La riqualificazione impiantistica ospedaliera. Il Caso Studio del reparto operatorio dell'Ospedale di San Marcello Pistoiese

Questo lavoro di Tesi Magistrale in Ingegneria Edile, che ho seguito come secondo Relatore insieme al Prof. Giuseppe Grazzini quale Primo Relatore e all’Ing. Luca Sani dello Studio Ing. Luca Sani di Firenze, Relatore esterno, si pone all’interno di una ricerca interdisciplinare che riguarda aspetti energetici ed impiantistici di edifici speciali, quali gli ospedali di cui le sale operatorie costituiscono parti e sistemi che dal punto di vista funzionale, impiantistico ed energetico risultano emblematici per complessità e criticità progettuale nonché normativa. Come si evince dal testo dell’articolo inerente la tesi, l’argomento è particolarmente complesso soprattutto perché affronta tematiche di refurbishment energetico ed impiantistico di strutture ospedaliere inserite in edifici storici spesso vincolati. Tematiche che appartengono al campo della fisica tecnica ambientale, degli impianti tecnici civili e dell’energetica. L’aspetto innovativo e cruciale di questo lavoro di Tesi è l’approccio non-convenzionale alla progettazione impiantistica e distributivo funzionale dell’ospedale che segue peraltro l’orientamento attuale del settore ospedaliero. La progettazione ospedaliera sia in termini di recupero energetico che di nuova progettazione attualmente infatti segue un processo di distribuzione ed organizzazione funzionale degli spazi nonché di progettazione degli impianti volta al rispetto di requisiti di sterilità, di risparmio energetico, uso razionale dell’energia con efficiente impiego delle risorse rinnovabili, ma anche di “umanizzazione” e quindi di qualità e comfort per gli occupanti. Il caso studio dell’Ospedale di S.Marcello Pistoiese ed in particolare il suo blocco operatorio, sono stai gli oggetti su cui poter valutare l’efficacia e la possibilità dell’approccio approccio non-convenzionale proposto

Classical cryptography as an opportunity for mathematical reasoning

Cryptography can be a tool that, enables students to consolidate knowledge and develop mathematical skills if used as a recreational or playful activity. In this paper we introduce Caesar’s cryptosystem, which is one of the simplest encryption schemes. It is very easy to crack the encryption of this system: a brute force attack allows easy recovery of the key and the plaintext. We made a simple modification that allows us to have a greater number of keys, in order to make such a brute force attack infeasible. Finally, we show a statistical method that allows us to force this cryptosystem.La crittografia può essere utilizzata come strumento che, mediante modalità ludiche, permette di consolidare conoscenze e sviluppare abilità matematiche. In questo lavoro si illustra il crittosistema di Cesare che è uno dei più semplici schemi di cifratura. Questo schema crittografico viene forzato facilmente: un attacco esaustivo permette di recuperare la chiave e il testo in chiaro. Viene inoltre presentata una semplice modifica che permette di avere un maggior numero di chiavi in modo da rendere impossibile l’attacco a forza bruta. L'esperienza Di Un Sito Web Per La Dieta Nelle Malattie Renali

Introduzione. La sensibilizzazione alle cause della malattia renale cronica e la diffusione delle conoscenze nutrizionali possono contrastare l'andamento cronico e progressivo della stessa. L'utilizzo del web è sempre più frequente come possibile mezzo di reperimento di risposte alle limitazioni imposte dalla malattia. Metodi. Sono stati esaminati altri siti web dedicati alla MRC e, sulla base delle esperienze nazionali e internazionali, è stato creato un sito web dedicato alla nutrizione in nefrologia. Risultati. A un anno di distanza dall'apertura del sito, le visite giornaliere medie sono circa 20, con picchi fino a 60-140 persone in relazione con gli inserimenti di nuove pagine. I visitatori sono per la maggior parte persone con MRC in fase conservativa oppure che hanno fatto il trapianto. Conclusioni. Nel trattamento delle nefropatie, la figura del dietista è centrale. A tutt'oggi, pochi sono i dietisti disponibili nei centri di nefrologia; il sito, oltre a fornire informazioni nutrizionali, potrebbe aumentare la sensibilità dell'opinione pubblica e delle istituzioni verso questo professionista

In vitro antibacterial activity of ceftazidime/avibactam in combination against planktonic and biofilm carbapenemase-producing Klebsiella pneumoniae isolated from blood

