    Factors Responsible for Poor Visual Outcome Following Emergency Eye Surgery in a Tertiary Eye Centre

    BACKGROUND: Ocular emergencies can cause permanent vision loss if they are not recognized and treated promptly. This study was carried out to identify the factors responsible for poor visual outcome following emergency eye surgeries.METHODS: A cross-sectional study was carried out on all patients who had surgical emergency eye procedures. Clinic and theatre records of all eligible patients were retrieved. Demographic characteristics, duration of symptoms, laterality, presenting visual acuity, documentary pictures, classfication of eye injury, duration of days before surgery, surgical procedures performed and six weeks post-operative visual acuity were obtained from the records. Data were analyzed using Statistical Package for Social Sciences, version 25.RESULTS: One hundred and ninety-four patients had emergency eye surgical procedures constituting 16.3% of all ocular emergencies. There were 145(74.7%) males and 49 (25.3%) females with a male-to-female ratio of 3:1. The proportion of students, 48(67.6%), that presented within 24 hours of injury was the highest while the civil servants, 8(17.8%), had the lowest proportion. Corneo-scleral repair topped the list of surgeries and was the commonest, 106(54.6%), procedure carried out the same day of presentation. A 3rd of the patients, 28(34.1%), who presented within 24 hours had normal vision 6 weeks post-operatively while over 4/5th of the patients who presented after 24 hours were blind six weeks post-operatively.CONCLUSION: The prevalence of eye emergency surgical procedures was high with over 4/5th of these patients becoming blind six weeks post-operatively. Some of the factors responsible for poor visual outcome were delayed presentation, pre-operative visual acuity and delayed surgical intervention.

    Efficacy of intervention strategies for bioremediation of crude oil in polluted soil microcosm

    Crude oil, though not manmade but largely manipulated by man to provide different oil-based products has become a major source of environmental pollution. This menace on land do contribute to the retardation of vegetation growth and human health hazards, while in water it may be toxic to aquatic animals. The search for the solution to ameliorate the seemingly unending pollution and its side effects necessitated the evaluation on the effect of bioaugmentation, biostimulation and natural attenuation of crude oil pollution in soil microcosms. The bacterial species selected for this study (Bacillus thuringensis strain LG32 and Burkolderia pseudomallei strain A81) were preliminarily identified using the conventional biochemical tests and further identification was carried out using the API kit. The results of the study carried out over a period of five weeks indicated that there was a marked reduction in the available phosphorous and potassium in the bioaugmented and biostimulated soils compared with that of the control. The mean values for total viable counts (TVC) of population of hydrocarbon utilizing bacteria (HUB) was higher in the bioaugmented soil ranged (LG32=6.0-7.5log10cfu g-1 ; A81=5.5- 7.5log10cfug-1 ; LG32+A81=6.0-7.5log10cfug-1 ) compared with that of the control (6.0-6.2log10cfug-1 ). When bioaugmentation was combined with biostimulation, the soil had higher counts of HUB (6.0-9.0log10cfug-1 ) and HUF (3.5-6.5log10cfug-1) compared to bioaugmentation without stimulation (HUB: 6.0-7.5; HUF: 3.5-5.5). The GC result indicated that by day 35, 96.92% of the aliphatic and aromatic components have been degraded in the augmented soil, higher than the natural attenuation control

    Women’s Behavioral Patterns in Domestic Tasks in Western Nigeria: Hazards Forecasting with Neural Network Classifier

