    Newer Oral Anticoagulants: Stroke Prevention and Pitfalls

    Warfarin is very effective in preventing stroke in patients with atrial fibrillation. However, its use is limited due to fear of hemorrhagic complications, unpredictable anticoagulant effects related to multiple drug interactions and dietary restrictions, a narrow therapeutic window, frequent difficulty maintaining the anticoagulant effect within a narrow therapeutic window, and the need for inconvenient monitoring. Several newer oral anticoagulants have been approved for primary and secondary prevention of stroke in patients with non-valvular atrial fibrillation. These agents have several advantages relative to warfarin therapy. As a group, these direct oral anticoagulants (DOAC), which include the direct thrombin inhibitor, dabigatran, and the factor Xa inhibitors (rivaroxaban, apixaban, and edoxaban), are more effective than dose adjusted warfarin for prevention of all-cause stroke (including both ischemic and hemorrhagic stroke), and have an overall more favorable safety profile. Nevertheless, an increased risk of gastrointestinal bleeding (with the exception of apixaban), increased risk for thrombotic complication with sudden discontinuation, and inability to accurately assess and reverse anticoagulant effect require consideration prior to therapy initiation, and pose a challenge for decision making in acute stroke therapy

    Predictors of Outcome following Stroke due to Isolated M2 Occlusions

    BACKGROUND: Factors influencing outcome after cerebral artery occlusion are not completely understood. Although it is well accepted that the site of arterial occlusion critically influences outcome, the majority of studies investigating this issue has focused on proximal large artery occlusion. To gain a better understanding of factors influencing outcome after distal large artery occlusion, we sought to assess predictors of outcome following isolated M2 middle cerebral artery occlusion infarcts. METHODS: We retrospectively analyzed patients with isolated acute M2 occlusion admitted to a single academic center from January 2010 to August 2012. Baseline clinical, laboratory imaging, and outcome data were assessed from a prospectively collected database. Factors associated with a modified Rankin Scale (mRS) score \u3c /=2 in univariable analyses (p \u3c 0.05) were entered into multivariable logistic regression analysis. The Admission National Institutes of Health Stroke Scale (aNIHSS) score, age, and infarct volume were also entered as dichotomized variables. Receiver operating characteristic curves were plotted to determine the optimal aNIHSS score, infarct volume, and age cut points predicting an mRS score \u3c /=2. Optimal thresholds were determined by maximizing the Youden index. Respective multivariable logistic regression analyses were used to identify independent predictors of a good 90-day outcome (mRS score \u3c /=2; primary analysis) as well as 90-day mortality (secondary outcome). RESULTS: 90 patients with isolated M2 occlusion were included in the final analyses. Of these, 69% had a good 90-day outcome which was associated with age \u3c 80 years (p = 0.007), aNIHSS \u3c 10 (p = 0.002), and infarct volume \u3c /=26 ml (p \u3c 0.001). Notably, 20% of patients (64% of those with a poor outcome) had died by 90 days. Secondary analysis for 90-day mortality was performed. This analysis indicated that infarct volume \u3e 28 ml (OR 11.874, 95% CI 2.630-53.604, p = 0.001), age \u3e 80 years (OR 4.953, 95% CI 1.087-22.563, p = 0.039), need for intubation (OR 7.788, 95% CI 1.072-56.604), and history of congestive heart failure (OR 5.819, 95% CI 1.140-29.695) were independent predictors of 90-day mortality (20% of all included patients). CONCLUSION: While the majority of patients with isolated M2 occlusion stroke has a good 90-day outcome, a substantial proportion of subjects dies by 90 days, as identified by a unique subset of predictors. The knowledge gained from our study may lead to an improvement in the prognostic accuracy, clinical management, and resource utilization in this patient population

    Changes in attitudes towards smoking during smoking cessation courses for Turkish- and Albanian-speaking migrants in Switzerland and its association with smoking behavior: A latent change score approach

    Introduction Migrant populations usually report higher smoking rates than locals. At the same time, people with a migration background have little or no access to regular smoking cessation treatment. In the last two decades, regular smoking cessation courses were adapted to reach out to Turkish- and Albanian-speaking migrants living in Switzerland. The main aims of the current study were (1) to analyze the effects of an adapted smoking cessation course for Turkish- and Albanian-speaking migrants in Switzerland on attitudes toward smoking and smoking behavior; and (2) to elucidate whether changes in attitudes toward smoking were associated to changes in smoking behavior in the short- and in the long-term. Methods A total of 59 smoking cessation courses (Turkish: 37; Albanian: 22) with 436 participants (T: 268; A: 168) held between 2014 and 2019 were evaluated. Attitudes toward smoking and cigarettes smoked per day were assessed at baseline and 3-months follow-up. One-year follow-up calls included assessment of cigarettes smoked per day. Data were analyzed by means of structural equation modeling with latent change scores. Results Participation in an adapted smoking cessation course led to a decrease of positive attitudes toward smoking (T: β = −0.65, p < 0.001; A: β = −0.68, p < 0.001) and a decrease of cigarettes smoked per day in the short-term (T: β = −0.58, p < 0.001; A: β = −0.43, p < 0.001) with only Turkish-speaking migrants further reducing their smoking in the long-term (T: β = −0.59, p < 0.001; A: β = −0.14, p = 0.57). Greater decreases in positive attitudes were associated with greater reductions of smoking in the short-term (T: r = 0.39, p < 0.001; A: r = 0.32, p = 0.03), but not in the long-term (T: r = −0.01, p = 0.88; A: r = −0.001, p = 0.99). Conclusion The adapted smoking cessation courses fostered changes in positive attitudes toward smoking that were associated with intended behavior change in the short-term. The importance of socio-cognitive characteristics related to behavior change maintenance to further increase treatment effectiveness in the long-term is discussed

