the activation of contractility and in the corresponding deformation pattern of the stress fiber, in good agreement with experimental findings.Comment: Revtex, 35 pages, 13 Postscript figures included, in press with New Journal of Physics, Special Issue on The Physics of the Cytoskeleto

    Energy averages and fluctuations in the decay out of superdeformed bands

    We derive analytic formulae for the energy average (including the energy average of the fluctuation contribution) and variance of the intraband decay intensity of a superdeformed band. Our results may be expressed in terms of three dimensionless variables: Γ/ΓS\Gamma^{\downarrow}/\Gamma_S, ΓN/d\Gamma_N/d, and ΓN/(ΓS+Γ)\Gamma_N/(\Gamma_S+\Gamma^{\downarrow}). Here Γ\Gamma^{\downarrow} is the spreading width for the mixing of a superdeformed (SD) state 0>|0> with the normally deformed (ND) states Q>|Q> whose spin is the same as 0>|0>'s. The Q>|Q> have mean level spacing dd and mean electromagnetic decay width ΓN\Gamma_N whilst 0>|0> has electromagnetic decay width ΓS\Gamma_S. The average decay intensity may be expressed solely in terms of the variables Γ/ΓS\Gamma^{\downarrow}/\Gamma_S and ΓN/d\Gamma_N/d or, analogously to statistical nuclear reaction theory, in terms of the transmission coefficients T0(E)T_0(E) and TNT_N describing transmission from the Q>|Q> to the SD band via 0|0\angle and to lower ND states. The variance of the decay intensity, in analogy with Ericson's theory of cross section fluctuations depends on an additional variable, the correlation length \Gamma_N/(\Gamma_S+\Gamma^{\downarrow})=\frac{d}{2\pi}T_N/(\Gamma_S+\Gamma^{\d ownarrow}). This suggests that analysis of an experimentally obtained variance could yield the mean level spacing dd as does analysis of the cross section autocorrelation function in compound nuclear reactions.