    Behavioral pattern is the characteristic ways a person acts and has been recognized as a cause of many home accidents (h-accd). This study reviewed the types and prevalence of injuries among women in domestic works and proposes a model using Artificial Neural Network (ANN) function to forecast the safety level of women in domestic duty. The study was conducted in some parts of Western Nigeria among 340 subjects (171 married and 169 unmarried) using questionnaire. SPSS was used for data analysis. The ANN function was developed in MATLAB 2015a using the subjects’ behavioral patterns and the model was used to predict safety in domestic duties (d-duties) among some women. ‘Cuts/laceration’ (40%) and ‘skin contact with hot substance’ (35.6%) were commonly reported. Carelessness (26.5%) and distraction (22.1%) were the main leading factors across the groups. Marital status and h-accd (Chi-square =4.323 and p= .038); ‘hours spent on domestic works’ and ‘the h-accd’ were both significant among other tested groups variables. With the developed ANN function, the results of the MSE was 0.33626 indicating that the function predicted the exact value. The result of the predicted h-accd (safety= -0.5445, hazards= 1.0228) in d-duties of the tested variables with the ANN function, showed a very low level of safety. The article concludes that the developed model is reliable and a recommended ergonomic tool useful in all homes, most especially where women perform most domestic works

    EXTENDING THE UNIFIED THEORY OF ACCEPTANCE AND USE OF TECHNOLOGY (UTAUT) MODEL: THE ROLE OF TECHNOLOGY CULTURATION

    The Unified Theory of Acceptance and Use of Technology (UTAUT) model has been widely used in explaining and assessing technology adoption in diverse contexts. However, it does not have Technology Culturation as one of the constructs. This variable constitutes a prevailing challenge in the developing world that could affect the adoption of technology in higher institutions of learning. The concept of Technology Culturation which is a critical factor for the adoption of e-learning tools and facilities in higher institutions of learning will be analytically examined in this study. The concept of Technology Culturation says that individuals who had prior exposure to relative technologies like cable satellites, television, video games, etc. are already tuned in their minds or acculturated to the use of technology; this concept assumes that this can affect an individual’s acceptance of other ICTs or other advanced technologies subsequently. This paper would therefore attempt to explore the extension of UTAUT by integrating Technology Culturation construct into the model that can be used in explaining or/and assessing the adoption of e-learning tools in higher institutions of learning in Nigeria. To this end, secondary sources of data such as Text Books, Journals, Magazines and the Internet will be used for this study. The conceptual framework should be of interest to both researchers and education administrators/stakeholders in developing nations in terms of planning and policy making

    Potentials of microorganisms associated with plantain peels in the Lagos metropolis for biodegradation and bioconversion.

    The role of microbes in the degradation of plantain derived-wastes and their potential to produce cellulolytic enzymes was assessed. Soil samples of decomposing waste piles were collected from two major plantain markets in the Lagos metropolis and analyzed for physicochemical properties, toxic heavy metal content and microbial populations. Findings revealed that the values of moisture content of the two soils varied between 7.27±0.04 and 8.06±0.19 %. M-12 site had the highest organic matter content of 6.89±0.14 %. A similar pattern was observed for nitrate, phosphate and chloride levels while some heavy metals were also detected in varying and high amounts. The highest viable bacterial counts was 58.0±2.9 x 104 cfu/g at MU and there were no fungi at the site whereas M-12 had a fungal count of 40.0±3.3 x 103 cfu/g. Out of the total of 34 isolates encountered, 8 isolates having maximum cellulase activities were selected for further studies by the primary screening technique. These test organisms were then evaluated by secondary screening for enzyme production. The test organisms were phenotypically and biochemically characterized and identified as Klebsiella pneumoniae spp pneumoniae (2 strains), Klebsiella pneumoniae spp ozaenae, Enterobacter aerogenes, Providencia alcalifaciens, Aspergillus flavus, Aspergillus fumigatus and Aspergillus niger respectively. Both the bacteria and moulds were found to be capable of utilizing lignin and cellulosic substrates for growth and for production of cellulolytic enzymes. It is suggested that such microorganisms could be useful in bioconversion of cellulosic substrates like plantain-derived wastes for biotechnological application

    Vitamin D status and serum vitamin D binding protein levels in Nigerian children with nephrotic syndrome

