Disseny i implementació d'un prototip de placa electrònica amb protocol de comunicació MODBUS per a aplicació industrial
El PFC té com a objectiu el disseny i posterior implementació d'un prototip de placa electrònica industrial, capaç de capturar i gestionar dades de qualsevol procés automatitzat mitjançant protocol de comunicació Modbus. Particularment, el nostre projecte permet fer una adquisició de dades de tots els possibles sensors i actuadors que poden formar part d'un procés industrial i transmetre-les a un PC per comunicació RS-485. Posteriorment, es tracten tot el seguit de dades, es verifiquen i gestionen a un correcte ús automatitzat. El desenvolupament del projecte, comprèn el disseny del prototip, la insolació de la placa de circuit imprès, muntatge del prototip, el software de programació de la placa en un entorn automatitzat amb protocol Modbus i una interfÃcie grà fica perquè l'usuari pugui interactuar amb la placa. Tot el mòdul d'entrades analògiques i entrades / sortides digitals està controlat per un microcontrolador PIC 18F4620

New aspects in cardiorenal syndrome and HFpEF
Cardiorenal syndrome; Chronic renal failure; DiureticsSÃndrome cardiorrenal; Insuficiencia renal crónica; DiuréticosSÃndrome cardiorenal; Insuficiència renal crònica; DiürèticsCardiorenal syndrome (CRS) is a complex disease in which the heart and kidneys are simultaneously affected, and subsequently, the malfunction of one organ promotes the deterioration of the other. Heart failure (HF) with preserved ejection fraction (HFpEF) is the most common form of HF. The pathophysiology of CRS is not well known and several mechanisms have been proposed. An elevation of central venous pressure seems to be one of the key points to consider, among others such as an increase in intraabdominal pressure. Several diagnostic tools have been identified to establish the diagnosis of CRS in patients with HFpEF. Currently, the availability of biomarkers of renal and cardiac injury, the use of pulmonary ultrasound, the monitoring of the size of the inferior vena cava and the study of the renal venous pattern offer a new dimension in accurately diagnosing and quantifying organ damage in CRS. Beyond the symptomatic treatment of congestion, until recently specific therapeutic tools for patients with CRS and HFpEF were not available. Interestingly, the development of new drugs such as the angiotensin/neprilysin inhibitors and sodium-glucose cotransporter-2 (SGLT-2) inhibitors offer new therapeutic strategies with potential benefits in reduction of cardiorenal adverse outcomes in this population. Randomized clinical trials that focus on patients with HFpEF are currently ongoing to delineate optimal new treatments that may be able to modify their prognosis. In addition, multidisciplinary teamwork (nephrologist, cardiologist and nurse) is expected to decrease the number of visits and the rate of hospitalizations, with a subsequent patient benefit

