Influence of prior delivery mode on perineal trauma risk.
OBJECTIVE: To evaluate the impact of a previous pregnancy and delivery on perineal trauma rates in the subsequent vaginal birth. METHODS: Retrospective cohort study. The perineal outcomes of secundiparous women with history of previous (first) delivery in one of three categories: failed operative vaginal delivery (FOVD) and second stage emergency caesarean section (EmCS); elective caesarean section (ELCS), and vaginal delivery (VD) with intact perineum, were compared with a control primiparous group. RESULTS: The percentage OASIs at first vaginal delivery after prior FOVD+EmCS was 17.3%(n=9), 12.9%(n=18) after previous ELCS, and 0.6%(n=9) after prior VD maintaining an intact perineum, compared with 6%(n=1193) in the control primiparous group of women. Multivariate regression analysis demonstrated prior FOVD+EmCS and ELCS were associated with a statistically significant increased risk of OASIs of 180% and 110% when compared to control (odds ratio (OR): 2.80; 95% confidence interval (CI): 1.35-5.78 and OR: 2.10; 95%CI: 1.27-3.48) respectively. Prior VD with intact perineum was associated with a statistically significantly reduced risk of OASIs (OR: 0.09; 95%CI: 0.04-0.17). CONCLUSIONS: Previous FOVD+EmCS and ELCS were associated with increased risk of OASIs in subsequent vaginal delivery compared to control, whilst previous VD with intact perineum was associated with decreased risk
Population-Level Benefits from Providing Effective HIV Prevention Means to Pregnant Women in High Prevalence Settings
Background:HIV prevalence among pregnant women in Southern Africa is extremely high. Epidemiological studies suggest that pregnancy increases the risk of HIV sexual acquisition and that HIV infections acquired during pregnancy carry higher risk of mother-to-child transmission (MTCT). We analyze the potential benefits from extending the availability of effective microbicide to pregnant women (in addition to non-pregnant women) in a wide-scale intervention.Methods and Findings:A transmission dynamic model was designed to assess the impact of microbicide use in high HIV prevalence settings and to estimate proportions of new HIV infections, infections acquired during pregnancy, and MTCT prevented over 10 years. Our analysis suggests that consistent use of microbicide with 70% efficacy by 60% of non-pregnant women may prevent approximately 40% and 15% of new infections in women and men respectively over 10 years, assuming no additional increase in HIV risk to either partner during pregnancy (RRHIV/preg = 1). It may also prevent 8-15% MTCT depending on the increase in MTCT risk when HIV is acquired during pregnancy compared to before pregnancy (RRMTCT/preg). Extending the microbicides use during pregnancy may improve the effectiveness of the intervention by 10% (RRHIV/preg = 1) to 25% (RRHIV/preg = 2) and reduce the number of HIV infections acquired during pregnancy by 40% to 70% in different scenarios. It may add between 6% (RRHIV/preg = 1, RRMTCT/preg = 1) and 25% (RRHIV/preg = 2, RRMTCT/preg = 4) to the reduction in the residual MTCT.Conclusion:Providing safe and effective microbicide to pregnant women in the context of wide-scale interventions would be desirable as it would increase the effectiveness of the intervention and significantly reduce the number of HIV infections acquired during pregnancy. The projected benefits from covering pregnant women by the HIV prevention programs is more substantial in communities in which the sexual risk during pregnancy is elevated. © 2013 Dimitrov et al
New ophthalmosaurid ichthyosaurs from the European lower cretaceous demonstrate extensive ichthyosaur survival across the Jurassic–Cretaceous boundary
Background
Ichthyosauria is a diverse clade of marine amniotes that spanned most of the Mesozoic. Until recently, most authors interpreted the fossil record as showing that three major extinction events affected this group during its history: one during the latest Triassic, one at the Jurassic–Cretaceous boundary (JCB), and one (resulting in total extinction) at the Cenomanian-Turonian boundary. The JCB was believed to eradicate most of the peculiar morphotypes found in the Late Jurassic, in favor of apparently less specialized forms in the Cretaceous. However, the record of ichthyosaurs from the Berriasian–Barremian interval is extremely limited, and the effects of the end-Jurassic extinction event on ichthyosaurs remains poorly understood.
Methodology/Principal Findings
Based on new material from the Hauterivian of England and Germany and on abundant material from the Cambridge Greensand Formation, we name a new ophthalmosaurid, Acamptonectes densus gen. et sp. nov. This taxon shares numerous features with Ophthalmosaurus, a genus now restricted to the Callovian–Berriasian interval. Our phylogenetic analysis indicates that Ophthalmosauridae diverged early in its history into two markedly distinct clades, Ophthalmosaurinae and Platypterygiinae, both of which cross the JCB and persist to the late Albian at least. To evaluate the effect of the JCB extinction event on ichthyosaurs, we calculated cladogenesis, extinction, and survival rates for each stage of the Oxfordian–Barremian interval, under different scenarios. The extinction rate during the JCB never surpasses the background extinction rate for the Oxfordian–Barremian interval and the JCB records one of the highest survival rates of the interval.
Conclusions/Significance
There is currently no evidence that ichthyosaurs were affected by the JCB extinction event, in contrast to many other marine groups. Ophthalmosaurid ichthyosaurs remained diverse from their rapid radiation in the Middle Jurassic to their total extinction at the beginning of the Late Cretaceous
High energy emission from microquasars
The microquasar phenomenon is associated with the production of jets by X-ray
binaries and, as such, may be associated with the majority of such systems. In
this chapter we briefly outline the associations, definite, probable, possible,

Bianchi Type-II String Cosmological Models in Normal Gauge for Lyra's Manifold with Constant Deceleration Parameter
The present study deals with a spatially homogeneous and anisotropic
Bianchi-II cosmological models representing massive strings in normal gauge for
Lyra's manifold by applying the variation law for generalized Hubble's
parameter that yields a constant value of deceleration parameter. The variation
law for Hubble's parameter generates two types of solutions for the average
scale factor, one is of power-law type and other is of the exponential form.
