    Hydatidiform mole resulting from sexual violence

    <p>Abstract</p> <p>Background</p> <p>Hydatidiform mole (HM) is characterized by abnormal proliferation of human trophoblast with producers functioning tissues of human chorionic gonadotropin. It can evolve with ovarian cysts tecaluteínicos, hypertension of pregnancy or hyperthyroidism. The incidence of HM is variable and its etiology poorly known, associated with nutritional factors, environmental, age, parity, history of HM, oral contraceptives, smoking, consanguinity or defects in germ cells. There is no reference in literature on HM resulting from sexual violence, objective of this report.</p> <p>Method</p> <p>Description of two cases of HM among 1146 patients with pregnancy resulting from sexual violence treated at Hospital Pérola Byington, São Paulo, from July 1994 to August 2011.</p> <p>Results</p> <p>The cases affected young, white, unmarried, low educated and low parity women. Sexual violence was perpetrated by known offenders unrelated to the victims, under death threat. Ultrasound and CT of the pelvis showed bulky uterus compatible with HM without myometrial invasion. One case was associated with theca lutein cysts. The two cases were diagnosed in the second trimester of pregnancy and evolved with hyperthyroidism. There was no hypertension, disease recurrence, metastasis or sexually transmitted infection.</p> <p>Conclusion</p> <p>The incidence of HM was 1:573 pregnancies resulting from rape, within the range estimated for Latin American countries. Trophoblastic material can be preserved to identify the violence perpetrator, considering only the paternal HM chromosomes. History of sexual violence should be investigated in cases of HM in the first half of adolescence and women in a vulnerable condition.</p

    A escolha correta do antimicrobiano requer diagnóstico clínico laboratorial (identificação do agente infeccioso e antibiograma) e conhecimento farmacológico das drogas utilizadas na terapêutica. Os antibióticos são dispensados livremente nas farmácias sem necessidade de prescrição médica, favorecendo a aparecimento de problemas como terapia ineficaz, resistência bacteriana e indução da auto medicação, uma vez que balconistas recebem comissão na venda de medicamentos. O atendimento requer a atenção farmacêutica onde se possa realizar a anamnese do paciente, estabelecendo-se um sistema seguro, efetivo e que beneficia a farmacoterapia com o objetivo de obter resultados terapêuticos definidos na saúde e qualidade de vida do paciente. PHARMACEUTICAL ASSISTANCE IN ANTIBIOTICS THERAPY Abstract The correct choice of an antimicrobial therapy requires clinical and laboratorial diagnosis (identification of infectious agent and antibiogram) and pharmacological knowledge of drugs used in therapeutics. Antibiotics are freely dispensed in drugstores with no need of medical prescription, leading to the appearance of problems such as inefficient therapy, bacterial resistance and induction of selfmedication, once sellers receive commission on sales of medicine. The correct selling requires the pharmaceutical care, when its possible to collect information from the patient, establishing a safe and effective system that benefits the pharmacotherapy in order to reach defined therapeutic results on health and quality of life of patient

    Chagas Cardiomyopathy Manifestations and Trypanosoma cruzi Genotypes Circulating in Chronic Chagasic Patients

    Chagas disease caused by Trypanosoma cruzi is a complex disease that is endemic and an important problem in public health in Latin America. The T. cruzi parasite is classified into six discrete taxonomic units (DTUs) based on the recently proposed nomenclature (TcI, TcII, TcIII, TcIV, TcV and TcVI). The discovery of genetic variability within TcI showed the presence of five genotypes (Ia, Ib, Ic, Id and Ie) related to the transmission cycle of Chagas disease. In Colombia, TcI is more prevalent but TcII has also been reported, as has mixed infection by both TcI and TcII in the same Chagasic patient. The objectives of this study were to determine the T. cruzi DTUs that are circulating in Colombian chronic Chagasic patients and to obtain more information about the molecular epidemiology of Chagas disease in Colombia. We also assessed the presence of electrocardiographic, radiologic and echocardiographic abnormalities with the purpose of correlating T. cruzi genetic variability and cardiac disease. Molecular characterization was performed in Colombian adult chronic Chagasic patients based on the intergenic region of the mini-exon gene, the 24Sα and 18S regions of rDNA and the variable region of satellite DNA, whereby the presence of T.cruzi I, II, III and IV was detected. In our population, mixed infections also occurred, with TcI-TcII, TcI-TcIII and TcI-TcIV, as well as the existence of the TcI genotypes showing the presence of genotypes Ia and Id. Patients infected with TcI demonstrated a higher prevalence of cardiac alterations than those infected with TcII. These results corroborate the predominance of TcI in Colombia and show the first report of TcIII and TcIV in Colombian Chagasic patients. Findings also indicate that Chagas cardiomyopathy manifestations are more correlated with TcI than with TcII in Colombia
