A pravasztatin hatása tetrahidrobiopterin-érzékeny és -rezisztens praeeclampsiás placenták NO-szintáz-aktivitására = Effect of pravastatin on tetrahydrobiopterin-sensitive and -resistant NO synthase activity of preeclamptic placentas
Absztrakt:
Bevezetés: A terhességek 3–8%-ában megjelenő praeeclampsia
kezelése még jelenleg sincs megoldva. Praeeclampsiában elégtelen a NO
szintézise, ami hozzájárulhat az emelkedett vérnyomáshoz, proteinuriához, a
placenta kóros vascularisatiójához is. Praeeclampsiás placentában a csökkent
NO-szintézisnek az is oka lehet, hogy a NO-szintáz affinitása csökken a
tetrahidrobiopterinhez (BH4), ezzel BH4-rezisztencia alakul ki. Az utóbbi
években állatmodellekben és humánvizsgálatokban is megfigyelték, hogy a
pravasztatin védhet a praeeclampsia kialakulása ellen. A pravasztatin egyik
ismert pleiotrop hatása az, hogy emeli a NO-szintáz aktivitását.
Célkitűzés: A pravasztatin hatásának leírása a
BH4-rezisztens NO-szintáz aktivitására praeeclampsiás placentában.
Módszer: A NO-szintáz aktivitását placentamikroszómában
mértük C14-arginin szubsztráttal egészséges (n = 9) és praeeclampsiás (n = 9)
minta felhasználásával. A NO-szintáz aktivitását 0,02 µM, fiziológiás 0,2 µM és
farmakológiás 50 µM BH4 mellett mértük. Eredmények: 9
praeeclampsiás mintából egy volt BH4-rezisztens; a fiziológiás BH4-koncentráció
nem növelte szignifikánsan a NO-szintáz aktivitását, míg az egészséges
placenta-mikroszómák aktivitását átlagosan 60%-kal (p<0,01), a BH4-szenzitív
praeeclampsiás minták NO-szintáz-aktivitását 67%-kal (p<0,01) növelte. 10 µM
pravasztatin 32–38%-kal növelte a NO-szintáz aktivitását mindegyik
BH4-koncentrációnál az egészséges, a BH4-szenzitív és a BH4-rezisztens
praeeclampsiás mintákban is. Következtetés: 10 µM pravasztatin
a BH4-rezisztens placenta NO-szintáz-aktivitását hasonló mértékben növelte, mint
a placenta fiziológás BH4-koncentrációja (0,06–0,20 µM) a BH4-érzékeny
NO-szintáz aktivitását. Vizsgálatunkkal tehát kimutattuk, hogy a pravasztatin a
BH4-rezisztens praeeclampsiás placenta NO-szintáz-aktivitását fiziológiás

Abstract:
Introduction: The treatment of preeclampsia, which occurs in
3–8% of pregnancies, is not yet resolved. In preeclampsia, NO synthesis is
insufficient, which can contribute to hypertension, proteinuria and abnormal
vascularization of the placenta. Decreased NO synthesis in the preeclamptic
placenta may also be due to a decrease in the affinity of NO synthase for
tetrahydrobiopterin (BH4), resulting in BH4 resistance. In recent years,
pravastatin has been shown to prevent preeclampsia in animal models and in human
studies. One of the known pleiotropic effects of pravastatin is that it
increases NO synthase activity. Aim: Description of the effect
of pravastatin on BH4-resistant NO synthase activity in the preeclamptic
placenta. Method: NO synthase activity in the placental
microsome was measured with C14 arginine substrate using healthy (n = 9) and
preeclamptic (n = 9) samples. NO synthase activity was measured at 0.02 µM,
physiological at 0.20 µM and pharmacological at 50 µM BH4.
Results: One of the 9 preeclamptic patients was
BH4-resistant; physiologic BH4 concentration did not significantly increase NO
synthase activity, whereas healthy placental microsomes showed a mean increase
of 60% (p<0.01), and BH4-sensitive preeclamptic specimen showed a 67%
(p<0.01) increase. 10 µM pravastatin increased NO synthase activity by 32–38%
at each BH4 concentration in healthy, BH4-sensitive and BH4-resistant
preeclampsia samples. Conclusion: 10 µM pravastatin increased
BH4-resistant placental NO synthase activity to a similar extent as placental
physiological BH4 concentration (0.06–0.20 µM) to BH4-sensitive NO synthase
activity. The NO synthase activity of BH4-resistant preeclamptic placenta can be

