Prótesis total de cadera con poropatita: Resultados a 12 años
El trabajo analiza 150 prótesis totales de cadera, con un seguimiento de 12 años. En todos los pacientes se implantaron los componentes de Poropatita fabricados con aleación de Titanio y recubiertos de Poropatita. La edad de los pacientes oscilo entre los 25 y 85 años, con una media de 61 años, y en las revisiones posteriores realizadas, tanto clínica como radiográficamente, se observó una rápida integración de los implantes con áreas de osteoconducción y osteointegración evidentes.We reviewed 150 hip arthroplasties with a follow-up of 12 years. In all patients we used the components made of a titanium alloy with poropatita. The average age of the patients was 85 years (range 25-85 years). The follow-up period showed good clinical outcome, and radiographic parameters disclosed excellent integration of the implants with areas of osteoconduction and osteointegration
Tratamiento de la pseudoartrosis con P15
La reconstrucción de los defectos óseos, es un problema que se plantea con frecuencia en Cirugía Traumatológica. El objetivo de este trabajo esa evaluar la regeneración ósea en paciente con retardo de consolidación de fracturas de húmero y tibia, utilizando un factor de crecimiento (P15 combinado con Hidroxiapatita) y observar la respuesta osteoinductiva de la regeneración ósea. Se intervinieron quirúrgicamente 12 pacientes, con un seguimiento mínimo de 2 años. Describimos la metodología en cuanto a técnica y control post-operatorio y valoramos el estado clínico, radiológico e histológico. Los resultados fueron muy buenos en 10 casos. El P15 combinado con la Hidroxiapatita, ha permitido ver una respuesta osteoconductiva y osteoinductiva en los defectos óseos, con un futuro presumiblemente prometedor en Cirugía Ortopédica.The reconstruction of bone defects is a problem in the current orthopaedic surgery practice. The object of this study is to evaluate the regeneration in bones defects under the influence of a growth factor (P15 combined with hidroxyapatite), recording the evidence of osteo-inductive activity. Twelve patients with bone defects were operated with a minimal follow up of two years, we detailed the technique and postoperative management of the patients. Clinical, radiology and histology results were assessed, with excellent outcome in 10 cases. El par cerámica-cerámica en la prótesis total de cadera: resultados clínicos a 12 años
Se analizan retrospectivamente 150 prótesis totales de cadera con par de fricción de cerámica alumínica, con seguimiento clínico y radiográfico a 12 años. se utilizó una prótesis de titanio con recubrimiento de porro mas hidroxiapatita, con superficies articulares de óxido de aluminia en cotilo y cabeza femoral. La edad de los pacientes fue entre 30 y 65 años, con una media de 56. En las revisiones realizadas anualmente se puso de manifiesto una rápida osteintegración de la prótesis y un buen resultado clínico y radiuográfico.A total of 150 hip arthroplasties with combination

Identificación precoz del riesgo post-operatorio en las prótesis totales de rodilla
El objetivo de este estudio es conocer la relación que mantienen diversos factores clínicos, quirúrgicos y hemostáticos analizados antes de la intervención quirúrgica, con la presencia de trombosis venosa (TV) post-operatoria, diagnosticada mediante unos parámetros preestablecidos. Se evaluaron 38 pacientes sometidos a prótesis total de rodilla (PTR). La edad media, fue de 66 años, y se administró profilaxix antitrombótica a todos los pacientes con Heparina (7000 u x 12 h.) y 200 mg de Aspirina. El protocolo establecido, incluyó cuatro periodos en los que se valoraron parámetros clínicos, bioquímicos y gradiente alveolo-arterial (AaD02). El AaD02 con valores de 0,6 y 0,7, resultó ser un excelente índice predictivo de riesgo.The objective of this study is to know the relation that different clinical, surgical and determined haemostatic factors maintain befote a surgical operation with the presence and charasteristics of the postoperative vein trombosis (VT). Gangrena gaseosa: revisión de cuatro casos
Desde el mes de enero de 1988 hasta diciembre de 1993, hemos atendido en
el Servicio de Cirugía Ortopédica y Traumatología 6.853 pacientes, de ellos 263 presentaron
infecciones post-quirúrgicas, post-traumáticas o expontáneas. Del total de infecciones cuatro se
manifestaron como un cuadro clínico de gangrena gaseosa. El origen fue post-traumático en
tres casos y por arma de fuego en uno, localizándose la enfermedad en todos los pacientes en
las extremidades inferiores. Para establecer el diagnóstico además de las manifestaciones clínicas
se ha valorado el resultado bacterológico y anatomo-patológico de las muestras analizadas.
