    Cardiogenic shock following administration of propofol and fentanyl in a healthy woman: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Cardiogenic shock is very uncommon in healthy people. The differential diagnosis for patients with acute heart failure in previously healthy hearts includes acute myocardial infarction and myocarditis. However, many drugs can also depress myocardial function. Propofol and fentanyl are frequently used during different medical procedures. The cardiovascular depressive effect of both drugs has been well established, but the development of cardiogenic shock is very rare when these agents are used.</p> <p>Case presentation</p> <p>After a minor surgical intervention, a 32-year-old Caucasian woman with no significant medical history went into sudden hemodynamic deterioration due to acute heart failure. An urgent echocardiogram showed severe biventricular dysfunction and an estimated left ventricular ejection fraction of 20%. Extracorporeal life support and mechanical ventilation were required. Five days later her ventricular function had fully recovered, which allowed the progressive withdrawal of medical treatment. Prior to her hospital discharge, cardiac MRI showed neither edema nor pathological deposits on the delayed contrast enhancement sequences. At her six-month follow-up examination, the patient was asymptomatic and did not require treatment.</p> <p>Conclusion</p> <p>Although there are many causes of cardiogenic shock, the presence of abrupt hemodynamic deterioration and the absence of a clear cause could be related to the use of propofol and fentanyl.</p

    Biostratigraphy or biochronology? Lessons from the Early and Middle Miocene small Mammal Events in Europe

    Since the proposition in 1975 of the European Neogene Mammal (MN) scale by Pierre Mein, the amount of taxonomical, stratigraphical and chronological information around Europe has increased exponentially. In this paper, the stratigraphical schemes of three of the best studied areas for the Lower and Middle Miocene, the Aragonian type area in Spain and the Upper Freshwater Molasse from the North Alpine Foreland Basin in Switzerland and Bavaria, are compared. The correlation of their local biostratigraphies are discussed. Sixteen rodent’s events are studied and ranked in the three areas according to their local biostratigraphy. This study shows, and quantifies for the first time, the significant asynchronies of the different included rodent events. The MN-system is discussed in the light of those results. In accordance, we propose that it is still useful but only in a biochronological way, as a sequence of time-ordered reference localities allowing coarse long-distance correlations. In order to obtain better temporal resolution, this system has to be combined with local biostratigraphies that are well calibrated to the time scale, implementing the information about synchrony and diachrony of mammal events in different areas

    Impact of late presentation of HIV infection on short-, mid- and long-term mortality and causes of death in a multicenter national cohort : 2004-2013

    To analyze the impact of late presentation (LP) on overall mortality and causes of death and describe LP trends and risk factors (2004-2013). Cox models and logistic regression were used to analyze data from a nation-wide cohort in Spain. LP is defined as being diagnosed when CD4 < 350 cells/ml or AIDS.
