    Association between Vitamin D levels and breast cancer risk using a Mendelian randomization approach

    Several studies have been done to evaluate the association of vitamin D deficiency and breast cancer risk. There is controversy in the literature about this association. Epidemiologic studies have investigated whether people with higher vitamin D intakes or higher blood levels of vitamin D have lower risks of specific cancers. The results of these studies have been inconsistent, possibly because of the challenges in carrying out such studies. Several randomized trials of vitamin D intake have been carried out, but these were designed to assess bone health or other non-cancer outcomes. Although some of these trials have yielded information on cancer incidence and mortality, the results need to be confirmed by additional research because the trials were not designed to study cancer specifically. Current evidence regarding vitamin D and breast cancer was reviewed to inform clinical practice and identify potential research directions. The evidence was inconsistent, inconclusive as to causality, and insufficient to inform nutritional requirements. Current evidence is sufficient to support further study of factors influencing 25(OH)D levels, associations between 25(OH)D levels and breast cancer in premenopausal and Black women, moderate dose (≤2,000 IU D3/day) supplemental vitamin D use and breast cancer incidence, and observational studies evaluating whether a threshold higher 25(OH)D level is associated with adverse clinical outcome in women with breast cancer. Before routine clinical application of any strategies targeting vitamin D status for breast cancer prevention or therapy are undertaken, the limitations of the current evidence should be considered.N

    Connectivity Patterns for Direct Developing Invertebrates in Fragmented Marine Habitats: Fish Farms Fouling as Source Population in the Establishment and Maintenance of Local Metapopulations

    Artificial structures can be considered as high spatially structured habitats in the marine pelagic system, where patch connectivity would be strongly dependent on the exchange of larvae or dispersing individuals. Fish-farms located offshore may alter ecological connectivity, modifying trophic resources, and species dispersal among patches. High population densities of invertebrates can be found associated with fish-farm fouling communities, which can act as a seed source, contributing to the patterns of connectivity through individuals exchange between subpopulations or with sink populations. A field experiment was performed to analyse the role of fish-farms in the colonisation of new uninhabited habitats (floating experimental units) located at different positions relative to the fish farm and the main current, containing artificial habitats with and without feed pellets similar to those used in the fish farm. Amphipods were used as example of direct developing invertebrates for studying dispersing individuals from the fish farm to the new habitats. The richest and most abundant populations in this study were found close to and downstream of the fish farm, surpassing 1,000 amphipods at their maximum. Moreover, some floating habitats located more than 2 km from the fish farm were colonised in only 15 days. Thus, the role of fish farms has been shown to extend beyond a ‘stepping-stones’ effect in species dispersal, and have an additional effect on ecological connectivity by increasing population sizes and acting as population source. Our study aims to provide recommendations for coastal zone management in order to predict potential spread from fish farms to other platforms in the future and promote solutions related to interactions and consequences of connectivity within and between marine facilities

    Injury incidence in a Spanish sub-elite professional football team: A prospective study during four consecutive seasons

    The aim of this study was to investigate the injury incidence and injury characteristics of a Spanish sub-elite professional football team during four consecutive seasons. A team was followed prospectively from the season 2003-2004 to 2006-2007 and individual player exposure and time loss injuries were recorded during all club training sessions and matches. A total of 313 time-loss injuries were recorded. The mean injury incidence was 10.9 injuries/1000 hours (5.2 injuries/1000 training hours and 44.1 injuries/1000 match hours). The injury incidence during competitive matches was higher (p 28 days absence) was 0.4 injuries/1000 hours. The thigh was the most commonly (35%) injured region and caused 29% of all competitive match absence. Muscle injuries in the four main groups of the lower limbs (hamstrings, adductors, quadriceps and calf muscles) caused 43% of competitive match unavailability. The results of this study show that the risk to sustain a major injury in the course of the season was low for sub-elite footballers in comparison to elite players. Thigh strains were the first cause of absence in competition due to injury

    Comparison of starts and turns of national and regional level swimmers by individualized-distance measurements