    Introduction: Nephrotic syndrome is a chronic relapsing condition associated with urinary loss of albumin and other proteins such as vitamin D binding protein (DBP). We determined vitamin D status and serum DBP levels in children with nephrotic syndrome and compared them to healthy controls. Methods: A cross-sectional study was performed over a six-month period in children less than 18 years of age. The children with nephrotic syndrome were categorised by disease status as either newly diagnosed, in remission, resistant to therapy, or in relapse. Vitamin D levels were regarded as sufficient if ≥75 nmol/L, insufficient if <75 nmol/L but ≥50 nmol/L, deficient if <50 nmol/L, and severely deficient if <25 nmol/L. Serum DBP was also measured. Results: Fifty-five children with nephrotic syndrome and 24 controls were included in the study. There was no significant difference between the median ages of the cases (72.0 months, interquartile range (IQR) 48.0–120.0 months) and the controls (84.0 months, IQR 39.0–129.0 months). Severe vitamin D deficiency, deficiency and insufficient levels were documented in 54.5%, 41.8% and 3.6% of cases, respectively, significantly lower than the controls (P = 0.003). Vitamin D levels were higher in children with nephrotic syndrome in remission than in those who were not (30.3 ± 15.2 nmol/L vs 19.6 ± 11.0 nmol/L, P = 0.004). In the groups who were in remission, newly diagnosed, relapsing, and resistant, the median vitamin D levels were 30.3 nmol/L, 20.1 nmol/L, 19.2 nmol/L and 9.4 nmol/L, respectively (P = 0.031). Conclusions: Hypovitaminosis D occurs frequently in Nigerian children with nephrotic syndrome as well as in apparently healthy controls. Routine supplementation of vitamin D should be considered in children with nephrotic syndrome irrespective of whether the disease is in remission or not, or whether it is steroid-sensitive or not

    Community engagement and compliance monitoring of COVID-19 safety protocols: innovative approach combining indigenous practice and GIS technology in Oyo State, Nigeria

    Background: One of the major challenges that has driven the spread of Coronavirus Disease 2019 (COVID-19) worldwide is the burden of enforcing the preventive measures required to contain the pandemic. Enforcement of COVID-19 precautionary behaviour should not be homogenous; every country needs to be creative to ensure that humane considerations guide all decisions during the extraordinary experience that COVID-19 pandemic portends. The model of self-policing is acceptable and maintained principally because the citizens of any communities operate, recognize, and accept them as preferred alternatives to the official models of policing for enforcement. Hence the approach presented in this paper, which deployed existing indigenous alternative systems in ensuring compliance with COVID-19 precautionary behaviour. This article therefore documents the unique approach deployed for the containment of COVID-19 in Oyo State, Nigeria. Objective: This intervention was designed to explore established indigenous alternative systems and models of control, justice, law, security, and enforcement in Nigeria. Additionally, geographic information system (GIS) technology and investigative journalism was used to monitor and evaluate the effectiveness of the intervention. Method: The method employed was community conversation; a method of increasing inclusive, community-based engagement harnessing the expertise and motivation of key stakeholders. The community conversations were convened after the pattern of a traditional Town-hall meeting. Community conversations were organized as a qualitative framework focusing on deploying the indigenous practice of self-policing associated with Nigerias trade unions and aims to inform COVID-19 preventive behaviour at the community level. Geographical information system technology was used to develop COVID-19 Containment Compliance Citizens Reporter App. The App was developed using ESRI ArcGIS online platform to crowd source public feedback on compliance or contravention of COVID-19 protocols. Social media platforms were also deployed for monitoring and evaluation of the intervention post townhall meeting. Results: The establishment of a State-wide Containment response network provided the required inroad for advocacy and deployment of state-wide community conversation framework in the different communities comprising diverse ethnic groups, religious leaders, market leaders, National Union of Road Transport Workers (NURTW), and so on. Testimonials from the various communities showed that the people have embraced the self-policing strategy and the network system was effective with good outcomes in terms of response to decontamination, containment, and advocacy. The COVID-19 Containment Compliance Citizens Reporter App, investigative reporting by mass media were highly effective tools for monitoring and evaluation of the outcome of the intervention as well as possible evidence for melting out incentive and disincentive measures as necessary. This approach is a template, which could be adapted and replicated in other parts of Nigeria and other African societies with similar structures, demographics, and indigenous practices

