    In this paper I analyse the intranational conflicts of Post-Cold War Mozambique with a particular focus on women’s struggle against sexist exploitation by Frelimo national politics in two films, Virgem Margarida, by Licínio de Azevedo (2012), and O jardim de outro homem, by João Luís Sol de Carvalho (2007). I also demonstrate how they engage with the new possibilities opened up by the cinematography of “accented cinema” (Hamid Naficy) replacing the old Marxist paradigms of “Third Cinema” and armed struggle.Neste artigo pretendo analisar a representação dos conflitos intranacionais de Moçambique, pós-Guerra Fria, focalizando a luta da mulher contra a exploração sexista da política nacional frelimiana em dois filmes: Virgem Margarida, de Licínio de Azevedo (2012), e O jardim de outro homem, de João Luís Sol de Carvalho (2007).     [EN] This paper discusses the 3D photogrammetric modelling and documentation of the Western High Gate at Medinet Habu by the University of Chicago’s Oriental Institute, Epigraphic Survey Project.     Meadows and more: a botanical journal of five days on the Uists, Benbecula and Eriskay

    In the first week of July 2018, the Floodplain Meadows Partnership (FMP) held its annual steering group meeting with field excursions on the Outer Hebrides. The aim of the field part of our meeting was to familiarise ourselves with wet meadows (including those on machair) of the Uists and Benbecula, and compare their composition and ecology with related habitats further south in the UK. However, a chance for us to explore the other habitats of these islands was too good an opportunity to miss. Hence five of us arrived on the islands a few days before the main group and roamed more extensively, looking at a wide range of habitats in addition to meadows. The lead author’s role in this botanical journal was to note incidental observations made in addition to the formal study of the meadows. Hilary Wallace directed the quadrat recording in wet meadows, and the overall programme of work was coordinated by David Gowing and Emma Rothero of the Open University. Even during the FMP field excursions, we could not ignore other habitats entirely and Stewart Clarke (National Trust Freshwaters and Estuaries Specialist) paid special attention to the aquatic flora

    Patient harm and institutional avoidability of out-of-hours discharge from intensive care: An analysis using mixed methods

This is an open access article distributed under the Creative Commons Attribution License 4.0 (CCBY). https://creativecommons.org/licenses/by/4.0/Objective: Out-of-hours discharge from ICU to the ward is associated with increased in-hospital mortality and ICU readmission. Little is known about why this occurs. We map the discharge process and describe the consequences of out-of-hours discharge to inform practice changes to reduce the impact of discharge at night. Design: This study was part of the REFLECT mixed methods study. We defined out-of-hours discharge as 16:00 - 07:59h. We undertook 20 in-depth case record reviews where in-hospital death after ICU discharge had been judged ‘probably avoidable’ in previous retrospective structured judgement reviews, and 20 where patients survived. We conducted semi-structured interviews with 55 patients, family members and staff with experience of ICU discharge processes. These, along with a stakeholder focus group, informed ICU discharge process mapping using the Human Factors-based Functional Analysis Resonance Method (FRAM). Setting: Three UK NHS hospitals, chosen to represent different hospital settings. Subjects: Patients discharged from ICU, their families and staff involved in their care. Interventions None. Measurements and Main Results: Out-of-hours discharge was common. Patients and staff described out-of-hours discharge as unsafe due to a reduction in staffing and skill mix at night. Patients discharged out-of-hours were commonly discharged prematurely, had inadequate handover, were physiologically unstable and did not have deterioration recognised or escalated appropriately. We identified five interdependent functions key to facilitating timely ICU discharge: multi-disciplinary team decision for discharge; patient prepared for discharge; bed meeting; bed manager allocation of beds; and ward bed made available. Conclusion: We identified significant limitations in out-of-hours care provision following overnight discharged from ICU. Transfer to the ward before 16:00 should be facilitated where possible. Our work highlights changes to help make day time discharge more likely. Where discharge after 16:00 is unavoidable, support systems should be implemented to ensure the safety of patients discharged from ICU at night.Peer reviewedFinal Published versio

    Choosing Motherhood : The complexities of pregnancy decision-making among young black women 'looked after' by the State

Copyright 2013 the Authors, published by Elsevier Ltd.Objective: This paper addresses the experiences of a group of young black teenage mothers looked after by the State, most of whom were also either migrants or asylum seekers. The paper explores the experience of discovery of pregnancy, attempts to seek professional help and the eventual decision to continue with the pregnancy. Design: An interpretative study with in-depth interviews. Settings: Interviews were carried out in the participants’ homes and focussed on their experiences of pregnancy decision-making. Participants: 15 young women (aged 16-19), from black minority ethnic groups, with a history of care (past or present), currently pregnant or mothers of a child no older than two years of age. Findings: All the pregnancies were unexpected: eight of the informants conceived as a result of rape and seven while in a relationship. All the young women chose motherhood over abortion despite their complex social and pregnancy background. Conclusions: The importance of social positioning of migrants in terms of the cluster of negative aspects and environmental disadvantage generally experienced by most immigrants in the host country is raised in this paper. Care practices of pregnant women with complex social factors were little observant of woman-centred care approaches.Peer reviewedFinal Published versio

    Randomized clinical trial of nitazoxanide or sofosbuvir/daclatasvir for the prevention of SARS-CoV-2 infection

    BACKGROUND: The COVER trial evaluated whether nitazoxanide or sofosbuvir/daclatasvir could lower the risk of SARS-CoV-2 infection. Nitazoxanide was selected given its favourable pharmacokinetics and in vitro antiviral effects against SARS-CoV-2. Sofosbuvir/daclatasvir had shown favourable results in early clinical trials. METHODS: In this clinical trial in Johannesburg, South Africa, healthcare workers and others at high risk of infection were randomized to 24 weeks of either nitazoxanide or sofosbuvir/daclatasvir as prevention, or standard prevention advice only. Participants were evaluated every 4 weeks for COVID-19 symptoms and had antibody and PCR testing. The primary endpoint was positive SARS-CoV-2 PCR and/or serology ≥7 days after randomization, regardless of symptoms. A Poisson regression model was used to estimate the incidence rate ratios of confirmed SARS-CoV-2 between each experimental arm and control. RESULTS: Between December 2020 and January 2022, 828 participants were enrolled. COVID-19 infections were confirmed in 100 participants on nitazoxanide (2234 per 1000 person-years; 95% CI 1837-2718), 87 on sofosbuvir/daclatasvir (2125 per 1000 person-years; 95% CI 1722-2622) and 111 in the control arm (1849 per 1000 person-years; 95% CI 1535-2227). There were no significant differences in the primary endpoint between the treatment arms, and the results met the criteria for futility. In the safety analysis, the frequency of grade 3 or 4 adverse events was low and similar across arms. CONCLUSIONS: In this randomized trial, nitazoxanide and sofosbuvir/daclatasvir had no significant preventative effect on infection with SARS-CoV-2 among healthcare workers and others at high risk of infection
