    Aim To compare the efficacy and safety of high-dose intravenous oxytocin and sustained-release dinoprostone vaginal pessaries for cervical ripening and labor induction in pregnant patients at term with poor Bishop scores. Material and Methods Women at term with a Bishop score 4 and 6 were randomized into two groups to undergo induction of labor with either high-dose oxytocin administered intravenously (n=90) or dinoprostone-only vaginal pessary without oxytocin augmentation (n=90). The main outcome measures were rate of cesarean delivery, induction to delivery interval, number of deliveries achieved within 4, 8, 12, and 16h of labor induction, maternal complications during induction, fetal outcome, and total hospital stay. In this study, per-protocol analysis was performed. Results There were fewer cesarean deliveries with oxytocin compared to dinoprostone-only groups (7/79 vs 14/89); however, the difference was not statistically significant. The inductiondelivery intervals (7.9h vs 12.0h, P<0.001; and 5.7 vs 10.4h, P<0.001; oxytocin vs dinoprostone-only for primiparous and multiparous patients, respectively) were significantly shorter in oxytocin-induced patients compared to dinoprostone-only. A significantly higher percentage of patients delivered in the oxytocin group compared to the dinoprostone-only group in 4, 8, 12, 16, and 20h. Conclusion Intravenous oxytocin is effective to stimulate labor at term for patients with Bishop scores 4 and 6, with a shorter time interval from induction to vaginal delivery

    Attitudes of nursing and medical school students towards ageism

    The present study aimed to evaluate the attitudes of nursing and medical school students towards age discrimination and to determine the association between these attitudes and various variables. A total of 662 students, 328 from the Faculty of Nurse Education and 334 from the Faculty of Medicine, participated in the present descriptive and cross-sectional study.Data were collected using Data Form for socio-demographic characteristicsand Ageism Attitude Scale (AAS). Descriptive statistics, t-test, Mann-Whitney U test, Chi-square test, One-Way ANOVA, and Kruskal Wallis test were used to evaluate the data using IBM SPSS Statistics for Windows (Version 20.0).Nursing school students had higher points in restricting the life of elderly people, positive discrimination towards them and Ageism Attitude Scale, but had lower points in negative discrimination towards them compared to medical school students. While the points of positive discrimination towards elderly and Ageism Attitude Scale of the students of two schools were not significantly different (p&gt;0.05), points of restricting the life of elderly people and negative discrimination towards elderly varied significantly between the students of two schools (p [Med-Science 2020; 9(4.000): 848-55

    Profuse bleeding from the lower uterine segment secondary to placenta praevia/accreta during caesarean delivery is a challenging problem in obstetrics. We present our experiences using intrauterine Foley balloon tamponade for the conservative management of post-partum haemorrhage from the lower uterine segment. Intraoperative haemostasis was achieved in all women who were unresponsive to other conservative methods. Foley balloon tamponade may be considered in the management of lower uterine segment bleeding at caesarean delivery

    Colon Resection For Endometriosis

    Endometriosis affects the women during reproductive period and can cause functional disorders. Sometimes general surgical intervention is necessary because of disease boundary. Especially the sigmoid colon and rectum are affected due to the close neighboring. In such a case, treatment must be individualized according to the patient and symptoms. If the lesion has penetrated the entire bowel wall, bowel resection may be inevitable. Laparoscopic resection of the sigmoid colon or rectum can be performed safely in this situation. When laparoscopic resection cannot be possible because of technical difficulties, open resection may be performed for treatment. Here we present two cases, one open and one laparoscopic colon resection performed due to endometriosis.WoSScopu

    Prognostic factors of radiosynovectomy in haemophilia patients with inhibitors: Survival analysis in a 19-year period

    Introduction People with haemophilia (PwH) with inhibitors have an increased risk of bleeding and early development of progressive arthropathy. Radiosynovectomy (RS) has been effective in dramatically reducing the frequency of haemarthroses. In the present study, the mid- and long-term results of the efficacy of RS in PwHs with inhibitors and prognostic factors that influence success and failure of RS were presented. Material and Method Radiosynovectomy was performed in 51 joints of 22 PwHs with inhibitors diagnosed with chronic haemophilic synovitis between January 2000 and December 2018. Two patients were lost to follow-up and four joints were excluded. Number of bleeding episodes within the pre- and post-treatment 6 months were documented. Treatment failure was defined as need for repeat RS injection. Results Results of 47 RS were analysed. The mean bleeding frequency of the joints was 11.2 +/- 6.2 (median 9) within the last 6 months in the pre-treatment evaluation. After the treatment, the mean bleeding frequency of the joints decreased to 1.2 +/- 2.8 (median 0) for first 6 months (P < .0001). The cumulative survival rate at 12 months was 87% and 78% at 36 months. The receiver operating characteristic (ROC) curve analysis revealed that cut-off points of 12 bleeding episodes within the last 6 months (sensitivity, 71.4; specificity, 81.8P = .0022) and an inhibitor titre of 63.4 BU (sensitivity, 57.1; specificity, 75.8;P = .31) were threshold levels for a predisposition for failure. Conclusion Radiosynovectomy is an effective and safe intervention in PwHs with inhibitors. Bleeding frequency is a prognostic marker for the success of RS treatment. Patients who have more than 12 bleeding episodes within the last 6 months before the RS treatment have a higher rate of failure

