    An association of boswellia, betaine and myo-inositol (EumastĂłs) in the treatment of mammographic breast density. A randomized, double-blind study

    Mammographic breast density is a recognized risk factor for breast cancer. The causes that lead to the proliferation of the glandular breast tissue and, therefore, to an increase of breast density are still unclear. However, a treatment strategy to reduce the mammary density may bring about very relevant clinical outcomes in breast cancer prevention. Myo-inositol is a six-fold alcohol of cyclohexane, has already been proved to modulate different pathways: inflammatory, metabolic, oxidative and endocrine processes, in a wide array of human diseases, including cancer and the genesis of mammary gland and breast diseases, like fibrosis, as well as metabolic and endocrine cues. Similarly, boswellic acid and betaine (three-methyl glycine) both inhibit inflammation and exert protective effects on breast physiology. Based on this scientific background, we hypothesized that a combination including, boswellic acid, betaine and myo-inositol would be able to reduce breast density working on different pathways.OBJECTIVE: Mammographic breast density is a recognized risk factor for breast cancer. PATIENTS AND METHODS: In this study, seventy-six premenopausal women were randomly assigned to the placebo and the experimental drug arms (Eumastós®) for six months. RESULTS: After 6 months of treatment, statistically significant difference between the two groups was recorded on the breast density reduction (60% vs. 9%), using mammographic as well as ultrasound examination. CONCLUSIONS: Preliminary data collected here with support the starting assumptions,that the association comprising boswellic acid, betaine and myo-inositol significantly reduces mammary density, providing the first evidence for a new and safe approach for the management of mammographic density treatment

    Sambucus nigra L. (fam. Viburnaceae) in Sicily: Distribution, Ecology, Traditional Use and Therapeutic Properties

    Sambucus nigra, the elderberry, has long been used for its medicinal properties in treating numerous diseases. Based on this traditional knowledge, its different pharmacological activities have been the focus of active research. All parts of the tree have long been used in traditional medicine, that is, the bark, the leaves, the flowers and the fruit. This study, carried out in Sicily (Italy), concerns the traditional uses of elder against human diseases. In order to trace the history of man's interaction with elder on the island, multidisciplinary research was carried out, aiming at (1) presenting a comprehensive overview of elderberry's applications and activities and (2) bridging traditional knowledge (uses and beliefs) with modern science, i.e., the most recent scientific findings in the biomedical and pharmacological fields. A rigorous literature review of scientific (and other local) reports on the elderberry tree and its application in food, health and household applications was undertaken. This article also provides a synthetic and updated picture of the ecology and distribution of S. nigra in Sicily. The elderberry is quite widespread in Sicily, yet its distribution is discontinuous. It prefers hedges, riparian woodlands, forest margins and clearings and is rather common along the watercourses flowing in the canyons of the Hyblaean Plateau, in the Madonie Mts. and in Enna province. Indeed, many old plants are often found near sacred places and rural houses, suggesting that in the past, it was extensively planted on purpose for its multiple uses. The complementary data obtained from multidisciplinary research confirm the usefulness of this approach in building a comprehensive and correct picture of the distribution of the most common woody species, for which the available knowledge is often fragmentary and imprecise

    Design and use of a Fatigue Test Machine in Plane Bending for Composite Specimens and Bonded Joints

    Polymeric and composites materials are increasingly used in industry as structural parts, so that specific information on their mechanical properties "at long term" (creep, relaxation, fatigue) are requested. The design and implementation of these components also requires that they are permanently connected together to perform certain functions. The use of adhesives in structural joints has increased over recent years thanks to the continuous development by companies which provide the market with adhesive having better and better mechanical properties. The main areas relate to the aerospace, aeronautical and automotive industries, where requirements are required for lightness and reliability. One of the main causes of out of order of the glue joints is due to the propagation of defects within the adhesive layer. Indeed, a microscopic defect could evolve because of fatigue loading and then propagate to the final break of the component. The defect may occur during the manufacturing process of the component, during the installation or for the incompatibility of the elements to connect. Delamination depends mainly on the extent of crack and on the mode of loading. Proper design and verification of structural elements cannot therefore leave apart from these aspects, so that in recent decades, in parallel with traditional methods, criteria based on fracture mechanics have been developed for the understanding of issues relating to the damage phenomena. Fatigue tests usually require several hours of application of cyclic stress to obtain an appreciable damage. These tests lead to results more or less uneven as to require the repetition of the test several times to obtain more accurate estimation of the endurance limit. All these considerations are economically incompatible with the use of particularly costly servo-hydraulic machines. Thus the development of a specific approach to fatigue tests based on the use of specific materials testing machines (FTM - Fatigue Testing Machine) becomes very important. Some ideal characteristics of such machines is described in the following: 1) adaptability to different geometries and rigidities of the specimens; 2) ease to perform various conditions of stress (alternate or pulsatory load); 3) possibility to develop studies of fatigue by the recording of the data obtained from different samples of different materials tested, to which any criterion for predicting the fatigue life can be applied; 4) ability to run tests by controlling the amount of deformation; 5) low cost of instrumentation to perform several tests simultaneously; 6) adaptability of the frequency of loading. The purpose of this chapter is to present a material testing machine for fatigue bending stress level that meets all the requirements listed above and to show the results of damage tests of composite material specimens and of the delaminations propagation in bonded joints