    Parametric amplification of vacuum fluctuations in a spinor condensate

    Parametric amplification of vacuum fluctuations is crucial in modern quantum optics, enabling the creation of squeezing and entanglement. We demonstrate the parametric amplification of vacuum fluctuations for matter waves using a spinor F=2 Rb-87 condensate. Interatomic interactions lead to correlated pair creation in the m_F= +/- 1 states from an initial unstable m_F=0 condensate, which acts as a vacuum for m_F unequal 0. Although this pair creation from a pure m_F=0 condensate is ideally triggered by vacuum fluctuations, unavoidable spurious initial m_F= +/- 1 atoms induce a classical seed which may become the dominant triggering mechanism. We show that pair creation is insensitive to a classical seed for sufficiently large magnetic fields, demonstrating the dominant role of vacuum fluctuations. The presented system thus provides a direct path towards the generation of non-classical states of matter on the basis of spinor condensates.Comment: 5 pages, 4 figure

    Alberta Stroke Program Early CT Score Infarct Location Predicts Outcome Following M2 Occlusion

    BACKGROUND: Although it is generally thought that patients with distal middle cerebral artery (M2) occlusion have a favorable outcome, it has previously been demonstrated that a substantial minority will have a poor outcome by 90 days. We sought to determine whether assessing the Alberta Stroke Program Early CT Score (ASPECTS) infarct location allows for identifying patients at risk for a poor 90-day outcome. METHODS: We retrospectively analyzed patients with isolated acute M2 occlusion admitted to a single academic center between January 2010 and August 2012. Infarct regions were defined according to ASPECTS system on the initial head computed tomography. Discriminant function analysis was used to define specific ASPECTS regions that are predictive of the 90-day functional outcome as defined as a modified Rankin Scale score of 3-6. In addition, logistic regression was used to model the relationship between each individual ASPECT region with poor outcome; for evaluation and comparison, odds ratios, c-statistics, and Akaike information criterion values were estimated for each region. RESULTS: Ninety patients with isolated M2 were included in the final analysis. ASPECTS score CONCLUSION: Infarction in ASPECTS regions M3 and M6 are key predictors of functional outcome following isolated distal M2 occlusion. These findings will be helpful in stratifying outcomes if validated in future studies

    An Observational Diagnostic for Ultraluminous X-Ray Sources

    We consider observational tests for the nature of Ultraluminous X-ray sources (ULXs). These must distinguish between thermal-timescale mass transfer on to stellar-mass black holes leading to anisotropic X-ray emission, and accretion on to intermediate-mass black holes. We suggest that long-term transient behavior via the thermal-viscous disk instability could discriminate between these two possibilities for ULXs in regions of young stellar populations. Thermal-timescale mass transfer generally produces stable disks and persistent X-ray emission. In contrast, mass transfer from massive stars to black holes produces unstable disks and thus transient behavior, provided that the black hole mass exceeds some minimum value. This minimum mass depends primarily on the donor mass and evolutionary state. We show that it exceeds 50 solar masses for a large fraction (greater than 90%) of the mass-transfer lifetime for the most likely donors in young clusters. Thus if long-term monitoring reveals a large transient fraction among ULXs in a young stellar population, these systems would be good candidates for intermediate-mass black holes in a statistical sense; information about the donor star is needed to make this identification secure in any individual case. Knowledge of where the poor are, and what characterises them, is patchy at best. Here we describe a global livestock and poverty mapping study designed to assist in targeting research and development activities concerning livestock. Estimates of the numbers of poor livestock keepers by production system and region are presented. While these estimates suffer from various problems, improvements in global databases are critical to improve the targeting of interventions that can meet the challenges posed by poverty and to chart progress against international development indicators

    Developmental differences in affective representation between prefrontal and subcortical structures

    Developmental studies have identified differences in prefrontal and subcortical affective structures between children and adults, which correspond with observed cognitive and behavioral maturations from relatively simplistic emotional experiences and expressions to more nuanced, complex ones. However, developmental changes in the neural representation of emotions have not yet been well explored. It stands to reason that adults and children may demonstrate observable differences in the representation of affect within key neurological structures implicated in affective cognition. Forty-five participants (25 children; 20 adults) passively viewed positive, negative, and neutral clips from popular films while undergoing functional magnetic resonance imaging (fMRI). Using representational similarity analysis (RSA) to measure variability in neural pattern similarity, we found developmental differences between children and adults in the amygdala, nucleus accumbens (NAcc), and ventromedial prefrontal cortex (vmPFC), such that children generated less pattern similarity within subcortical structures relative to the vmPFC; a phenomenon not replicated among their older counterparts. Furthermore, children generated valence-specific differences in representational patterns across regions; these valence-specific patterns were not found in adults. These results may suggest that affective representations grow increasingly dissimilar over development as individuals mature from visceral affective responses to more evaluative analyses

    Multi-resonant spinor dynamics in a Bose-Einstein condensate

    We analyze the spinor dynamics of a Rb-87 F=2 condensate initially prepared in the m_F=0 Zeeman sublevel. We show that this dynamics, characterized by the creation of correlated atomic pairs in m_F=+/-1, presents an intriguing multi-resonant magnetic field dependence induced by the trap inhomogeneity. This dependence is directly linked to the most unstable Bogoliubov spin excitations of the initial m_F = 0 condensate, showing that, in general, even a qualitative understanding of the pair creation efficiency in a spinor condensate requires a careful consideration of the confinement.Comment: 5 pages, 4 figure