    Nuclear factor-κB expression in the endometrium of normal and overweight women with polycystic ovary syndrome

    The aim of this study was to investigate whether the expression levels of endometrial NFκB p65 differ between normal weight and overweight PCOS women and to compare them with BMI-matched control subjects without PCOS. The study group comprised 20 normal weight (BMI: 18.5–24.9 kg/m2) and 15 overweight PCOS women (BMI: 25–29.9 kg/m2) with infertility. Healthy fertile women without PCOS were recruited as the control group. The patients in the normal weight PCOS group and control group were age and BMI-matched. Endometrial samples were obtained during the mid-luteal phase for immunohistochemical staining. The H-Score method was used to evaluate NF-κB p65 (Rel A) expression. Both normal and overweight PCOS women demonstrated significantly higher endometrial NF-κB p65 expression than the women without PCOS. The H-scores of endometrial NF-κB p65 expression were similar in both groups of PCOS women. NF-κB p65 was positively correlated with serum insulin, HOMA-IR and total testosterone levels in PCOS women. By leading to pathological inflammation, an increase in NF-κB p65 expression in the endometrium of normal and overweight PCOS women may contribute to PCOS-related subfertility.Impact statement What is already known on this subject: Although the pathogenesis of PCOS has not yet been clarified, low-grade chronic inflammation is gradually being established as an important pathogenetic factor. Increased levels of inflammatory cytokines such as IL-6 and TNF-α have been reported in women with PCOS. Causes of pathological endometrial inflammation may arise from either a local endometrial disease or linked to diseases which are located in a distant reproductive tissue. Nevertheless, possible role of endometrial NF-κB, basic cellular regulatory of inflammation, in the pathophysiology of PCOS related implantation defect has not been elucidated yet. What do the results of this study add: This study provides first and novel insights into the relationship between PCOS related infertility and pathological endometrial inflammation. We demonstrated that there is a close association between PCOS and pathological endometrial inflammation. Moreover, we clearly showed that pathological endometrial inflammation occurs in both normal and overweight women with PCOS. Further, endometrial NF-κB p65 (Rel A) expression were found to be positively correlated with serum insulin levels and hyperandrogenism in overweight PCOS women. What are the implications of these findings for clinical practice: If we can analyse pathological endometrial inflammation by measuring endometrial NF-κB p65 (Rel A) expression, treatment could be directed towards eliminating the source of pathological endometrial inflammation

    Three different citrus-producing regions of Turkey—Edremit Gulf, Coastal Aegean, and eastern Black Sea—were surveyed in 2005 and 2006. A total of 119 samples were collected from Satsuma Owari mandarin (Citrus unshiu) trees grafted on Citrus tristeza virus (CTV)-resistant Poncirus trifoliata rootstocks in commercial groves and home gardens. All samples were tested for the presence of CTV by direct tissue blot immunoassay (DTBIA) and double antibody sandwich enzyme-linked immunosorbent assay (DAS-ELISA) using young shoots. The samples that tested positive were indexed for biological properties. The results obtained from DTBIA tests showed that 20 of 119 (16.8%) tested trees were infected with CTV and 99 trees were virus free. All samples that tested positive in DTBIA were also positive in DAS-ELISA. After biological indexing, none of these 20 isolates showed any symptoms on sour orange, grapefruit, and sweet orange plants, but all isolates induced vein-clearing symptoms on Mexican lime (Citrus aurantifoli) in 9 months. Some isolates also caused leaf-cupping and chlorosis in Mexican lime. However, 15 months after initial grafting, all isolates induced mild to moderate stem-pitting on Mexican lime. In addition, young tissues, petioles, and leaf samples were collected periodically at monthly intervals for 1 year (2006) from a 25-year-old Satsuma Owari mandarin grafted on P. trifoliata in the Edremit Gulf. CTV was readily detected in tissue blots of young shoots and petioles from CTV-infected plants during the autumn, winter, and spring seasons. Similarly, the highest ELISA values were obtained in April; the lowest values were noted in September.