El tratamiento consistió en Cirugía precoz, antibioticoterapia y oxigenoterapia hiperbárica
cuando era necesario. La evolución ha sido buena en dos casos y desfavorable en el resto de
los enfermos.From January 1988 to December 1993, we have seen in our institution 6853
patients, 263 of them presented post-surgical, post-traumatic or spontaneus infections. Of the
total of infections, four of them were manifested as clinical cases of gaseous gangrene. The origin
was post-traumatic in three cases, and gun shot in one case, localizing the disease in all patients
in inferior extremities. For diagnosis clinical manifestations and the bacteriological and
patologic results were evaluated. The treatment consisted of early surgery, antibiotical therapy
and hiperbaric oxigenotherapy when it was necessary. The outcome was satisfactory in two cases
and non favorable in the other two patients
Comportamiento del polietileno en un simulador de cadera
Se presentan los resultados del comportamiento friccional de cinco diferentes
combinaciones de cabezas protésicas femorales y polietileno acetabular, investigadas
en un simulador de cadera. Las cabezas femorales de aleaciones de Cr-Co-Mo, Cr-Co-Mo
con implantación iónica y de cerámica de zirconia, se articularon con cotilos de polietileno
de ultra-alto-peso molecular y se sometieron a tres millones de ciclos de marcha cada
uno, utilizando un simulador de cadera de cinco estaciones. El desgaste de los cotilos protésicos,
evaluado por mediciones gravimétricas demostró que cuando el espesor de polietileno
era de 10,9 mm el desgaste producido por cabezas de Cr-Co-Mo de 28 mm era de
0,14 mm/año y de 0,09 mm/año si la cabeza utilizada era de 28 mm de aleación de Cr-Co-
Mo con implantación iónica. Cuando el espesor del acetábulo era de 7 mm el desgaste ocasionado
por cabezas de cerámica de zirconia de 28 mm fue de 0,04 mm/año; sin embargo
si tenía espesor 10,9 mm y las cabezas de cerámica de zirconia eran de 28 y 32 mm, no aparecía
desgaste alguno. Estos resultados indican que el diámetro de la cabeza de cerámica
de 28 y 32 mm, parece no tener importancia alguna, pero sí la tiene el que el espesor del
acetábulo protésico sea menor de 10 mm.Ultra high molecular weight polyethylene (UHMWPE) metal backed acetabular
cups were articulated against Cr-Co-Mo alloy, ion implanted Cr-Co-Mo and zirconia
ceramic femoral heads in a five station hip joint simulator, running three tests of three millions
walking cycles with each one of the five different head-cup combinations. The wear
rate of the acetabular cups was measured gravimetrically. When the thickness of the cup
was 10.9 mm, the mean wear rate against 28 mm diameter Cr-Co-Mo femoral heads was
0.14 mm/year, and 0.09 mm/year 'when the cup was articulated against 28 mm ion implanted
Cr-Co-Mo femoral heads. When the thickness of the cup was 7 mm the mean wear rate
was 0.04 mm/year against 28 mm zirconia heads. The results indicate that significant reduction
in the wear of the UHMWPE cup can be achieved when zirconia femoral head is
used and that the diameter of the zirconia femoral head, 28 vs 32 mm, seems unimportant
as wear factor. A cup thickness below 10 mm may result disadvantageous
Estallido de la cabeza de cerámica en 1 prótesis total de cadera cementada
Se describen las características clínicas y quirúrgicas de 1 caso de estallido de la
cabeza de cerámica en 1 artroplastia total de cadera cementada tipo Weller. A los 6 años y 2 meses
de su implante requirió nueva intervención quirúrgica para sustituir el componente dañado.
