    Using phase space attractors to evaluate system safety constraint enforcement : case study in space shuttle mission control procedure rework

Cataloged from PDF version of thesis.Includes bibliographical references (p. 390-409).As the complexity and influence of engineering systems in modern society increases, so too does their potential to create counterintuitive and catastrophic accidents. Increasingly, the accidents encountered in these systems are defying the linearized notions of accident causality that-though developed for the simpler engineered systems of the past-are prevalently used for accident prevention today. In this dissertation, an alternative approach to accident prevention based on systems theory-the Systems-Theoretic Accident Model and Processes (STAMP) and STAMP-based hazard analysis (STPA)-is augmented with the notion of using phase space attractors to evaluate how well STAMP safety control structures enforce system safety constraints. Phase space attractors are mathematical results that emerge from the behavior of systems with dynamic structures that draw or constrain these systems to specific regions of their phase space in spite of a range of conditions. Accordingly, the goal in using this notion for the evaluation of safety constraint enforcement is to identify and analyze the attractors produced by a safety control structure to determine if it will adequately "attract" the system to safe states in spite of a range of unforeseeable conditions. Support for this approach to evaluating STAMP safety control structures is provided through the study of a safety control structure in an existing complex, socio-technical system. This case study is focused on a safety control process-referred to as Procedure Rework-used in Space Shuttle Mission Control to update procedures during in-flight operations as they are invalidated by changes in the state of the Space Shuttle and its environment.(cont.) Simulation models of procedure rework are developed through physical and human factors principles and calibrated with data from five Space Shuttle missions; producing simulation results with deviations from the historical data that are-as characterized by Theil Inequality Statistics-small and primarily due to cycles and noise that are not relevant to the models' purpose. The models are used to analyze the attractor produced by the Procedure Rework Process across varied conditions, including a notional crewed spacecraft mission to a distant celestial body. A detrimental effect in the process is identified-and shown to be potentially far more severe than light delay on a mission to a distant celestial body-and approaches to mitigating the effect are explored. Finally, the analysis conducted is described as a generalizeable process for using phase space attractors to evaluate system safety constraint enforcement in engineering systems.by Brandon D.     Free-flight investigation of forebody blowing for stability and control

    A free-flight wind-tunnel investigation was conducted on a generic fighter model with forebody pneumatic vortex control for high angle-of-attack directional control. This is believed to be the first flight demonstration of a forebody blowing concept integrated into a closed-loop flight control system for stability augmentation and control. The investigation showed that the static wind tunnel estimates of the yaw control available generally agreed with flight results. The control scheme for the blowing nozzles consisted of an on/off control with a deadband. Controlled flight was obtained for the model using forebody blowing for directional control to beyond 45 deg. angle of attack

    Overview of Dynamic Test Techniques for Flight Dynamics Research at NASA LaRC (Invited)

    An overview of dynamic test techniques used at NASA Langley Research Center on scale models to obtain a comprehensive flight dynamics characterization of aerospace vehicles is presented. Dynamic test techniques have been used at Langley Research Center since the 1920s. This paper will provide a partial overview of the current techniques available at Langley Research Center. The paper will discuss the dynamic scaling necessary to address the often hard-to-achieve similitude requirements for these techniques. Dynamic test techniques are categorized as captive, wind tunnel single degree-of-freedom and free-flying, and outside free-flying. The test facilities, technique specifications, data reduction, issues and future work are presented for each technique. The battery of tests conducted using the Blended Wing Body aircraft serves to illustrate how the techniques, when used together, are capable of characterizing the flight dynamics of a vehicle over a large range of critical flight conditions

    Unmanned Aircraft Systems (UAS) Traffic Management (UTM) National Campaign II

    The Unmanned Aircraft System (UAS) Traffic Management (UTM) effort at NASA aims to enable access to low-altitude airspace for small UAS. This goal is being pursued partly through partnerships that NASA has developed with the UAS stakeholder community, the FAA, other government agencies, and the designated FAA UAS Test Sites. By partnering with the FAA UAS Test Sites, NASA's UTM project has performed a geographically diverse, simultaneous set of UAS operations at locations in six states. The demonstrations used an architecture that was developed by NASA in partnership with the FAA to safely coordinate such operations. These demonstrations-the second or 'Technical Capability Level (TCL 2)' National Campaign of UTM testing-was performed from May 15 through June 9, 2017. Multiple UAS operations occurred during the testing at sites located in Alaska, Nevada, Texas, North Dakota, Virginia, and New York with multiple organizations serving as UAS Service Suppliers and/or UAS Operators per the specifications provided by NASA. By engaging various members of the UAS community in development and operational roles, this campaign provided initial validation of different aspects of the UTM concept including: UAS Service Supplier technologies and procedures; geofencing technologies/conformance monitoring; ground-based surveillance/sense and avoid; airborne sense and avoid; communication, navigation, surveillance; and human factors related to UTM data creation and display. Additionally, measures of performance were defined and calculated from the flight data to establish quantitative bases for comparing flight test activities and to provide potential metrics that might be routinely monitored in future operational UTM systems

