Ebola virus disease epidemic in West Africa: Lessons learned and issues arising from West African countries

All rights reserved.The current Ebola virus disease (EVD) outbreak ravaging three nations in West Africa has affected more than 14,000 persons and killed over 5,000. It is the longest and most widely spread Ebola epidemic ever seen. At the time of this overview (written November 2014), having affected eight different nations, Nigeria and Senegal were able to control and eliminate the virus within a record time. Ghana has successfully, to date, kept the virus away from the country, despite economic and social relationships with affected nations. What lessons can we learn from Nigeria, Senegal and Ghana in the current epidemic? How can the world improve the health systems in low- and middle-income countries to effectively manage future outbreaks? Recently, the Royal College of Physicians launched a new partnership with the West African College of Physicians to curtail the effects of HIV/AIDS, malaria and tuberculosis in the region. We believe that strengthened health systems, skilled human resources for health and national ownership of problems are key to effective management of outbreaks such as EVD
Integrated waveguide and nanostructured sensor platform for surface-enhanced Raman spectroscopy
Limitations of current sensors include large dimensions, sometimes limited sensitivity and inherent single-parameter measurement capability. Surface-enhanced Raman spectroscopy can be utilized for environment and pharmaceutical applications with the intensity of the Raman scattering enhanced by a factor of 106. By fabricating and characterizing an integrated optical waveguide beneath a nanostructured precious metal coated surface a new surface-enhanced Raman spectroscopy sensing arrangement can be achieved. Nanostructured sensors can provide both multiparameter and high-resolution sensing. Using the slab waveguide core to interrogate the nanostructures at the base allows for the emission to reach discrete sensing areas effectively and should provide ideal parameters for maximum Raman interactions. Thin slab waveguide films of silicon oxynitride were etched and gold coated to create localized nanostructured sensing areas of various pitch, diameter, and shape. These were interrogated using a Ti:Sapphire laser tuned to 785-nm end coupled into the slab waveguide. The nanostructured sensors vertically projected a Raman signal, which was used to actively detect a thin layer of benzyl mercaptan attached to the sensors
Physiological Response Of Laying Birds To Neem (Azadirachta Indica) Leaf Meal-Based Diets: Body Weight Organ Characteristics And Haematology
A 12-weeks feeding trial was conducted to evaluate the effects of Neem (Azadirachta indica) leafmeal (NLM) on body weight gain, carcass and organ characteristics and haematological values of laying hens. The leaves were harvested, chopped to facilitate drying in the sun until they became crispy but still greenish in coloration. The Sun-dried leaves were milled using a hammer mill to produce the leaf meal. Four layers diets were formulated to contain the NLM at 0%,5%, 10% and 15% dietary levels respectively and were used to feed 120 Shikka brown layers already 10 months in lay. The birds were divided into 4 groups of 30 each and randomly assigned to the 4 treatment diets in a completely randomized design (CRD). NLM did not show any appreciable difference in weight gain between the birds at 0% and those at 5%, 10% dietary levels. Carcass weight, dressed weight, liver, heart and gizzard weights were significantly (P<0.05) increased at 5% dietary level of NLM. There were no significant difference in Hb and PCV between birds on O% and 5% treatment diets. However, these differed significantly (P<0.05%) from those of birds on 10% and 15% treatment diets. There were variations in the differential WB count , marked lymphocytopenia adversely affected the total leucocyte counts in the birds on 5%, 10% and 15% treatment diets. The results of this study suggest that laying birds could tolerate 5%- 15% dietary levels of NLM without deleterious effects
Effect of Toluene and Dioctylphthalate on the Rebar Corrosion of Medium Carbon Steel in Seawater and Cassava Fluid