Describimos las posibles causas de esta complicación en base a la literatura revisada. El interés del
caso estriba en la rareza de su presentación y en la conservación de la prótesis sustituyendo solamente
el núcleo cefálico dañado.The clinical and surgical characteristics of one case of breakage of the ceramic
head in a Weller cemented hip arthroplasty is described. The patient needed revision surgery six
years and two months after the primary arthroplasty in order to replace the demaged component
of the implant. The possible reasons of this complication, according to a literature review, are discussed.
The interest of this clinical case is the rare of its presentation, its cause and the surgical
solution that allow preservation of the prosthesis, doing exclusively the replacement of the damaged
part of the implant
Prótesis modular de cadera Poropalcar : resultados clínicos en 106 pacientes con 3 años de evolución
Los resultados tras reemplazamiento total de cadera con prótesis de superficie
porometal «poropros», para fijación biológica, han sido evaluados en 106 pacientes después
de tres años de evolución. La valoración clínica se ha realizado según el sistema de Harris modificado,
obteniendo un 91,5% de buenos resultados y un 8,5% de resultados regulares incluyendo
todos los enfermos de cualquier edad y patología. El estudio realizado a partir de
parámetros clínicos y radiográficos permite apreciar una buena fijación primaria de la prótesis
y una excelente estabilidad de la misma, tanto para el componente femoral como cotiloideo.In a series of 106 patients who undewent porocoated total hip arthroplasty, the
results were evaluated after three years follow-up. Harris'score was used for clinical assesment,
obtaining 91,5% of good results and 8,5% of fair results, including all the cases whichever
aetiology and age. Clinical and radiographic studies showed a good primary fixation and excellent
stability in both the acetabular and the femoral stem components
The cosmic evolution of the spatially-resolved star formation rate and stellar mass of the CALIFA survey
We investigate the cosmic evolution of the absolute and specific star
formation rate (SFR, sSFR) of galaxies as derived from a spatially-resolved
study of the stellar populations in a set of 366 nearby galaxies from the
CALIFA survey. The analysis combines GALEX and SDSS images with the 4000 break,
H_beta, and [MgFe] indices measured from the datacubes, to constrain parametric
models for the SFH, which are then used to study the cosmic evolution of the
star formation rate density (SFRD), the sSFR, the main sequence of star
formation (MSSF), and the stellar mass density (SMD). A delayed-tau model,
provides the best results, in good agreement with those obtained from
cosmological surveys. Our main results from this model are: a) The time since
the onset of the star formation is larger in the inner regions than in the
outer ones, while tau is similar or smaller in the inner than in the outer
regions. b) The sSFR declines rapidly as the Universe evolves, and faster for
early than for late type galaxies, and for the inner than for the outer regions
of galaxies. c) SFRD and SMD agree well with results from cosmological surveys.
At z< 0.5, most star formation takes place in the outer regions of late spiral
galaxies, while at z>2 the inner regions of the progenitors of the current E
and S0 are the major contributors to SFRD. d) The inner regions of galaxies are
the major contributor to SMD at z> 0.5, growing their mass faster than the
outer regions, with a lookback time at 50% SMD of 9 and 6 Gyr for the inner and
outer regions. e) The MSSF follows a power-law at high redshift, with the slope
evolving with time, but always being sub-linear. f) In agreement with galaxy
surveys at different redshifts, the average SFH of CALIFA galaxies indicates
that galaxies grow their mass mainly in a mode that is well represented by a
delayed-tau model, with the peak at z~2 and an e-folding time of 3.9 Gyr.Comment: 23 pages, 16 figures, 6 tables, accepted for publication in Astronomy