    Impact of pericardial effusion on cardiac mechanics in patients with dilated cardiomyopathy

    Dilated cardiomyopathy (CDM) is a degenerative disease of the myocardium accompanied by left ventricular (LV) remodeling, resulting in an impaired pump performance. Differently, pericardial effusion (PE) is a liquid accumulation in the pericardial cavity, which may inhibit blood filling of heart chambers. Clinical evidence show that PE may improve pump performance in patients with CDM. Therefore, this study aims to assess wall stress and global function of patients with CDM, PE as compared to healthy patient. These findings suggests that CDM has an important implication in the mechanical changes of LV and right ventricle by increasing wall stress and reducing pump function. Conversely, PE determines lowering myocardial fiber stress and improves global function as compared to those of CD

    This paper illustrates the botanic heritage of Parche di Bilello, a site located in the municipality of Castelvetrano. The study area hosts several woodland fragments dominated by Olea europaea var. sylvestris, Quercus suber and Quercus ilex, respectively. According to historical data, these nuclei represent the last remnants of an open forestland which covered a much wider coastal area between Mazara del Vallo and Sciacca until the end of Middle Age. Phytosociological relevés were focused on these forest nuclei, probably the most representative of south-western Sicily, which correspond to three habitats included in the 92/43 EEC Directive (9320, 9330 and 9340, respectively) and represent the final stage of three different edaphic series. Wild olive forests probably dominated on sandy calcareous soils, holm oaks prevailed on steep calcareous and N-exposed slopes. In contrast, cork oaks mostly occurred on sandy subacid soils issuing from pedogenetic processes on palaeodunes. Moreover, field surveys allowed to list 331 vascular plant taxa. Among them, Linaria multicaulis subsp. humilis, Orobanche balsensis and Serapias orientalis subsp. siciliensis are new to Trapani Province. Detailed information on the current distribution and the synecology of several plant taxa of high biogeographic and conservation interest is also provided. The study site also hosts one habitat of priority interest (6220, i.e. xerophilous Mediterranean perennial grasslands and annual swards) and two species protected by international laws, i.e. the orchid Ophrys lunulata and the lichen Teloschistes chrysophthalmus. Considering the high value of its natural heritage, this territory deserves the adoption of more effective protection measures. For this reason the authors recommend its inclusion as a new Site of Community Interest within the Sicilian Natura 2000 network

    Surgical management of ascending thoracic aortic aneurysms (aTAAs) relies on maximum diameter, growth rate, and presence of connective tissue disorders. The surgical decision however is often not considering that dissection and rupture do occur in patients who do not meet criteria for surgical repair [1,2]. In this study the authors aim to investigate the mechanical properties of aTAAs to be implemented in computational biomechanics models for a preclinical risk evaluation. Additionally, in some recent studies, some data about the biomechanical properties of the aTAAs have been reported [3], but without any relation to bicuspidal or tricuspidal aTAA.     The effects of post-pasture woody plant colonization on soil and aboveground litter carbon and nitrogen along a bioclimatic transect.

    We investigated the effects of woody plant colonization of abandoned pastures on soil and litter organic carbon (C) stocks and nitrogen (N) content along a bioclimatic transect in a semi-arid environment (Sicily, Italy). Soil samples were taken in three successional stages (grazed pasture, shrubland, forest) within each of three bioclimates (supramediterranean - \u201csupra\u201d, mesomediterranean - \u201cmeso\u201d, thermomediterranean - \u201cthermo\u201d). Organic C and N in litter and soil (0-10 cm and 10-30 cm depth) were determined, as well as soil bulk density. Especially at 0-10 cm depth, changes in C and N contents along successional stages differed among bioclimates. Soil organic carbon (SOC) stock decreased from pasture to shrubland and increased from shrubland to forest in \u201csupra\u201d, increased from pasture to shrubland and then remained stable in \u201cthermo\u201d, and was stable in \u201cmeso\u201d. Soil C/N ratio decreased with succession in \u201csupra\u201d, showed no significant trend in \u201cmeso\u201d, and increased with succession in \u201cthermo\u201d. Litter C stock increased with succession in \u201cmeso\u201d, increased from pasture to shrubland and decreased from shrubland to forest in \u201cthermo\u201d, and increased from pasture to shrubland and then remained stable in \u201csupra\u201d. Litter C/N ratio increased in \u201cthermo\u201d and \u201csupra\u201d from pasture to shrubland and from shrubland to forest, but did not change significantly with succession in \u201cmeso\u201d. The different trends in SOC among bioclimates may be caused by changes in the importance of litter input, litter decay rate and mineralization. Successional changes in \u201cmeso\u201d and \u201csupra\u201d appeared to be most affected by litter quality, while those in \u201cthermo\u201d appeared to be strongly influenced by limited litter decay due to low soil moisture and high temperature