Diabetes mellitus (DM) carries a higher risk of HF and a worse prognosis. Furthermore, almost half of people with DM will have chronic kidney disease (CKD), which means that DM is the main cause of kidney failure. The triad of cardiorenal syndrome and diabetes is known to be associated with increased risk of hospitalization and mortality. Cardiorenal units, with a multidisciplinary team (cardiologist, nephrologist, nursing), multiple tools for diagnosis, as well as new treatments that help to better control cardio-renal-metabolic patients, offer holistic management of patients with CRS. In recent years, the appearance of drugs such as sodium-glucose cotransporter type 2 inhibitors, have shown cardiovascular benefits, initially in patients with type 2 DM and later in CKD and heart failure with and without DM2, offering a new therapeutic opportunity, especially for cardiorenal patients. In addition, glucagon-like peptide-1 receptor agonists have shown CV benefits in patients with DM and CV disease in addition to a reduced risk of CKD progression
Proposta didà ctica : obtenció de nanopartÃcules de plata a partir d'extracte de te verd
En els darrers temps ha adquirit gran importà ncia la sÃntesi de nanopartÃcules metà l·liques a partir d'extractes naturals fornint mètodes de preparació més respectuosos amb el medi ambient (l'anomenada "sÃntesi verda"). Molts extractes de plantes s'han emprat per obtenir nanopartÃcules principalment de plata i or. Aquest treball proposa la utilització de te verd com a extracte a partir del qual sintetitzar nanopartÃcules de plata. Tot i que la bibliografia esmenta ja la sÃntesi de nanopartÃcules de plata amb te, tractarem d'aprofundir en les condicions experimentals en què té lloc. La caracterització de les nanopartÃcules es farà mitjançant la seva absorbà ncia en la regió del visible. Aquesta proposta didà ctica està destinada a alumnes de 4t ESO o batxillerat. Per tal de fer un seguiment de l'alumnat i una avaluació formativa de l'activitat es proposa que els alumnes facin un bloc explicatiu de cada etapa de la proposta didà ctica.In recent times, the synthesis of metal nanoparticles from natural extracts has become veryimportant, providing more environmentally friendly preparation methods (the so-called "green synthesis").Many plant extracts have been used to obtain nanoparticles, mainly silver and gold. This work proposes theuse of green tea as extract to synthesize silver nanoparticles. Although the literature already mentions thesynthesis of silver nanoparticles with tea, we will try to go in deep into the experimental conditions in which ittakes place. The characterization of the nanoparticles will be done by their absorbance in the visible region.This didactic proposal is addressed to students of 4th ESO and "batxillerat" (upper-secondary education). Inorder to monitor the students and their formative evaluation of the activity, it is proposed to make anexplanatory blog of each stage of the didactic proposal.En los últimos tiempos ha adquirido gran importancia la sÃntesis de nanopartÃculas metálicas a partir de extractos naturales proporcionando métodos de preparación más respetuosos con el medio ambiente (la llamada "sÃntesis verde"). Muchos extractos de plantas se han empleado para obtener nanopartÃculas principalmente de plata y oro. Este trabajo propone la utilización de té verde como extracto a partir del cual sintetizar nanopartÃculas de plata. Aunque la bibliografÃa menciona ya la sÃntesis de nanopartÃculas de plata con té, trataremos de profundizar en las condiciones experimentales en las que tiene lugar. La caracterización de las nanopartÃculas se hará mediante su absorbancia en la región de lo visible. Esta propuesta didáctica está destinada a alumnos de 4º ESO o bachillerato. Con el fin de realizar un seguimiento del alumnado y una evaluación formativa de la actividad se propone que los alumnos hagan un blog explicativo de cada etapa de la propuesta didáctica

Mechanical circulatory support in severe primary graft dysfunction: peripheral cannulation but not earlier implantation improves survival in heart transplantation
[Abstract] Background. Primary graft dysfunction (PGD) still affects 2-28% of heart transplants (HT). Severe PGD requires mechanical circulatory support (MCS) and is the main cause of death early after HT. Earlier initiation has been suggested to improve prognosis but the best cannulation strategy is unknown.
Methods. Analysis of all HT in Spain between 2010 and 2020. Early (<3 hours after HT) vs late initiation (≥3 hours after HT) of MCS was compared. Special focus was placed on peripheral vs central cannulation strategy.
Results. 2376 HT were analyzed. 242 (10.2%) suffered severe PGD. 171 (70.7%) received early MCS and 71 (29.3%) late MCS. Baseline characteristics were similar. Patients with late MCS had higher inotropic scores and worse renal function at the moment of cannulation. Early MCS had longer cardiopulmonary bypass times and late MCS was associated with more peripheral vascular damage. No significant differences in survival were observed between early and late implant at 3 months (43.82% vs 48.26%; log-rank p=0.59) or at 1 year (39.29% vs 45.24%, log-rank p=0.49). Multivariate analysis did not show significant differences favoring early implant. Survival was higher in peripheral compared to central cannulation at 3 months (52.74% vs. 32.42%, log-rank p 0.001) and one year (48.56% vs. 28.19%, log-rank p 0.0007). In the multivariate analysis, peripheral cannulation remained a protective factor.
Conclusions. Earlier MCS initiation for PGD was not superior compared to a more conservative approach with deferred initiation. Peripheral compared to central cannulation showed superior 3-month and 1-year survival rates