Abstract Objectives The aim of this study was to report on in vitro tests of antibacterial activity of ceftazidime/avibactam in combination against planktonic or biofilm KPC carbapenemase-producing Klebsiella pneumoniae (KPC-Kp), the rate of KPC-Kp blood isolates in University of Perugia Hospital over a 5-year period, and their antimicrobial susceptibility patterns. Methods The antibacterial activity of ceftazidime/avibactam in combination with other antimicrobials was assessed against planktonic and biofilm bacteria by Etest and checkerboard assay. A retrospective review of laboratory data was performed to evaluate the rate of KPC-Kp from blood samples and their antimicrobial susceptibility patterns. Results Between 2014 and 2019, 130/4241 (3.1%) KPC-Kp were identified from blood cultures. Their rate increased from 2.3% in 2014–2015 to 4.5% over the last 3 years. Overall, 4.6% (6/130) of KPC-Kp isolates were susceptible to meropenem, 65.4% (85/130) to colistin, 65.1% (84/129) to tigecycline, 34.6% (45/130) to amikacin, 36.2% (42/116) to gentamicin, 40.2% (39/97) to fosfomycin and 91.5% (65/71) to ceftazidime/avibactam. Five of six ceftazidime/avibactam-resistant KPC-Kp were isolated from patients not treated with ceftazidime/avibactam. Synergism was detected both by Etest and checkerboard assay for the combination of ceftazidime/avibactam plus meropenem against planktonic isolates, whilst lower bactericidal activity was observed in biofilm KPC-Kp isolates. Conclusions Our in vitro data suggest that the combination of ceftazidime/avibactam plus meropenem has a synergistic antibacterial activity against planktonic bacteria, whilst a lower activity was detected against biofilm, suggesting worse clinical outcomes whenever biofilm infections are present. Further analyses are required to confirm these results before extending them to clinical practice

In vitro Antimicrobial Activity of Ampicillin-Ceftriaxone and Ampicillin-Ertapenem Combinations Against Clinical Isolates of Enterococcus faecalis with High Levels of Aminoglycoside Resistance

This paper reports on the in vitro antimicrobial activity of ampicillin-ceftriaxone and ampicillin-ertapenem combinations against five strains of E. faecalis with high-level aminoglycoside resistance recovered from blood of septicemic patients. Double disk diffusion test and time killing curves were used. A bacteriostatic synergistic effect between ampicillin and ceftriaxone was detected using the disk diffusion assay for three of the five enterococcal strains studied. With the same three isolates enhanced bactericidal activity was also observed using time killing experiments. Overall, for these three strains, after 24 hr of contact, a decrease ≥ 2 log10 from the initial bacterial inoculum was registered with most ampicillin-ceftriaxone combinations, reaching with some of them a colony reduction ≥ 3 log10. This bactericidal interaction was negatively influenced increasing the bacterial inoculum. In all five isolates neither a bacteriostatic nor a bactericidal cooperation was observed for ampicillin combined with 2 mg/l of ertapenem

An Attempt at Creating Total Theatre. Scene del potere by Domenico Guaccero

When approaching Scene del potere – ‘azione scenica’ conceived and developed by Domenico Guaccero over the span of nearly a decade (1961–1968) and staged for the first and only time on 30 December 1968 at Palermo’s Teatro Biondo – there is a high risk of being overwhelmed by the preparatory materials alone. The bulk of the textual and scenic-dramaturgical preliminary materials that leads from the first draft, tentatively titled Il potere, to the 1965 Scene del potere and its Palermo debut, is of a multifold entity. Its amount of sources, formative elements and programmatic intent is both fascinating and confusing in its labyrinthine combinations. Let it suffice to point out that the textual and scenic-dramaturgical preparatory materials consist of more than one hundred pages of handwritten and typed notes, summaries, sketches, drawings, newspaper and magazine clippings. The Fondo Domenico Guaccero also preserves letters regarding the inception and outcomes of the project, a ‘skeleton’ score of the preliminary draft Il potere and a handwritten copy of 1968’s final score of Scene del potere. Among these sources we can also find a photocopy of the score with side-notes by Egisto Macchi. It is therefore no wonder that the essays written on Scene del potere have mainly focused on the groundwork and on the composer’s programmatic intent. Given such considerations, the present article aims at offering an in-depth examination of Scene del potere, seen not as its initiatory process but observed in its completion: the final version of 1968. Through a detailed study of the score, it is possible to determine whether and how the composer’s programmatic intent found its fulfilment in the ultimate outcome of his work