    Usefulness of regional right ventricular and right atrial strain for prediction of early and late right ventricular failure following a left ventricular assist device implant: A machine learning approach

    Background: Identifying candidates for left ventricular assist device surgery at risk of right ventricular failure remains difficult. The aim was to identify the most accurate predictors of right ventricular failure among clinical, biological, and imaging markers, assessed by agreement of different supervised machine learning algorithms. Methods: Seventy-four patients, referred to HeartWare left ventricular assist device since 2010 in two Italian centers, were recruited. Biomarkers, right ventricular standard, and strain echocardiography, as well as cath-lab measures, were compared among patients who did not develop right ventricular failure (N = 56), those with acute–right ventricular failure (N = 8, 11%) or chronic–right ventricular failure (N = 10, 14%). Logistic regression, penalized logistic regression, linear support vector machines, and naïve Bayes algorithms with leave-one-out validation were used to evaluate the efficiency of any combination of three collected variables in an “all-subsets” approach. Results: Michigan risk score combined with central venous pressure assessed invasively and apical longitudinal systolic strain of the right ventricular–free wall were the most significant predictors of acute–right ventricular failure (maximum receiver operating characteristic–area under the curve = 0.95, 95% confidence interval = 0.91–1.00, by the naïve Bayes), while the right ventricular–free wall systolic strain of the middle segment, right atrial strain (QRS-synced), and tricuspid annular plane systolic excursion were the most significant predictors of Chronic-RVF (receiver operating characteristic–area under the curve = 0.97, 95% confidence interval = 0.91–1.00, according to naïve Bayes). Conclusion: Apical right ventricular strain as well as right atrial strain provides complementary information, both critical to predict acute–right ventricular failure and chronic–right ventricular failure, respectively

    Ecologia e distribuzione di Chamaerops humilis L. (Arecaceae) nella Sicilia nord-orientale

    I settori costieri e collinari della Sicilia nord-orientale e delle Eolie sono stati esplorati ai fini della raccolta di informazioni aggiornate sulla distribuzione dei popolamenti di palma nana e sulle caratteristiche ecologiche delle stazioni che li ospitano. I risultati delle indagini di campo sono stati integrati dalla verifica dei toponimi locali legati ai nomi dialettali della specie. Si con- ferma la totale assenza della palma nana lungo l’intero settore costiero dei Nebrodi. Dei nove popolamenti accertati, quattro si trovano alle Eolie, due sul versante ionico e tre sul versante tir- renico dei Peloritani; uno di questi ultimi, localizzato su Capo Calavà, risulta inedito. La conco- mitanza di diversi fattori di stress e l’impatto plurisecolare dell’uomo è probabilmente responsa- bile del carattere relittuale di gran parte dei popolamenti considerati, che danno vita a consorzi molto discontinui, e sono rappresentati da piccoli nuclei che formano aggruppamenti semplifi- cati sotto un profilo fisionomico e poveri in termini floristici, perlopiù confinati in contesti roc- ciosi costieri. Di contro, la recente espansione della palma nana registrata a Lipari sembra dipen- dere dalla cessazione delle attività agricole. Infine, i casi di naturalizzazione registrati a Milazzo, Tindari, Panarea, Scaletta Zanclea e Taormina derivano dal suo uso come pianta ornamentale e pongono in risalto l’urgente necessità di regolamentare la sua introduzione per evitare l’inquina- mento genetico dei nuclei autoctoni.The coastal and hilly sectors of NE Sicily and the Aeolian Archipelago have been explored, in order to obtain up-to-date information on the distribution of the populations of Chamaerops humilis and on their site conditions. In addition to the field surveys, any local toponym possibly related to the Sicilian vernacular names of the dwarf palm was checked and verified throughout the investigat- ed area. The total absence of the species along the coasts of Nebrodi Massif is confirmed. Of the 9 extant populations, 4 are located on the Aeolian Islands, 2 along the Ionian coast and 3 along the Tyrrhenian coast of Peloritani Mountains: among them, the one of Capo Calavà represents a new record. The co-occurrence of centuries-long human disturbance and several forms of envi- ronmental stresses could be the reason for the relict connotation of most of the considered popu- lations, which also are very discontinuous and consisting of small patches formed by a few indi- viduals, in simplified and species-poor assemblages, mostly found on sea-facing rocky cliffs. On the other hand, the recent spread of Chamaerops humilis in Lipari could be related to the aban- donment of agricultural practices. Finally, the naturalisation cases recorded in Milazzo, Tindari, Panarea, Scaletta Zanclea and Taormina, where the dwarf palm escaped from public and private gardens, point out the urgent need of regulating its introduction, in order to avoid the genetic pol- lution of the autochthonous populations
