Health-related quality of life in patients with pulmonary arterial hypertension
BACKGROUND: Improved outcomes with expanding treatment options for patients with pulmonary arterial hypertension present the opportunity to consider additional end-points in approaching therapy, including factors that influence health-related quality of life. However, comparatively little is known about health-related quality of life and its determinants in patients with pulmonary arterial hypertension. METHODS: Health-related quality of life was evaluated in a cross sectional study of 155 outpatients with pulmonary arterial hypertension using generic and respiratory-disease specific measurement tools. Most patients had either World Health Organization functional Class II or III symptoms. Demographic, hemodynamic and treatment variables were assessed for association with health-related quality of life scores. RESULTS: Patients with pulmonary arterial hypertension suffered severe impairments in both physical and emotional domains of health-related quality of life. Patients with idiopathic ("primary") pulmonary arterial hypertension had the best, and those with systemic sclerosis the worst health-related quality of life. Greater six-minute walk distance correlated with better health-related quality of life scores, as did functional Class II versus Class III symptoms. Hemodynamic measurements, however, did not correlate with health-related quality of life scores. No differences in health-related quality of life were found between patients who were being treated with calcium channel antagonists, bosentan or continuously infused epoprostenol at the time of quality of life assessment. CONCLUSION: Health-related quality of life is severely impaired in patients with pulmonary arterial hypertension and is associated with measures of functional status. Specific associations with impaired health-related quality of life suggest potential areas for targeted intervention
Microscopic dynamics in liquid metals: the experimental point of view
The experimental results relevant for the understanding of the microscopic
dynamics in liquid metals are reviewed, with special regards to the ones
achieved in the last two decades. Inelastic Neutron Scattering played a major
role since the development of neutron facilities in the sixties. The last ten
years, however, saw the development of third generation radiation sources,
which opened the possibility of performing Inelastic Scattering with X rays,
thus disclosing previously unaccessible energy-momentum regions. The purely
coherent response of X rays, moreover, combined with the mixed
coherent/incoherent response typical of neutron scattering, provides enormous
potentialities to disentangle aspects related to the collectivity of motion
from the single particle dynamics.
If the last twenty years saw major experimental developments, on the
theoretical side fresh ideas came up to the side of the most traditional and
established theories. Beside the raw experimental results, therefore, we review
models and theoretical approaches for the description of microscopic dynamics
over different length-scales, from the hydrodynamic region down to the single
particle regime, walking the perilous and sometimes uncharted path of the
generalized hydrodynamics extension. Approaches peculiar of conductive systems,
based on the ionic plasma theory, are also considered, as well as kinetic and
mode coupling theory applied to hard sphere systems, which turn out to mimic
with remarkable detail the atomic dynamics of liquid metals. Finally, cutting
edges issues and open problems, such as the ultimate origin of the anomalous
acoustic dispersion or the relevance of transport properties of a conductive
systems in ruling the ionic dynamic structure factor are discussed.Comment: 53 pages, 41 figures, to appear in "The Review of Modern Physics".

