    Anesthetic management in awake craniotomy

    Resection of brain tumors may cause neurological sequelae, according to the site and size of the brain tissue removed. Awake craniotomy has been proposed as a surgical approach to satisfy criteria of radical surgery while minimizing eloquent brain damage.The most critical aspect of awake craniotomy is to maintain adequate patient comfort, analgesia, immobility and cooperation during a long surgical operation, ensuring in the meanwhile the safety, control and maintenance of vital functions. Apart from pharmacological, surgical, technical knowledge and skillfulness, the ability to maintain close psycho-emotionalcontact and support with the patient throughout the operation is a fundamental task that the anesthesiologist has to pursue for the operation to be successfully managed. This review summarizes the current opinion on anesthetic management of patients undergoing awake craniotomy

    Conversions related to adhesions in abdominal surgery. Robotic versus laparoscopic approach: a multicentre experience

    Background: An advantage of robotic surgery over laparoscopy is the lower rate of unplanned conversion. One of the implicated reasons for conversion is adhesions from previous abdominal surgeries (PASs). Methods: A comparative analysis of 98 patients with history of open PAS treated by laparoscopic or robotic surgery was performed. Primary endpoint was the rate of conversion to open surgery related to adhesiolysis. Secondary endpoints were short-term outcomes and complications. Results: Conversion rate specifically related to adhesiolysis was significantly lower in robotic group (13 for laparoscopic group vs. 2 for robotic group; p = 0.046). Conversions occurred during adhesiolysis were significantly related to severity of adhesions expressed by peritoneal adhesion index (PAI) score (p < 0.001), number of abdominal areas involved by adhesions (p < 0.001) and severity of PAI into the target area of surgical intervention (p = 0.021). Conclusions: Benefits of robotic surgery are more noticeable in performing procedures with increasing technical difficulties

    MEDIRAD project “implications of medical low-dose radiation exposure”: Enhancing the protection of patients and health professionals from exposure to low-dose medical radiation

    The EC-funded project MEDIRAD addresses numerous low-dose exposure situations for patients and workers in the medical context, to further develop risk models and draw operational recommendations for improving radiation protection. The four-year project (2017–2021) relies on a 33-partner consortium from 14 European countries and is coordinated by the European Institute for Biomedical Imaging Research (EIBIR, AT). Prof. Elisabeth Cardis (ISGlobal, ES) and Prof. Guy Frija (Universit´e Paris Descartes, FR) are the scientific coordinator and the clinical coordinator, respectively. The MEDIRAD Project consists of six interdependent and complimentary Work Packages (WPs). WP1: project management and dissemination; WP2: dose evaluation and optimisation in medical imaging; WP3: impact of low-dose radiation exposure from I-131 radioiodine in thyroid cancer treatment; WP4: breast radiotherapy and secondary cardiovascular risks; WP5: possible health impact of paediatric scanning; WP6: bringing together medical and nuclear scientific communities for radiation protection purposes. The Italian National Institute of Health (ISS), thanks to its consolidated experience in the field of quality assurance in radiological sciences, will contribute to the latter issue coordinating a Working Group to develop recommendations on patient radiological protection, directed to the medical communities, considering the scientific outcomes of the MEDIRAD WPs and the stakeholders’comments

    Microwave-Assisted Extraction of Bioactive Compounds from Lentil Wastes: Antioxidant Activity Evaluation and Metabolomic Characterization

    The recovery of industrial by-products is part of the zero-waste circular economy. Lentil seed coats are generally considered to be a waste by-product. However, this low-value by-product is rich in bioactive compounds and may be considered an eco-friendly source of health-promoting phytochemicals. For the first time, a sustainable microwave-assisted extraction technique was applied, and a solvent screening was carried out to enhance the bioactive compound content and the antioxidant activity of green and red lentil hull extracts. With respect to green lentil hull extracts that were obtained with different solvents, the aqueous extract of the red lentil seed coats showed the highest total phenolic and total flavonoid content (TPC = 28.3 ± 0.1 mg GAE/g dry weight, TFC = 1.89 ± 0.01 mg CE/100 mg dry weight, respectively), as well as the highest antioxidant activity, both in terms of the free radical scavenging activity (ABTS, 39.06 ± 0.73 mg TE/g dry weight; DPPH, IC50 = 0.39 μg/mL) and the protection of the neuroblastoma cell line (SH-SY5Y, IC50 = 10.1 ± 0.6 μg/mL), the latter of which has never been investigated so far. Furthermore, a metabolite discovery analysis was for the first time performed on the aqueous extracts of both cultivars using an HPLC separation which was coupled with an Orbitrap-based high-Resolution Mass Spectrometry technique