Chemical mitigation is regularly used as one of the principal prevention and control techniques in reinforcement corrosion. Hence this study presents the effect of toluene and dioctylphthalate on the rebar corrosion of medium carbon steel in seawater and cassava fluid with a view to determining inhibitive potentials of the different in-hibitors in the two media. Gravimetric and voltametric techniques were employed in this study and a total of forty-five corrosion coupons of different dimensions were produced. Forty coupons were used for gravimetry and the remaining five for corrosion potentials measurements. Eight of the samples were used as control; while other eight samples were admixed with dioctylphthalate and toluene in concrete cubes. It was later immersed in seawater and cassava fluid for a total duration of 32 days and the measurements were taken at interval of 4 days in order to determine the corrosion rates in mils per year (mmpy). Two controls and admixed samples were later immersed in seawater and cassava fluid, respectively, for durations of thirty-two days to determine the corrosion potentials using a voltmeter and a Copper-Copper Sulphate Electrode (Cu/CuSO4). The pH of each medium was also measured throughout the period of exposure. The results obtained showed that all the samples except the control samples, displayed some degree of inhibition. The inhibition levels for the admixed samples in seawater were on the higher side compared with those in cassava fluid. The inhibition efficiencies for different inhibitors followed different trends in different environment. The inhibition efficiencies for toluene in cassava fluid and seawater were 21.64% and 45.78% respectively. The study concluded that organic inhibitors were effective in inhibiting corrosion in cyanide and chloride contaminated concrete cubes

Effect of xylazine sedation on some clinico-physiological and haematological parameters in Sokoto red goats
Xylazine hydrochloride (Xylazin® injection, 2% solution contains xylazine hydrochloride 23.33 mg/kg, Indian immunological Ltd., India) is an 2-adrenergic agonist used in animals. It is a potent sedative/hypnotic agent (Hall and Clarke, 1983). Chemically, it is 2(2, 6-dimethylphenylamino) -4H-5, 6-dihydro-1, 3-thiamine hydrochloride (Adams, 2001). Xylazine is classified pharmacologically as an effective sedative, analgesic, muscle relaxant, immobilizing and hypnotic agent in domestic animals (Torre and Erausquine, 1988; Ewing, 1990; Adams, 2001). Xylazine is also known to significantly ameliorate the effects induced by stress stimuli (Ali et al., 2006). It does not possess the undesirable side-effects and deficiencies of the phenothiazine-derived tranquilizer (Mohammed and Yelwa, 1993), and xylazine infusion should not be used in horses during transition from isoflurane anaesthesia to recovery (Wagner et al., 2008). As a well-defined breed in Africa, Sokoto red goats are known for quality skin which is in high demand for leather product with a high retail value (Devendra and McLeroy, 1988). Sokoto State is estimated to have 2.46 million goats with Sokoto red goats found virtually in every household within the state (Anonymous, 1992).The objective of this research was to evaluate the sedative effect of xylazine in Sokoto red goats by way of ascertaining the onset of action, duration of action, effect on some clinico-physiological and haematological parameters
Supportive supervision for medicines management in government health facilities in Kiambu County, Kenya: A health workers’ perspective
Introduction: effective supportive supervision is widely recognized as essential for optimal management of medicines in government healthfacilities and also in contributing towards improved access and utilization of health services. This study sought to examine the extent supportivesupervision for medicines management in government health facilities from a health worker perspective. Methods: a cross-sectional study was done targeting health workers managing medicines in government health facilities in Kiambu County. One hundred and thirty eight respondents took part in the study which explored the quality of supportive supervision from a health worker's perspective, and also examined the factors influencing their contentment with the level of supervision received. A statistical analysis was done using SPSS 21 and Excel 2013. Results: supervisory visits from all levels of health management were not regularly done, standard checklists were not routinely used, and action plans irregularly developed and followed up. Only 54 (38.6%) respondents were satisfied with the levels of supportive supervision that they received,with satisfaction significantly differing across the professional cadres, <2 (12, n = 138) = 29.762, p = .003; across the different tiers of healthfacilities, rs (138) = 0.341, p < .001; and with the education levels of the respondents, rs (138) = 0.381, p < .001. Conclusion: the study concluded that supportive supervision for medicines management that government health facilities received was still inadequate, and health workers were dissatisfied with the level of supervision that they received. The study recommends a review of the support supervision policy at the county level to address the unearthed inefficiencies and improve supervision for medicines management in government health facilities
