An Appreciative Inquiry of an Exemplary Hospice Interdisciplinary Group Caring for Individuals With Alzheimer’s Disease

Alzheimer’s disease is a debilitating illness that is the 6th leading cause of death among the elderly. The treatment of Alzheimer’s requires multiple interventions due to the complexity of the disease. The interdisciplinary group (IDG) model of care is considered a best practice for patients’ medical management (Molyneux, 2001). The IDG focuses on a holistic approach, which includes both patients and their caregivers. The IDG in hospice consists of professionals from different clinical disciplines whose collaborative knowledge and skills assist in caring for patients and their families. This study focused on what works well in an exemplary IDG, using appreciative inquiry as to the method of inquiry. Data were collected from 6 participants of an exemplary IDG caring for patients diagnosed with Alzheimer’s disease. The data were analyzed using the appreciative inquiry 4-D cycle: Discovery, Dream, Design, and Destiny. In the Discovery phase, 10 themes emerged, showing the connection to the Dream phase. The Dream phase led into the Design phase, focusing on provocative propositions, which bridge the best of what is with what might be. This then connected with the Destiny phase, bringing the dreams of the future to the present. I found that what works well with this exemplary IDG is the connection to other members of the team and the larger system; dedication; commitment; and valuing of team members, their patients, and patients’ families. The findings suggested the need for increased training of marriage and family therapists for IDG settings as the systemic thinking of marriage and family therapy appears to be a good fit for the IDG

An analysis of import demand for U.S. fresh citrus in Japan

Export markets for fresh citrus have taken on increasing importance in the last decade with more important increase in the Japanese market. Japan is one of the largest import markets for U.S. fresh citrus. Fresh fruit plays a special role in Japanese life, from both a cultural and economic point of view. The long-term outlook for U.S. fresh fruit is for gradual, steady growth. Many factors have helped the U.S. fresh citrus exports. The reasons are: 1) exports have benefitted from the depreciation of the U.S. dollar since 1985; 2) rising world -wide incomes; and 3) U.S. market promotion activities. The healthfulness of foods, and particularly fresh foods, is becoming more important in Japan as it is in the United States. The purpose of this study was to examine the import demand for fresh citrus in Japan and analyze the nature of price and expenditures linkages. The Almost Ideal Demand System (AIDS) of Deaton and Muellbauer (1980a, 1980b) was applied to model the Japanese market from 1991 - 1996. The findings of study indicated that fresh grapefruit dominates the import demand market for Japan, followed closely by fresh oranges. Fresh other citrus types indicated relative small importance to the Japan market. Also results show that price and quantity of fresh citrus exports have had an important effect on Japanese expenditures on imports of fresh citrus. Analysis suggests that U.S. producers can effectively promote fresh citrus in Japan market and trade negotiations between the U.S. and Japan can influence fresh citrus exports

Fatalism and its Effect on Health Behaviors in Mississippi and Louisiana Residents

The Health Belief Model maps out multiple sources to explain actions people take or do not take in regard to health behaviors. One’s sense of control over one’s own health is a central component of the health belief model. Individuals with a strong sense of control over their outcomes are said to have high self-efficacy. Individuals with a weak sense of control over their outcomes are said to have low levels of self-efficacy which can be thought of as high levels of fatalism. Previous studies suggest that individuals with high self-efficacy are better able to make changes to improve their health, while individuals with high fatalism are less able to make changes to improve their health. Relatively few studies, however, have examined the impact of other variables outside of the health belief model. Using survey data from residents in southeastern Mississippi and Louisiana, this study examined the impact of fatalism (low self-efficacy) on self-reported physical health and dietary intake controlling for social support, community health perceptions, and religiosity. The results suggest that fatalism is not statistically related to the outcomes examined. Furthermore, inclusion of the variables external to the health belief model did not alter the relationship between fatalism and the outcomes
