Minicraniotomia supra-orbitária superciliar no tratamento de meningiomas na fossa craniana anterior e para-selares
The improvement of surgical techniques as well as the introduction of new surgical instruments promoted the use of keyhole craniotomies in neurosurgery. We evaluated the technical aspects of the supra-orbital keyhole approach considering the indications, limitations, and complications of this approach to treat anterior cranial fossa and parasellar meningiomas. Twenty-four patients (21 females; mean age, 53±8.6 years) operated on between 2002 and 2006 through a supra-orbital eyebrow approach were studied. Maximal tumor diameter ranged from 1.6 to 6 cm. Gross total resection was done in 20 (83.3%). All tumors were histologically benign. Two patients (8%) experienced CSF rinorhea and another two patients suffered transitory diabetes insipidus (8%). One patient experienced transitory hemiparesis. There was one case of meningitis and one mortality. Follow-up ranged between 6 to 66 months (mean 31.5±20.1 months), with no recurrence. The supra-orbital keyhole craniotomy is a useful minimally invasive approach to treat selected anterior fossa and parasellar meningiomas.A evolução técnica e a introdução de instrumentais cirúrgicos mais delicados proporcionaram o uso de craniotomias menores no tratamento de patologias intracranianas. Avaliamos os aspectos técnicos da minicraniotomia supra-orbitária superciliar, considerando as indicações, limitações e complicações no tratamento de meningiomas na fossa craniana anterior e para-selares. Vinte e quarto pacientes (21 mulheres; idade média, 53±8,6 anos) operados entre 2002 e 2006 foram estudados. O diâmetro tumoral máximo variou de 1,6 a 6 cm. Ressecção total foi obtida em 20 (83,3%). Todos os tumores eram histologicamente benignos. Dois pacientes (8%) apresentaram fistula liquórica pós-operatória e outros dois diabetes insipido transitórioa (8%). Um paciente evoluiu com hemiparesia transitória. Houve um caso de meningite e um de evolução fatal. O seguimento variou de 6 a 66 meses (média 31,5±20,1 meses), não houve recidiva. Olfactory groove meningioma
Olfactory groove meningiomas compromise approximately 8-18% of intracranial meningiomas and harbor frequently large sizes when diagnosticated. We report 13 consecutive cases operated on Hospital São Paulo and Hospital Professor Edmundo Vasconcelos between 1995 and 2003. These patients were retrospectively studied regarding clinical preservation, surgical results and complications. The patients underwent uni or bifrontal craniotomies. There were nine complete resections including duramater and infiltrate bone and four patients with tumor removal and duramater coagulation. One patient died on post-operative period due to brain infarction and three patients had cerebrospinal fluid fistula that resolved. There were no symptomatic recurrence on the follow-up period of 3.3 years(range 11 m to 8 years). With current microsurgical techniques, these tumors can be resected safely with low morbidity.Meningeomas da goteira olfatória representam 8-18% dos meningeomas intracranianos e geralmente são diagnosticados quando alcançam grande tamanho. Relatamos uma série de 13 casos consecutivos de meningeomas de goteira olfativa operados nos Hospitais São Paulo (UNIFESP) e Professor Edmundo Vasconcelos no perÃodo de 1995 a 2003, estudados retrospectivamente quanto ao quadro clÃnico, resultados cirúrgicos e complicações. Os pacientes foram submetidos a ressecção cirúrgica destes tumores pela via subfrontal, em 9 casos a ressecção foi completa incluindo dura-máter e osso infiltrado por tumor e em 4 a dura-máter foi somente coagulada. Um paciente morreu devido a infarto cerebral e três pacientes evoluÃram com fÃstulas liquóricas. Não houve recorrência sintomática no perÃodo que variou de 11 meses a 8 anos. Thalamic intracerebral schwannoma: case report
The intracranial schwannomas cover about 8% of all the brain tumors, although, those localized inside the encephalic parenchyma are rare only 55 cases being reported in the literature. The histhopatologic diagnosis is based on the cellular type and arrangement common to the nervous sheat tumors, the immunohistochemical findings (GFAP, S-100 protein, EMA) and electronmicroscopic findings as well. The clinical presentation related to the intraparenchimatous schwannoma is variable, depending on its localization, the image diagnosis does not show any typical aspect. The surgical treatment is curative once a complete extirpation is obtained. These tumors are benign but one case reported presented malignant signs. We present a patient that had a thalamic schwannoma with histological confirmation. We found no case with this localization in the literature.Os schwanomas intracranianos compreendem cerca de 8% de todos os tumores cerebrais porém, aqueles classificados como intra-axiais são mais raros, havendo somente 55 casos relatados na literatura. O diagnóstico é feito pelo tipo e arranjo celular comum aos tumores de bainha nervosa, pela imuno-histoquÃmica (GFAP, proteÃna S-100, EMA) e pela microscopia eletrônica. Sua apresentação clÃnica é variável, dependendo de sua localização; suas caracterÃsticas neurorradiológicas não se apresentam de maneira tÃpica. O tratamento cirúrgico, com exérese total da lesão, propicia a cura. Possuem caracteres benignos; um dos casos relatados na literatura, possuÃa entretanto caracteres de malignidade. Apresentamos um caso de schwanoma cerebral intra-axial supratentorial localizado na região talâmica esquerda, com confirmação diagnóstica anatomopatológica e imuno-histoquÃmica. Mandibular trigeminal schwannoma: case report
