    Alcoholic liver cirrhosis, more than a simple hepatic disease – A brief review of the risk factors associated with alcohol abuse

    Liver cirrhosis is a significant public health problem, being an important cause of mortality and morbidity, responsible for approximately 1.8% of the total number of deaths in Europe. Chronic alcohol consumption is the most common cause of liver cirrhosis in developed countries. Europe has the highest level of alcohol consumption among all the global World Health Organisation (WHO) regions. In this paper, we briefly review major factors leading to excessive alcohol consumption in order to draw attention to the fact that alcoholic liver cirrhosis is more than a simple liver disease, and if those risk/causal factors can be prevented, the incidence of this disease could be reduced greatly. Although excessive alcohol consumption is regarded as the cause of alcoholic liver cirrhosis, the etiology is complex, involving multiple factors that act in synchrony, and which, if prevented, could greatly reduce the incidence of this disease. Children of addicts are likely to develop an alcohol-related mental disorder; however, there is no “gene for alcoholism”

    The relationship between gut microbiota and spontaneous bacterial peritonitis in patients with liver cirrhosis - a literature review

    Gut microbiota is an essential component in the pathogenesis of liver cirrhosis and its complications. There is a direct relationship between the gut and the liver called the gutliver axis through which bacteria can reach the liver through the portal venous blood. However, it remains unclear how bacteria leave the intestine and reach the fluid collection in the abdomen. A series of mechanisms have been postulated to be involved in the pathogenesis of spontaneous bacterial peritonitis (SBP) and other complications of liver cirrhosis, including bacterial translocation, bacterial overgrowth, altered intestinal permeability and dysfunctional immunity. The hepatic function may also be affected by the alteration of intestinal microbiota composition. Current treatment in SBP is antibiotic therapy, but lately, probiotics have been the useful treatment suggested to improve the intestinal barrier and prevent bacterial translocation. However, studies are contradictory regarding their usefulness. In this review, we will summarize the literature data on the pathogenesis of spontaneous bacterial peritonitis concerning the existence of a relationship with the microbiota and the useful use of probiotics

    New oral anticoagulants and their reversal agents

    Atrial fibrillation is a commonly encountered pathology in medical practice, and its prevalence has shown a continuous rise over the past years. Atrial fibrillation has a significant impact on patients\u27 quality of life, not only due to the standard anticoagulant treatment with vitamin K antagonists that require close monitoring and dose adjustment, but also due to the fragile equilibrium between hemorrhagic and thrombotic risks. The introduction of new oral anticoagulants (NOACs) in the treatment guidelines for atrial fibrillation has improved the quality of life, as NOACs do not require close monitoring or dose adjustments. However, even if the safety profile of the NOACs regarding the hemorrhagic risk is superior to vitamin K antagonists, the problem raised by an unexpected hemorrhage (e.g. severe hemorrhage after an accident) and the need for efficient hemostasis in a chronic anticoagulated patient has remained unsolved. To find a solution for this problem, reversal agents for NOACs have been developed and tested, and two of them, idarucizumab and andexanet-alpha, have already been approved by the FDA, thus making NOACs increasingly appealing as a choice of anticoagulation treatment

    Does sex of the patient play a role in survival for MSI colorectal cancer?

    Microsatellite instability (MSI) is a feature of colorectal tumors that develops as a result of inactivation of the DNA mismatch repair system. It is found in about 15% of all colorectal cancers and is an important prognostic molecular marker when assessing patients with colorectal cancer. It can influence prognosis and treatment decisions in both the advanced and early stages. Although in early stages this marker suggests a favorable prognosis and presents an important argument against adjuvant treatment in stage II disease, in metastatic stages it no longer associated with such an optimistic outcome. The present trial is a prospective, single-center study which included 122 colorectal cancer patients who were tested for MSI using immunohistochemistry. The trial included patients with stage II to IV colorectal cancer, treated in the Prof. Dr. Agrippa Ionescu Emergency Hospital, Bucharest, Romania. Follow-up data were collected during a 24-month period. The study attempted to determine whether differences exist in overall survival for MSI (microsatellite instability) vs. MSS (microsatellite stable) colorectal cancer and to ascertain whether sex of the patient influences prognosis in MSI patients, irrespective of stage or treatment. Results demonstrated no significant differences in survival for MSI vs MSS colorectal patients, and patients’ gender proved not to influence the outcome in MSI patients

    Data Driven Classification Using fMRI Network Measures: Application to Schizophrenia