    The aim of this study was to compare the race characteristics of the start and turn segments of national and regional level swimmers. In the study, 100 and 200-m events were analysed during the finals session of the Open Comunidad de Madrid (Spain) tournament. The “individualized-distance” method with two-dimensional direct linear transformation algorithm was used to perform race analyses. National level swimmers obtained faster velocities in all race segments and stroke comparisons,although significant inter-level differences in start velocity were only obtained in half (8 out of 16) of the analysed events. Higher level swimmers also travelled for longer start and turn distances but only in the race segments where the gain of speed was high. This was observed in the turn segments, in the backstroke and butterfly strokes and during the 200-m breaststroke event, but not in any of the freestyle events. Time improvements due to the appropriate extension of the underwater subsections appeared to be critical for the end race result and should be carefully evaluated by the “individualized-distance” method

    Association of socioeconomic deprivation with life expectancy and all-cause mortality in Spain, 2011-2013

    Life tables summarise a population's mortality experience during a time period. Sex- and age-specific life tables are needed to compute various cancer survival measures. However, mortality rates vary according to socioeconomic status. We present sex- and age-specific life tables based on socioeconomic status at the census tract level in Spain during 2011-2013 that will allow estimating cancer relative survival estimates and life expectancy measures by socioeconomic status. Population and mortality data were obtained from the Spanish Statistical Office. Socioeconomic level was measured using the Spanish Deprivation Index by census tract. We produced sex- and age-specific life expectancies at birth by quintiles of deprivation, and life tables by census tract and province. Life expectancy at birth was higher among women than among men. Women and men in the most deprived census tracts in Spain lived 3.2 and 3.8 years less than their counterparts in the least deprived areas. A higher life expectancy in the northern regions of Spain was discovered. Life expectancy was higher in provincial capitals than in rural areas. We found a significant life expectancy gap and geographical variation by sex and socioeconomic status in Spain. The gap was more pronounced among men than among women. Understanding the association between life expectancy and socioeconomic status could help in developing appropriate public health programs. Furthermore, the life tables we produced are needed to estimate cancer specific survival measures by socioeconomic status.     Analysis of matched geographical areas to study potential links between environmental exposure to oil refineries and non-Hodgkin lymphoma mortality in Spain

    BACKGROUND: Emissions from refineries include a wide range of substances, such as chrome, lead, nickel, zinc, arsenic, cadmium, benzene, dioxins and furans, all of which are recognized by the International Agency for Research on Cancer (IARC) as carcinogens. Various studies have shown an association between non-Hodgkin lymphoma (NHL) and residence in the vicinity of industrial areas; however, evidence of specific association between refineries and residence in the vicinity has been suggested but not yet established. The aim of this study is to investigate potential links between environmental exposure to emissions from refineries and non-Hodgkin lymphoma mortality in Spain. The spatial distribution of NHL in Spain has an unusual pattern with regions some showing higher risk than others. METHODS: We designed an analysis of matched geographical areas to examine non-Hodgkin lymphoma mortality in the vicinity of the 10 refineries sited in Spain over the period 1997-2006. Population exposure to refineries was estimated on the basis of distance from town of residence to the facility in a 10 km buffer. We defined 10 km radius areas to perform the matching, accounting for population density, level of industrialization and socio-demographic factors of the area using principal components analysis. For the matched towns we evaluated the risk of NHL mortality associated with residence in the vicinity of the refineries and with different regions using mixed Poisson models. Then we study the residuals to assess a possible risk trend with distance. RESULTS: Relative risks (RRs) associated with exposure showed similar values for women and for men, 1.09 (0.97-1.24) and 1.12 (0.99-1.27). RRs for two regions were statistically significant: Canary Islands showed an excess of risk of 1.35 (1.05-1.72) for women and 1.50 (1.18-1.92) for men, whilst Galicia showed an excess of risk of 1.35 (1.04-1.75) for men, but not significant excess for women. CONCLUSIONS: The results suggest a possible increased risk of NHL mortality among populations residing in the vicinity of refineries; however, a potential distance trend has not been shown. Regional effects in the Canary Islands and Galicia are significantly greater than the regional average