    APOE E4 is associated with impaired self-declared cognition but not disease risk or age of onset in Nigerians with Parkinson's disease

    The relationship between APOE polymorphisms and Parkinson's disease (PD) in black Africans has not been previously investigated. We evaluated the association between APOE polymorphic variability and self-declared cognition in 1100 Nigerians with PD and 1097 age-matched healthy controls. Cognition in PD was assessed using the single item cognition question (item 1.1) of the MDS-UPDRS. APOE genotype and allele frequencies did not differ between PD and controls (p > 0.05). No allelic or genotypic association was observed between APOE and age at onset of PD. In PD, APOE ε4/ε4 conferred a two-fold risk of cognitive impairment compared to one or no ε4 (HR: 2.09 (95% CI: 1.13-3.89; p = 0.02)), while APOE ε2 was associated with modest protection against cognitive impairment (HR: 0.41 (95% CI 0.19-0.99, p = 0.02)). Of 773 PD with motor phenotype and APOE characterized, tremor-dominant (TD) phenotype predominated significantly in ε2 carriers (87/135, 64.4%) compared to 22.2% in persons with postural instability/gait difficulty (PIGD) (30/135) and 13.3% in indeterminate (ID) (18/135, 13.3%) (p = 0.037). Although the frequency of the TD phenotype was highest in homozygous ε2 carriers (85.7%), the distribution of motor phenotypes across the six genotypes did not differ significantly (p = 0.18). Altogether, our findings support previous studies in other ethnicities, implying a role for APOE ε4 and ε2 as risk and protective factors, respectively, for cognitive impairment in PD

    Changes in preterm birth and stillbirth during COVID-19 lockdowns in 26 countries.

    Preterm birth (PTB) is the leading cause of infant mortality worldwide. Changes in PTB rates, ranging from -90% to +30%, were reported in many countries following early COVID-19 pandemic response measures ('lockdowns'). It is unclear whether this variation reflects real differences in lockdown impacts, or perhaps differences in stillbirth rates and/or study designs. Here we present interrupted time series and meta-analyses using harmonized data from 52 million births in 26 countries, 18 of which had representative population-based data, with overall PTB rates ranging from 6% to 12% and stillbirth ranging from 2.5 to 10.5 per 1,000 births. We show small reductions in PTB in the first (odds ratio 0.96, 95% confidence interval 0.95-0.98, P value <0.0001), second (0.96, 0.92-0.99, 0.03) and third (0.97, 0.94-1.00, 0.09) months of lockdown, but not in the fourth month of lockdown (0.99, 0.96-1.01, 0.34), although there were some between-country differences after the first month. For high-income countries in this study, we did not observe an association between lockdown and stillbirths in the second (1.00, 0.88-1.14, 0.98), third (0.99, 0.88-1.12, 0.89) and fourth (1.01, 0.87-1.18, 0.86) months of lockdown, although we have imprecise estimates due to stillbirths being a relatively rare event. We did, however, find evidence of increased risk of stillbirth in the first month of lockdown in high-income countries (1.14, 1.02-1.29, 0.02) and, in Brazil, we found evidence for an association between lockdown and stillbirth in the second (1.09, 1.03-1.15, 0.002), third (1.10, 1.03-1.17, 0.003) and fourth (1.12, 1.05-1.19, <0.001) months of lockdown. With an estimated 14.8 million PTB annually worldwide, the modest reductions observed during early pandemic lockdowns translate into large numbers of PTB averted globally and warrant further research into causal pathways