End-stage renal disease preceded by rapid declines in kidney function: a case series
<p>Abstract</p> <p>Background</p> <p>Few studies have defined alternate pathways by which chronic kidney disease (CKD) patients transition into end-stage renal disease (ESRD).</p> <p>Methods</p> <p>We studied all consecutive patients initiated on maintenance hemodialysis or peritoneal dialysis over several years at two dialysis units in Northern California. Rapid decline in kidney function was considered to have occurred if a patient was documented to have estimated GFR > 30 ml/min/1.73 m<sup>2 </sup>within three months prior to the initiation of chronic dialysis.</p> <p>Results</p> <p>We found that 8 out of 105 incident chronic dialysis patients one dialysis unit (7.6%; 95% confidence interval 3.4-14.5%) and 9 out of 71 incident patients at another (12.7%, 95% CI 6.0%-22.7%) suffered rapid decline in kidney function that was the immediate precipitant for the need for permanent renal replacement therapy. All these patients started hemodialysis and all relied on catheters for vascular access. Documentation submitted to United States Renal Data System did not fully reflect the health status of these patients during their "pre-ESRD" period.</p> <p>Conclusions</p> <p>A sizeable minority of ESRD cases are preceded by rapid declines in kidney function. The importance of these periods of rapid decline may have been under-appreciated in prior studies of the natural history of CKD and ESRD.</p
Acute kidney disease and renal recovery : consensus report of the Acute Disease Quality Initiative (ADQI) 16 Workgroup
Consensus definitions have been reached for both acute kidney injury (AKI) and chronic kidney disease (CKD) and these definitions are now routinely used in research and clinical practice. The KDIGO guideline defines AKI as an abrupt decrease in kidney function occurring over 7 days or less, whereas CKD is defined by the persistence of kidney disease for a period of > 90 days. AKI and CKD are increasingly recognized as related entities and in some instances probably represent a continuum of the disease process. For patients in whom pathophysiologic processes are ongoing, the term acute kidney disease (AKD) has been proposed to define the course of disease after AKI; however, definitions of AKD and strategies for the management of patients with AKD are not currently available. In this consensus statement, the Acute Disease Quality Initiative (ADQI) proposes definitions, staging criteria for AKD, and strategies for the management of affected patients. We also make recommendations for areas of future research, which aim to improve understanding of the underlying processes and improve outcomes for patients with AKD

Randomized Clinical Trial of Aspirin and Simvastatin for Pulmonary Arterial Hypertension
Background—Pulmonary arterial hypertension (PAH) is a progressive disease that causes exercise limitation, heart failure, and death. We aimed to determine the safety and efficacy of aspirin and simvastatin in PAH.
Methods and Results—We performed a randomized, double-blind, placebo-controlled 2×2 factorial clinical trial of aspirin and simvastatin in patients with PAH receiving background therapy at 4 centers. A total of 92 patients with PAH were to be randomized to aspirin 81 mg or matching placebo and simvastatin 40 mg or matching placebo. The primary outcome was 6-minute walk distance at 6 months. Sixty-five subjects had been randomized when the trial was terminated by the Data Safety and Monitoring Board after an interim analysis showed futility in reaching the primary end point for simvastatin. After adjustment for baseline 6-minute walk distance, there was no significant difference in the 6-minute walk distance at 6 months between aspirin (n=32) and placebo (n=33; placebo-corrected difference −0.5 m, 95% confidence interval −28.4 to 27.4 m; P=0.97) or between simvastatin (n=32) and placebo (n=33; placebo-corrected difference −27.6 m, 95% confidence interval −59.6 to 4.3 m; P=0.09). There tended to be more major bleeding episodes with aspirin than with placebo (4 events versus 1 event, respectively; P=0.17).