    Using classification to identify biomarkers for various brain disorders has become a common practice among the functional MR imaging community. Typical classification pipeline includes taking the time series, extracting features from them, and using them to classify a set of patients and healthy controls. The most informative features are then presented as novel biomarkers. In this paper, we compared the results of single and double cross validation schemes on a cohort of 170 subjects with schizophrenia and healthy control subjects. We used graph theoretic measures as our features, comparing the use of functional and anatomical atlases to define nodes and the effect of prewhitening to remove autocorrelation trends. We found that double cross validation resulted in a 20% decrease in classification performance compared to single cross validation. The anatomical atlas resulted in higher classification results. Prewhitening resulted in a 10% boost in classification performance. Overall, a classification performance of 80% was obtained with a double-cross validation scheme using prewhitened time series and an anatomical brain atlas. However, reproducibility of classification within subjects across scans was surprisingly low and comparable to across subject classification rates, indicating that subject state during the short scan significantly influences the estimated features and classification performance

    Characterization of defect structures in nanocrystalline materials by X-ray line profile analysis

    X-ray line profile analysis is a powerful alternative tool for determining dislocation densities, dislocation type, crystallite and subgrain size and size-distributions, and planar defects, especially the frequency of twin boundaries and stacking faults. The method is especially useful in the case of submicron grain size or nanocrystalline materials, where X-ray line broadening is a well pronounced effect, and the observation of defects with very large density is often not easy by transmission electron microscopy. The fundamentals of X-ray line broadening are summarized in terms of the different qualitative breadth methods, and the more sophisticated and more quantitative whole pattern fitting procedures. The efficiency and practical use of X-ray line profile analysis is shown by discussing its applications to metallic, ceramic, diamond-like and polymer nanomaterials

    Efficiency of the national drug pacovirin in the treatment of infectious mononucleosis in children

    National Center of Public Health, the Republic of Moldova, Municipal Infectious Diseases Hospital for Children, Chisinau, Toma Ciorba Hospital of Infectious Diseases, Chisinau, the Republic of MoldovaBackground: In the article are presented the clinical results of the treatment of children with infectious mononucleosis where the basic therapy has been combined with the administration of the local drug pacovirin which has antiviral, immunomodulatory, interferonogenic and antioxidant activity. Material and methods: Pacovirin has been administered for one month by 50 mg capsules 2 times a day per os. The object of study has been presented by 175 patients (84 patients in the experimental group and 91 ones in the comparison group) and the records of the patients surveillance (form 003/e), containing the clinical diagnosis of infectious mononucleosis with Epstein-Barr virus and hepatitis syndrome. The duration of the treatment has been one month, the doses being age-appropriate and applied 2 times a day. Results: The administration of pacovirin as a medicinal remedy in addition to the traditional treatment, in accordance with the method proposed by us, has led to the more intensive normalization of clinical and laboratory indices, decrease of the pathological process and the rapid recovery of the patients, as well as the more intensive normalization of biochemical indices and significant reduction of the duration of the main clinical signs in the experimental group compared with the control group. Conclusions: The drug pacovirin has a positive effect on the main clinical signs of general toxic syndrome and digestive disorders, which have disappeared in a significant number of patients after the treatment. In most patients in the experimental group a decrease in the size of liver and spleen has been observed through palpatory evaluation by the end of the treatment course with pacovirin. In the the experimental group a positive dynamics of paraclinical indices (Alt, AST and γGTP) has been evident

    DEAR project: Lunar dust surface interactions, risk and removal investigations

    The DEAR project (Dusty Environment Application Research) investigates the interaction between lunar regolith and surfaces and components relevant for lunar exploration. Based on the TUBS regolith simulant which is representative in chemistry, size and shape properties to Moon soils to study the regolith transport, adhesion and strategies for cleaning. The regolith simulant will be applied to thermal, structural, optical sensor, sealing and other astronautic systems, providing input for requirements, justification and verification. The key applications are split in human space flight regolith investigations, wrinkled surface with random movement and hardware surfaces, flat material defined movement. The paper provides an overview of the DEAR project including a discussion of the first results, in particular vibration, shock and micro-vibration on regolith bearing surfaces. The investigation shall enable better understand the regolith layers interaction and the release mechanism, as well as potential cross contamination and cleaning strategies. The research is complemented by simulation of the regolith motion as parameter surface plasma interactions. The project is funded and supported by the European Space Agency (ESA). DEAR specifically addresses the development and testing of lunar dust removal strategies on optics, mechanisms and human space flight hardware (e.g., space suits). As the Moons regolith is known to be highly abrasive, electrically chargeable, and potentially chemically reactive, lunar dust might reduce the performance of hardware, such as cameras, thermal control surfaces and solar cells. The dust can cause malfunction on seals for on/off mechanisms or space suits. Of particular interest are risk assessment, avoidance, and cleaning techniques such as the use of electric fields to remove lunar dust from surfaces. Representative dust (e.g., regolith analogues of interesting landing sites) will be used in a dedicated test setup to evaluate risks and effects of lunar dust. We describe designs and methods developed by the DEAR consortium to deal with the regolith-related issues, in particular an electrode design to deflect regolith particles, cleaning of astronautical systems with CO2, design of a robotic arm for the testing within the DEAR chamber, regolith removal via shock, and regolith interaction with cleanroom textile
