Endoluminal calprotectin measurement in assessment of pouchitis and a new index of disease activity. A pilot study
Pouchitis is the most common complication following proctocolectomy with ileal pouch-anal anastomosis for ulcerative colitis (UC). To provide a standardized definition of pouchitis clinical, endoscopic and histological markers were grouped and weighted in the pouch disease activity index (PDAI). However, the delay in the assessment of the final score due to the time requested for histological analysis remains the main obstacle to the index implementation in clinical practice so that the use of modified-PDAI (mPDAI) with exclusion of histologic subscore has been proposed. We tested the ability of calprotectin measurement in the pouch endoluminal content to mimic the histologic score as defined in the PDAI, the index that we adopted as gold standard for pouchitis diagnosis. Calprotectin was measured by ELISA in the pouch endoluminal content collected during endoscopy in 40 consecutive patients with J-pouch. In each patient PDAI and mPDAI were calculated and 15% of patients were erroneously classified by mPDAI. ROC analysis of calprotectin values vs. acute histological subscore 3 identified different calprotectin cut-off values with corresponding sensitivity and specificity allowing the definition and scoring of different range of calprotectin subscores. We incorporated the calprotectin score in the mPDAI obtaining a new score that shows the same specificity as PDAI for diagnosis of pouchitis and higher sensitivity when compared with mPDAI. The use of the proposed new score, once validated in a larger series of patients, might be useful in the early management of patients with symptoms of pouchitis
Self-rated health among Greenlandic Inuit and Norwegian Sami adolescents: associated risk and protective correlates
Objectives. Self-rated health (SRH) and associated risk and protective correlates were investigated among two
indigenous adolescent populations, Greenlandic Inuit and Norwegian Sami.
Design. Cross-sectional data were collected from ‘‘Well-being among Youth in Greenland’’ (WBYG) and
‘‘The Norwegian Arctic Adolescent Health Study’’ (NAAHS), conducted during 2003 2005 and comprising
10th and 11th graders, 378 Inuit and 350 Sami.
Methods. SRH was assessed by one single item, using a 4-point and 5-point scale for NAAHS and WBYG,
respectively. Logistic regressions were performed separately for each indigenous group using a dichotomous
measure with ‘‘very good’’ (NAAHS) and ‘‘very good/good’’ (WBYG) as reference categories. We simultaneously
controlled for various socio-demographics, risk correlates (drinking, smoking, violence and suicidal
behaviour) and protective correlates (physical activity, well-being in school, number of close friends and
adolescent parent relationship).
Results. A majority of both Inuit (62%) and Sami (89%) youth reported ‘‘good’’ or ‘‘very good’’ SRH. The
proportion of ‘‘poor/fair/not so good’’ SRH was three times higher among Inuit than Sami (38% vs. 11%,
p50.001). Significantly more Inuit females than males reported ‘‘poor/fair’’SRH(44% vs. 29%, p50.05),while
no gender differences occurred among Sami (12% vs. 9%, p50.08). In both indigenous groups,
suicidal thoughts (risk) and physical activity (protective) were associated with poor and good SRH, respectively.
Conclusions. In accordance with other studies of indigenous adolescents, suicidal thoughts were strongly
associated with poorer SRH among Sami and Inuit. The Inuit Sami differences in SRH could partly be due
to higher ‘‘risk’’ and lower ‘‘protective’’ correlates among Inuit than Sami. The positive impact of physical
activity on SRH needs to be targeted in future intervention programs
Cardiac involvement in hereditary myopathy with early respiratory failure: A cohort study.
OBJECTIVE: To assess whether hereditary myopathy with early respiratory failure (HMERF) due to the c.951434T>C; (p.Cys31712Arg) TTN missense mutation also includes a cardiac phenotype. METHOD: Clinical cohort study of our HMERF cohort using ECG, 2D echocardiogram, and cross-sectional cardiac imaging with MRI or CT. RESULTS: We studied 22 participants with the c.951434T>C; (p.Cys31712Arg) TTN missense mutation. Three were deceased. Cardiac conduction abnormalities were identified in 7/22 (32%): sustained atrioventricular tachycardia (n = 2), atrial fibrillation (n = 2), nonsustained atrial tachycardia (n = 1), premature supraventricular complexes (n = 1), and unexplained sinus bradycardia (n = 1). In addition, 4/22 (18%) had imaging evidence of otherwise unexplained cardiomyopathy. These findings are supported by histopathologic correlation suggestive of myocardial cytoskeletal remodeling. CONCLUSIONS: Coexisting cardiac and skeletal muscle involvement is not uncommon in patients with HMERF arising due to the c.951434T>C; (p.Cys31712Arg) TTN mutation. Influence of rainfall on E. coli concentrations in clams: results of collaboration between competent health authority and producers’ association in the Province of Fermo (Italy)
The Area Vasta di Fermo (the competent health authority of the Marche Region), in
collaboration with the local Producers’ Association conducted a series of studies on the
presence of E. coli as an indicator of faecal contamination in the environment, in shellfish and
in fishery waters, and on the factors that can affect their presence. These studies, carried out
from 2008 to 2011, included an assessment of the currents along the coast, of the
precipitations, and data from the monitoring of E. coli on shellfish harvested in the collection
areas were examined. The results showed that in most cases, small concentrations of
microorganisms in shellfish corresponded to little or no precipitations, while an increase in
the levels of E. coli was preceded by more or less abundant rainfalls.
The conclusions suggest that it is advisable to carry out a more detailed risk analysis
which should take into account the above-mentioned factors. Furthermore, monitoring alone
based on the determination of indicator organisms, especially when carried out as a single
analysis or with a small number of E. coli determinations, does not provide a satisfactory
indication of safety. The regional surveillance plans should be applied timely and rigorously,
together with on-site investigations aimed at identifying changes that can affect the presence
of E. coli in shellfish. Food business operators themselves could implement good
manufacturing practices to verify whether the microbiological parameters are within the
prescribed limits after rainfalls, especially if heavy