Spontán aortaruptura várandósság alatt = Spontaneous aortic rupture during pregnancy
Az aortadissectio ritka kórkép. A 40 évnél fiatalabb nőkben kialakuló aortadissectiók 50%-a várandósság alatt jelentkezik. A szerzők közleményükben a terhesség harmadik trimeszterében járó multipara esetét ismertetik, akinél hirtelen halál következett be, amelynek hátterében az aorta ascendens rupturáját igazolta a boncolás. Az esetet érdekessé teszi, hogy a beteg családjában már előfordult hasonló megbetegedés. A szerzők ismertetik a tüneteket, a háttérben álló esetleges genetikai eltéréseket és áttekintik a nemzetközi irodalomban előforduló hasonló eseteket is. Orv. Hetil., 2011, 152, 929–933.

Aortic dissection is a rare entity. Half of the aortic dissection cases occur during pregnancy in women under the age of 40. The authors report a case of a multiparous woman at the third trimester of her sixth pregnancy, who died from a sudden and intractable cardiovascular shock. Autopsy revealed the dissection of the ascending aorta. The case is interesting, especially because in the pregnant woman’s family it was not the first sudden death during pregnancy. Authors review the relevant literature regarding the symptoms and the genetic basis of this rare but potentially lethal complication of pregnancy. Orv. Hetil., 2011, 152, 929–933

2-es típusú diabetesesek és nem cukorbetegek területen szerzett, belgyógyászati osztályos felvételt igénylő bakteriális infekcióinak klinikai összehasonlítása = Community-acquired bacterial infections among type 2 diabetic and non-diabetic patients hospitalized on a general medical ward: a clinical comparison
Absztrakt:
Bevezetés: Korábban azt gondoltuk, hogy a bakteriális fertőzések
cukorbetegségben gyakoribbak, súlyosabbak, nagyobb halálozással járnak, mint a
nem cukorbetegek körében. Az újabb megfigyelések azonban ellentmondásosak.
Célkitűzés: Prospektív, obszervációs vizsgálatunk célja az
volt, hogy összehasonlítsuk 2-es típusú cukorbetegek (T2DM) és nem cukorbetegek
(K) területen szerzett, belgyógyászati osztályos felvételt igénylő bakteriális
infekcióit: 1) lokalizáció, 2) kórokozóspektrum, 3) 3 hónapos halálozás alapján.
Módszer: Vizsgálatunkba konszekutív módon vontuk be a
betegeket (T2DM: n = 205, K: n = 202). Jellemeztük a két csoport infekcióit,
klinikai paramétereit, mortalitásukat, és összevetettük azokat a nemzetközi
adatokkal. Eredmények: A glikémiát jellemző paramétereket és a
BMI-t kivéve nem volt különbség a két csoport klinikai jellemzőiben. T2DM-ben a
bőr- és lágyrész-fertőzés (37,1%), a K csoportban a légúti infekció volt a
leggyakoribb (37,1%), melyeket a húgyúti infekciók követték (31,2%; 31,7%). A
bőr- és lágyrész-infekció előfordulása T2DM-ben vizsgálatunkban meghaladja a
nemzetközi adatokat (37,1% vs. 16%). T2DM-ben, a K csoporthoz viszonyítva, ezen
infekciótípusban többször találtuk Gram-pozitív és Gram-negatív kórokozók
együttes jelenlétét (23/76 vs. 5/46, p = 0,0149), a húgyúti fertőzésekben
polimikrobás infekciókat (34,0% vs. 15,1%, p = 0,0335). Mortalitásbeli
különbséget nem találtunk a két csoport között. T2DM-ben a bőr- és
lágyrész-fertőzések, a nem cukorbetegeknél a légúti infekciók okozták a legtöbb
halált. Következtetés: Területen szerzett, bakteriális infekció
miatt belgyógyászati osztályra került 2-es típusú cukorbetegeinknél a nemzetközi
adatokhoz képest nagyobb arányban találtunk bőr- és lágyrész-fertőzést;
kórokozóként a leggyakrabban Gram-pozitív és Gram-negatív baktériumok együttes
jelenlétét tudtuk kimutatni. Összességében nem találtunk különbséget a T2DM- és
a nem cukorbeteg csoport között a 3 hónapos halálozásban. Eredményeink felhívják
a figyelmet a primer prevenció fontosságára és hazai elégtelenségére a bőr- és
lágyrész-fertőzés megelőzésében 2-es típusú cukorbetegségben. Orv Hetil. 2019;