We report one case of peripheral trigeminal schwannoma originated from the mandibular branch (V3) and located inside the infratemporal and pterygopalatine fossae with slight intracranial extension. The tumor was completelly removed by a transmaxilar approach. The revision of the literature showed to be a very rare tumor in this location.Relatamos um caso de schwanoma do ramo mandibular do nervo trigêmeo, localizado na fossa infratemporal e ptérigo-palatina, com pequena extensão intracraniana e que foi totalmente extirpado por via transmaxilar. Giant intracranial aneurysm in three years old boy: case report
Cerebral aneurysms are rare in the pediatric age group and differ from adults with aneurysms in size, localization and incidence. We report a 3-year-old boy with giant middle cerebral artery aneurysms who presented with subarachnoid hemorrhage. The patient was submitted to surgical treatment and the postoperative period was uneventful.Aneurismas cerebrais são raros na faixa etária pediátrica. A apresentação destas lesões difere significativamente em relação á população adulta no que se refere a tamanho, localização e incidência. Relatamos caso de menimo de três anos de idade que apresentou quadro de hemorragia subaracnóidea sem história de infecções ou traumatismos prévios sendo, posteriormente, diagnosticado aneurisma gigante localizado na artéria cerebral média. Meningoceles intratorácicas gigantes associadas a neurofibromatose tipo I: relato de caso
BACKGROUND: Intrathoracic meningocele is a rare pathology, almost always associated with neurofibromatosis type I and with a few cases related in the literature. In the majority of cases cysts are small or asymptomatic, and the surgery is indicated when big or symptomatic cysts are present. We report a case of giant intrathoracic cysts surgically extirpated through out thoracotomy. CASE: A 59-year-old male with familiar Von Recklinghausen's disease which developed thoracic radicular pain after a fall. On examination he presented some difficulty in walking fast and dyspneia on small efforts. The chest plain x-ray showed the presence of 3 huge left side intrathoracic cysts (10 to 15cm). The patient was submitted to a surgical treatment and complete extirpation of the cysts was performed through a left side thoracotomy. During the surgery a fourth smaller cyst was detected and also extirpated. Evolution was uneventful and the patient remains well in these last 12 years. This finding of intrathoracic cysts related to neurofibromatosis type I is rare and is probably unique in the literature the presence of 4 huge cysts in one side of the thorax.Meningocele intratorácica é patologia rara e, quando presente, está quase sempre associada a neurofibromatose tipo I. Há poucos casos descritos na literatura e em sua maioria destes são de cistos pequenos ou assintomáticos, sendo a cirurgia indicada quando são grandes ou sintomáticos. Effect of racial crossing on the seminal parameters of rams submitted to heat stress
The aim of this study was to evaluate the effect of racial crossing on seminal parameters of eight Santa Inês and crossbred (Santa Inês x Dorper) rams submitted to heat stress, and to monitor the return of these parameters to previously reported. Before to place the insulation bags, two collects of semen through electroejaculation were performed. The insulation pouches were made with double-layer plastic, internally lined with cotton, and fixed around the spermatic funiculus and scrotum with adhesive tape and bandage remaining on the testes of the animals for seven days. The first collect was performed on the day that the pouches were taken (day 0) and thereafter, every seven days, totalizing 15 measurements. Data were submitted to analysis of variance (ANOVA). The analyzed variables were subjected to Dunnett test at 5% probability to compare the values obtained before treatment with those obtained in the following days. In this study it was found that the animals restored normal seminal parameter after the insulation effects, however, the return rate differed slightly among the studied breeds. The crossbred animals restored the seminal patterns, on average, a week before Santa Inês. It is concluded that the racial crossing influences the semen parameters of rams submitted to heat stress.