    Cataclysmic Variables from SDSS II. The Second Year

    The first full year of operation following the commissioning year of the Sloan Digital Sky Survey has revealed a wide variety of newly discovered cataclysmic variables. We show the SDSS spectra of forty-two cataclysmic variables observed in 2002, of which thirty-five are new classifications, four are known dwarf novae (CT Hya, RZ Leo, T Leo and BZ UMa), one is a known CV identified from a previous quasar survey (Aqr1) and two are known ROSAT or FIRST discovered CVs (RX J09445+0357, FIRST J102347.6+003841). The SDSS positions, colors and spectra of all forty-two systems are presented. In addition, the results of follow-up studies of several of these objects identify the orbital periods, velocity curves and polarization that provide the system geometry and accretion properties. While most of the SDSS discovered systems are faint (>18th mag) with low accretion rates (as implied from their spectral characteristics), there are also a few bright objects which may have escaped previous surveys due to changes in the mass transfer rate.Comment: Accepted for publication in The Astronomical Journal, Vol. 126, Sep. 2003, 44 pages, 25 figures (now with adjacent captions), AASTeX v5.

    Factors predicting treatment of World Trade Center-related lung injury : a longitudinal cohort study

    The factors that predict treatment of lung injury in occupational cohorts are poorly defined. We aimed to identify patient characteristics associated with initiation of treatment with inhaled corticosteroid/long-acting beta-agonist (ICS/LABA) >2 years among World Trade Center (WTC)-exposed firefighters. The study population included 8530 WTC-exposed firefighters. Multivariable logistic regression assessed the association of patient characteristics with ICS/LABA treatment for >2 years over two-year intervals from 11 September 2001-10 September 2017. Cox proportional hazards models measured the association of high probability of ICS/LABA initiation with actual ICS/LABA initiation in subsequent intervals. Between 11 September 2001-1 July 2018, 1629/8530 (19.1%) firefighters initiated ICS/LABA treatment for >2 years. Forced Expiratory Volume in 1 s (FEV1), wheeze, and dyspnea were consistently and independently associated with ICS/LABA treatment. High-intensity WTC exposure was associated with ICS/LABA between 11 September 2001-10 September 2003. The 10th percentile of risk for ICS/LABA between 11 September 2005-10 Septmeber 2007 was associated with a 3.32-fold increased hazard of actual ICS/LABA initiation in the subsequent 4 years. In firefighters with WTC exposure, FEV1, wheeze, and dyspnea were independently associated with prolonged ICS/LABA treatment. A high risk for treatment was identifiable from routine monitoring exam results years before treatment initiation

    Effects of Anacetrapib in Patients with Atherosclerotic Vascular Disease

    BACKGROUND: Patients with atherosclerotic vascular disease remain at high risk for cardiovascular events despite effective statin-based treatment of low-density lipoprotein (LDL) cholesterol levels. The inhibition of cholesteryl ester transfer protein (CETP) by anacetrapib reduces LDL cholesterol levels and increases high-density lipoprotein (HDL) cholesterol levels. However, trials of other CETP inhibitors have shown neutral or adverse effects on cardiovascular outcomes. METHODS: We conducted a randomized, double-blind, placebo-controlled trial involving 30,449 adults with atherosclerotic vascular disease who were receiving intensive atorvastatin therapy and who had a mean LDL cholesterol level of 61 mg per deciliter (1.58 mmol per liter), a mean non-HDL cholesterol level of 92 mg per deciliter (2.38 mmol per liter), and a mean HDL cholesterol level of 40 mg per deciliter (1.03 mmol per liter). The patients were assigned to receive either 100 mg of anacetrapib once daily (15,225 patients) or matching placebo (15,224 patients). The primary outcome was the first major coronary event, a composite of coronary death, myocardial infarction, or coronary revascularization. RESULTS: During the median follow-up period of 4.1 years, the primary outcome occurred in significantly fewer patients in the anacetrapib group than in the placebo group (1640 of 15,225 patients [10.8%] vs. 1803 of 15,224 patients [11.8%]; rate ratio, 0.91; 95% confidence interval, 0.85 to 0.97; P=0.004). The relative difference in risk was similar across multiple prespecified subgroups. At the trial midpoint, the mean level of HDL cholesterol was higher by 43 mg per deciliter (1.12 mmol per liter) in the anacetrapib group than in the placebo group (a relative difference of 104%), and the mean level of non-HDL cholesterol was lower by 17 mg per deciliter (0.44 mmol per liter), a relative difference of -18%. There were no significant between-group differences in the risk of death, cancer, or other serious adverse events. CONCLUSIONS: Among patients with atherosclerotic vascular disease who were receiving intensive statin therapy, the use of anacetrapib resulted in a lower incidence of major coronary events than the use of placebo.