Abstract:
Introduction: Previous data showed bacterial infections among
diabetic patients to be more serious and frequent, with higher mortality rates
in comparison with non-diabetics. Recent investigations, however, are
contradictory. Aim: The goal of our prospective, observational
study was to compare patients hospitalized on a general medical ward due to
community-acquired bacterial infections with type 2 diabetes mellitus (T2DM) to
those of non-diabetics (K) by 1) infection localization, 2) spectrum of
pathogens, 3) three-month mortality rates. Method: Patients
were consecutively involved (T2DM: n = 205, K: n = 202). We characterized the
infections, clinical parameters, mortalities of the two groups, and matched them
to international data. Results: No difference regarding
clinical details of the groups were found except for glycemic parameters and
BMI. In the T2DM group the skin- and soft tissue- (37.1%), in the K patients
respiratory infections (37.1%) were the most common, followed by urinary ones
(31.2% and 31.7%, respectively). Skin- and soft tissue infection incidence among
T2DM subjects were higher compared to international results (37.1% vs. 16%).
Co-presence of Gram positive and Gram negative bacteria in the skin- and soft
tissue infections (23/76 vs. 5/46, p = 0.0149), and polymicrobial origin in the
urinary tract infections (34.0% vs. 15.1%, p = 0.0335) were found to be more
frequent in T2DM than in K. No difference regarding mortality rates were
detected. In T2DM the skin- and soft tissue while in the K group the respiratory
infections had the most death counts. Conclusions: We found
higher rates of skin- and soft tissue infections among T2DM patients
hospitalized on a general medical ward compared to international data. In total
we did not find difference regarding three-month mortality between the groups.
Our results highlight the importance of primary prevention and shows its
inadequacy concerning skin and soft tissue infections among type 2 diabetics in

A praeeclampsia kórélettana a kétlépcsős modell tükrében [The pathophysiology of preeclampsia in view of the two-stage mode]
Preeclampsia is a common and severe disease in pregnancy, a major cause of maternal and fetal morbidity and mortality. The main features of the disease are de novo hypertension after the 20th gestational week and proteinuria, and it is frequently accompanied by edema and other subjective symptoms. The origin of the disease is the placenta, but its sequelae affect multiple organ systems. According to the two-stage model of preeclampsia, the abnormal and hypoperfused placenta (stage 1) releases factors to the bloodstream, which are responsible for the maternal symptoms (stage 2). Oxidative stress, impaired function of nitric-oxide synthase, cellular and humoral immunological factors play an important role in the pathophysiology of the placenta. Endothelial dysfunction is the common denominator of the clinical symptoms. The theory explains the origins of hypertension, proteinuria, edema and other symptoms as well