    Measurements and tests on FBK silicon sensors with an optimized electronic design for a CTA camera

    In October 2013, the Italian Ministry approved the funding of a Research & Development (R&D) study, within the "Progetto Premiale TElescopi CHErenkov made in Italy (TECHE)", devoted to the development of a demonstrator for a camera for the Cherenkov Telescope Array (CTA) consortium. The demonstrator consists of a sensor plane based on the Silicon Photomultiplier (SiPM) technology and on an electronics designed for signal sampling. Preliminary tests on a matrix of sensors produced by the Fondazione Bruno Kessler (FBK-Trento, Italy) and on electronic prototypes produced by SITAEL S.p.A. will be presented. In particular, we used different designs of the electronics in order to optimize the output signals in terms of tail cancellation. The disease is caused by ingestion of larval nematodes in lightly cooked or raw seafood. Because symptoms are vague and serodiagnosis is difficult, the disease is often misdiagnosed and cases are understimated

    INFN Camera demonstrator for the Cherenkov Telescope Array

    The Cherenkov Telescope Array is a world-wide project for a new generation of ground-based Cherenkov telescopes of the Imaging class with the aim of exploring the highest energy region of the electromagnetic spectrum. With two planned arrays, one for each hemisphere, it will guarantee a good sky coverage in the energy range from a few tens of GeV to hundreds of TeV, with improved angular resolution and a sensitivity in the TeV energy region better by one order of magnitude than the currently operating arrays. In order to cover this wide energy range, three different telescope types are envisaged, with different mirror sizes and focal plane features. In particular, for the highest energies a possible design is a dual-mirror Schwarzschild-Couder optical scheme, with a compact focal plane. A silicon photomultiplier (SiPM) based camera is being proposed as a solution to match the dimensions of the pixel (angular size of ~ 0.17 degrees). INFN is developing a camera demonstrator made by 9 Photo Sensor Modules (PSMs, 64 pixels each, with total coverage 1/4 of the focal plane) equipped with FBK (Fondazione Bruno Kessler, Italy) Near UltraViolet High Fill factor SiPMs and Front-End Electronics (FEE) based on a Target 7 ASIC, a 16 channels fast sampler (up to 2GS/s) with deep buffer, self-trigger and on-demand digitization capabilities specifically developed for this purpose. The pixel dimensions of 6×66\times6 mm2^2 lead to a very compact design with challenging problems of thermal dissipation. A modular structure, made by copper frames hosting one PSM and the corresponding FEE, has been conceived, with a water cooling system to keep the required working temperature.     RENEB accident simulation exercise

    Purpose: The RENEB accident exercise was carried out in order to train the RENEB participants in coordinating and managing potentially large data sets that would be generated in case of a major radiological event. Materials and methods: Each participant was offered the possibility to activate the network by sending an alerting email about a simulated radiation emergency. The same participant had to collect, compile and report capacity, triage categorization and exposure scenario results obtained from all other participants. The exercise was performed over 27 weeks and involved the network consisting of 28 institutes: 21 RENEB members, four candidates and three non-RENEB partners. Results: The duration of a single exercise never exceeded 10 days, while the response from the assisting laboratories never came later than within half a day. During each week of the exercise, around 4500 samples were reported by all service laboratories (SL) to be examined and 54 scenarios were coherently estimated by all laboratories (the standard deviation from the mean of all SL answers for a given scenario category and a set of data was not larger than 3 patient codes). Conclusions: Each participant received training in both the role of a reference laboratory (activating the network) and of a service laboratory (responding to an activation request).