Using these two forms, Einstein's modified field equations are solved
separately that correspond to expanding singular and non-singular models of the
universe respectively. The energy-momentum tensor for such string as formulated
by Letelier (1983) is used to construct massive string cosmological models for
which we assume that the expansion () in the model is proportional to
the component of the shear tensor . This
condition leads to , where A, B and C are the metric coefficients
and m is proportionality constant. Our models are in accelerating phase which
is consistent to the recent observations. It has been found that the
displacement vector behaves like cosmological term in the
normal gauge treatment and the solutions are consistent with recent
observations of SNe Ia. It has been found that massive strings dominate in the
decelerating universe whereas strings dominate in the accelerating universe.
Some physical and geometric behaviour of these models are also discussed.Comment: 24 pages, 10 figure
Combination of electroweak and QCD corrections to single W production at the Fermilab Tevatron and the CERN LHC
Precision studies of the production of a high-transverse momentum lepton in
association with missing energy at hadron colliders require that electroweak
and QCD higher-order contributions are simultaneously taken into account in
theoretical predictions and data analysis. Here we present a detailed
phenomenological study of the impact of electroweak and strong contributions,
as well as of their combination, to all the observables relevant for the
various facets of the p\smartpap \to {\rm lepton} + X physics programme at
hadron colliders, including luminosity monitoring and Parton Distribution
Functions constraint, precision physics and search for new physics signals.
We provide a theoretical recipe to carefully combine electroweak and strong
corrections, that are mandatory in view of the challenging experimental
accuracy already reached at the Fermilab Tevatron and aimed at the CERN LHC,
and discuss the uncertainty inherent the combination. We conclude that the
theoretical accuracy of our calculation can be conservatively estimated to be
about 2% for standard event selections at the Tevatron and the LHC, and about
5% in the very high transverse mass/lepton transverse momentum tails. We
also provide arguments for a more aggressive error estimate (about 1% and 3%,
respectively) and conclude that in order to attain a one per cent accuracy: 1)
exact mixed corrections should be computed in
addition to the already available NNLO QCD contributions and two-loop
electroweak Sudakov logarithms; 2) QCD and electroweak corrections should be

Reconciled climate response estimates from climate models and the energy budget of Earth
Climate risks increase with mean global temperature, so knowledge about the amount of future global warming should better inform risk assessments for policymakers. Expected near-term warming is encapsulated by the transient climate response (TCR), formally defined as the warming following 70 years of 1% per year increases in atmospheric CO2 concentration, by which point atmospheric CO2 has doubled. Studies based on Earth’s historical energy budget have typically estimated lower values of TCR than climate models, suggesting that some models could overestimate future warming. However, energy-budget estimates rely on historical temperature records that are geographically incomplete and blend air temperatures over land and sea ice with water temperatures over open oceans. We show that there is no evidence that climate models overestimate TCR when their output is processed in the same way as the HadCRUT4 observation-based temperature record3, 4. Models suggest that air-temperature warming is 24% greater than observed by HadCRUT4 over 1861–2009 because slower-warming regions are preferentially sampled and water warms less than air5. Correcting for these biases and accounting for wider uncertainties in radiative forcing based on recent evidence, we infer an observation-based best estimate for TCR of 1.66 °C, with a 5–95% range of 1.0–3.3 °C, consistent with the climate models considered in the IPCC 5th Assessment Report
The Change Up Project : using social norming theory with young people to address domestic abuse and promote healthy relationships
This paper presents the findings of a secondary analysis of data collected during a pilot project, Change Up, which used a social norming approach (SNA) to address domestic violence and abuse (DVA) with young people aged 13–14. A SNA is based upon a well-articulated theory of behavior and evidence-based methodology for addressing social justice issues. This reflects a paradigm shift focusing upon strengths and positives, rather than pathologizing behaviours. Adopting a SNA, the Change Up project comprised a baseline survey followed by the intervention (workshop and peer-to-peer campaign), ending with a post-intervention survey. It was delivered in two high schools in a UK city between 2015 and 16. A secondary analysis of the survey data collected during the surveys and qualitative data collected at the end of each workshop was undertaken and this is reported here. Change Up data illustrates that most young people in the sample thought that DVA is unacceptable. There was, however, a gender difference in the norms held about the social acceptability of girls using physical violence against boys (and vice versa). The analysis of Change Up data indicates that a social norming approach to DVA programs aimed at young people can be successful in promoting attitude and behaviour change. It also highlights a continuing need for young people’s education about relationships and gender equality
Ordered hierarchy versus scale invariance in sequence stratigraphy
200 x 10(6) years) are symmetrical transgressive- regressive cycles. However, the sequence record in the range of 1 x 10(4)-200 x 10(6) years, the principal domain of sequence stratigraphy, shows a rather irregular succession of sequences with variable symmetry and bounded by flooding surfaces or exposure surfaces. For these time scales, scale-invariant models are a good first approximation, particularly because the evidence for scale-invariance and randomness in the stratigraphic record is strong: Frequency spectra of sea-level change as well as rates of sedimentation and rates of accommodation change plotted against length of observation span show basic trends indistinguishable from random walk. These trends, combined with scale-invariant sequence models may be the most efficient tools for across-the-board predictions on sequences and for locating islands of order in the sequence record