Conclusions—Neither aspirin nor simvastatin had a significant effect on the 6-minute walk distance, although patients randomized to simvastatin tended to have a lower 6-minute walk distance at 6 months. These results do not support the routine treatment of patients with PAH with these medications
Prognostic Significance of Biomarkers in Pulmonary Arterial Hypertension
Rationale: Pulmonary arterial hypertension (PAH) is a rare progressive disease of the pulmonary vasculature that is characterized by endothelial dysfunction, inflammation, and right ventricular dysfunction. Objectives: The main objective was to determine whether endothelial, inflammatory, and cardiac biomarkers would be associated with the World Health Organization functional assessment and survival in patients with PAH. Methods: We performed a retrospective cohort study of patients with PAH enrolled in the Randomized Clinical Trial of Aspirin and Simvastatin for Pulmonary Arterial Hypertension (ASA-STAT). Biomarkers (N-terminal fragment of pro-BNP [NT-pro-BNP], von Willebrand factor [vWF], soluble P selectin, C-reactive protein, total and high-density lipoprotein cholesterol, triglycerides, tumor necrosis factor, IL-6, β-thromboglobulin, and thromboxane B2) were measured at baseline. Patients from the study were followed until lung transplantation, death, or August 1, 2013. Ordinal logistic regression and Cox regression analyses were performed. Measurements and Main Results: Sixty-five patients with PAH were enrolled. The mean age was 51 years, and 86% were women. Higher vWF activity, lower high-density lipoprotein cholesterol, and higher thromboxane B2 levels were associated with worse World Health Organization functional class after adjustment for age, sex, and etiology of PAH. Higher NT-pro-BNP levels, lower vWF activity, and lower total cholesterol were associated with an increased risk of death or lung transplant after adjustment for age, sex, etiology of PAH, and 6-minute-walk distance. Conclusions: In patients with PAH, lower vWF activity and cholesterol levels and higher NT-pro-BNP levels at baseline were associated with an increased risk of death or transplantation. Recommendations for the clinical management of patients receiving macitentan for pulmonary arterial hypertension (PAH): A Delphi consensus document
In patients treated with macitentan (Opsumit®, Actelion Pharmaceuticals Ltd., Basel, Switzerland) for pulmonary arterial hypertension (PAH), prevention and/or effective management of treatment-related adverse events may improve adherence. However, management of these adverse events can be challenging and the base of evidence and clinical experience for macitentan is limited. In the absence of evidence, consensus recommendations from physicians experienced in using macitentan to treat PAH may benefit patients and physicians who are using macitentan. Consensus recommendations were developed by a panel of physicians experienced with macitentan and PAH using a modified Delphi process. Over three iterations, panelists developed and refined a series of statements on the use of macitentan in PAH and rated their agreement with each statement on a Likert scale. The panel of 18 physicians participated and developed a total of 118 statements on special populations, add-on therapy, drug-drug interactions, warnings and precautions, hospitalization and functional class, and adverse event management. The resulting consensus recommendations are intended to provide practical guidance on real-world issues in using macitentan to treat patients with PAH
Comparative Demography of the Spider Mite, Oligonychus afrasiaticus, on four Date Palm Varieties in Southwestern Tunisia
The date palm mite, Oligonychus afrasiaticus (McGregor) (Acari: Tetranychidae), is a serious pest of palm date fruits. Life cycle, fecundity, and longevity of this mite were studied on fruits of four date palms, Phoenix dactylifera L. (Arecales: Arecaceae)(varieties: Deglet Noor, Alig, Kentichi, and Besser), under laboratory conditions at 27 = 1 °C, 60 ± 10% RH. Total development time of immature female was shorter on Deglet Noor fruits than on the other cultivars. O. afrasiaticus on Deglet Noor had the highest total fecundity per female, while low fecundity values occurred on Besser. The comparison of intrinsic rates of natural increase (rm), net reproductive rates (Ro), and the survival rates of immature stage of O. afrasiaticus on the host plants suggests that O. afrasiaticus performs better on Deglet Noor fruits. The mite feeding on Alig showed the lowest intrinsic rate of natural population increase (rm = 0.103 day 1). The estimation of difference in susceptibility of cultivars to O. afrasiaticus is crucial for developing efficient pest control programs. Indeed, less susceptible cultivars can either be left unsprayed or sprayed at low threshold
Quasi-Elastic Scattering in the Inclusive (He, t) Reaction
The triton energy spectra of the charge-exchange C(He,t) reaction
at 2 GeV beam energy are analyzed in the quasi-elastic nucleon knock-out
region. Considering that this region is mainly populated by the charge-exchange
of a proton in He with a neutron in the target nucleus and the final proton
going in the continuum, the cross-sections are written in the distorted-wave
impulse approximation. The t-matrix for the elementary exchange process is
constructed in the DWBA, using one pion- plus rho-exchange potential for the
spin-isospin nucleon- nucleon potential. This t-matrix reproduces the
experimental data on the elementary pn np process. The calculated
cross-sections for the C(He,t) reaction at to triton
emission angle are compared with the corresponding experimental data, and are
