    Homicidal violence during foreign military missions prevention and legal issues

    Objectives. The study involved Nigerian soldiers engaged in peacekeeping missions in Liberia and Yugoslavia. Using case illustrations, the study sought to describe patterns of homicidal violence among soldiers from the same country or soldiers from allied forces, and to suggest possible reasons for the attacks.Design and setting. Nigeria was actively involved in peacekeeping missions in Liberia between 1990 and 1996. During this period, intentional homicidal attacks occurred among the Nigerian military personnel. Post-homicidal interviews conducted among the perpetrators were combined with evidence obtained at military courts to produce the case studies.Subjects;. Six Nigerian military personnel who attacked other Nigerians or soldiers from allied forces, with homicidal intent Results, Possible predisposing and precipitating factors for these attacks were highlighted. The possibility of recognising these factors before embarking on overseas missions was discussed, so that preventive measures could be instituted as far as possible. Finally, medico-legal implications of homicide in the military were discussed. Conclusions. A certain degree of pre-combat selection is essential to exclude soldiers with definite severe psychopathology. A clearly defined length of duty in the mission areas and adequate communication with home could reduce maladjustment Health personnel deployed to mission areas should be very conversant with mental health issues so that early recognition of psychological maladjustment is possible

    Transcriptomic and functional analysis of the Anopheles gambiae salivary gland in relation to blood feeding

    <p>Abstract</p> <p>Background</p> <p>The <it>Anopheles gambiae </it>salivary glands play a major role in malaria transmission and express a variety of bioactive components that facilitate blood-feeding by preventing platelet aggregation, blood clotting, vasodilatation, and inflammatory and other reactions at the probing site on the vertebrate host.</p> <p>Results</p> <p>We have performed a global transcriptome analysis of the <it>A. gambiae </it>salivary gland response to blood-feeding, to identify candidate genes that are involved in hematophagy. A total of 4,978 genes were found to be transcribed in this tissue. A comparison of salivary gland transcriptomes prior to and after blood-feeding identified 52 and 41 transcripts that were significantly up-regulated and down-regulated, respectively. Ten genes were further selected to assess their role in the blood-feeding process using RNAi-mediated gene silencing methodology. Depletion of the salivary gland genes encoding <it>D7L2</it>, <it>anophelin</it>, <it>peroxidase</it>, the <it>SG2 precursor</it>, and a <it>5'nucleotidase </it>gene significantly increased probing time of <it>A. gambiae </it>mosquitoes and thereby their capacity to blood-feed.</p> <p>Conclusions</p> <p>The salivary gland transcriptome comprises approximately 38% of the total mosquito transcriptome and a small proportion of it is dynamically changing already at two hours in response to blood feeding. A better understanding of the salivary gland transcriptome and its function can contribute to the development of pathogen transmission control strategies and the identification of medically relevant bioactive compounds.</p

    Malaria Parasite Invasion of the Mosquito Salivary Gland Requires Interaction between the Plasmodium TRAP and the Anopheles Saglin Proteins

    SM1 is a twelve-amino-acid peptide that binds tightly to the Anopheles salivary gland and inhibits its invasion by Plasmodium sporozoites. By use of UV-crosslinking experiments between the peptide and its salivary gland target protein, we have identified the Anopheles salivary protein, saglin, as the receptor for SM1. Furthermore, by use of an anti-SM1 antibody, we have determined that the peptide is a mimotope of the Plasmodium sporozoite Thrombospondin Related Anonymous Protein (TRAP). TRAP binds to saglin with high specificity. Point mutations in TRAP's binding domain A abrogate binding, and binding is competed for by the SM1 peptide. Importantly, in vivo down-regulation of saglin expression results in strong inhibition of salivary gland invasion. Together, the results suggest that saglin/TRAP interaction is crucial for salivary gland invasion by Plasmodium sporozoites

    Post-traumatic stress disoder, survivor guilt and substance use - a study of hospitalised Nigerian army veterans

    Objectives. To investigate the prevalence of post-traumatic stress disorder (PTSD) and survivor guilt in a sample of hospitalised soldiers evacuated from the Liberian and Sierra-Leonean wars in which Nigerians were involved as peace keepers. The relationships between PTSD, survivor guilt and substance use were also investigated. Design. A socio-demographic data questionnaire, the PTSD checklist and a validated World Health Organization substance use survey instrument were used to obtain data from the subjects. Setting. The study took place at the 68 Nigerian Army Reference Hospital, Lagos, Nigeria, which was the base hospital for all casualties from the Liberian and Sierra- Leonean operations. Subjects. All hospitalised patients from the military operations during a 4-year period (1990 - 1994) who were physically capable of being assessed were included in the study. Results. The prevalence rate for PTSD was found to be 22% and survivor guilt was found in 38% of the responders. PTSD was significantly associated with long duration of stay in the mission area, current alcohol use, lifetime use of an alcohol/gunpowder mixture, and lifetime cannabis use. Survivor guilt was significantly associated with avoidance of trauma-related stimuli but not duration of combat exposure. Conclusions. Although the sample studied was specific, PTSD might be quite common and probably undetected among Nigerian military personnel engaged in battle in Liberia and Sierra-Leone. Detection of such persons through deliberate screening in military community studies should help to alleviate the symptoms since good intervention methods are now available. Primary prevention efforts with regard to alcohol and cannabis use should help to reduce the incidence of PTSD

    Alcohol Use and HIV Risk taking Behaviours in a Military Setting.

    Background Military personnel are well known to be at high risk for human immunodeficiency virus infection, and being mostly youthful, commonly use alcohol. Aim The aim of the study was to compare HIV positive and negative subjects with respect to alcohol use and abuse, condom use and multiple sex partnership. Method Using a survey questionnaire we enquired into the use and abuse of alcohol, knowledge about acquired immune deficiency syndrome, use of condom and number of heterosexual relationships in a group of HIV positive patients. Their responses were compared with those from a control group of HIV negative subjects. Results Married persons were more likely to be HIV positive. Use and abuse of alcohol appear to be risk factors for being HIV positive. The HIV positive and negative groups were well knowledgeable about AIDS, but they differed in the use of condom in terms of previous use or use in the previous three months. The HIV positive one were also more likely to use the condom inconsistently with prostitutes and casual partners. Conclusion Health education campaigns on HIV/AIDS need to include alcohol use as a major risk factor in addition to the other well known risk factors including multiple sexual relationships and non-use of condom.
