    Effect of intermittent liver ischemia on outcome in patients with hepatocellular carcinoma on liver cirrhosis

    The influence on postoperative liver function of intermittent normothermic hepatic ischemia in cirrhotic patients was studied retrospectively. The mean total ischemia time was 88 (range30 - 140) minutes in the hemi-hepatic occlusion group, and68 (range 10 - 187) minutes in the total occlusion group. There were no operative deaths due to hepatic failure. Postoperative liver function improved within 1 week of the opera-tion. There was no significant difference in the incidence of postoperative complications between the groups. Thus normothermic hepatic ischemia is tolerated for up to 180minutes in the cirrhotic liver when an intermittent technique (15 minutes clamped and5 minutes unclamped) is used

    A search for new isotopes using in-flight fission of a 345 MeV/nucleon 238U beam has been carried out at the RI Beam Factory at the RIKEN Nishina Center. Fission fragments were analyzed and identified by using the superconducting in-flight separator BigRIPS. We observed 45 new neutron-rich isotopes: 71Mn, 73,74Fe, 76Co, 79Ni, 81,82Cu, 84,85Zn, 87Ga, 90Ge, 95Se, 98Br, 101Kr, 103Rb, 106,107Sr, 108,109Y, 111,112Zr, 114,115Nb, 115,116,117Mo, 119,120Tc, 121,122,123,124Ru, 123,124,125,126Rh, 127,128Pd, 133Cd, 138Sn, 140Sb, 143Te, 145I, 148Xe, and 152Ba

    Low-Dose Intravenous Alteplase in Wake-Up Stroke

    Background and Purpose—We assessed whether lower-dose alteplase at 0.6 mg/kg is efficacious and safe for acute fluid-attenuated inversion recovery-negative stroke with unknown time of onset. Methods—This was an investigator-initiated, multicenter, randomized, open-label, blinded-end point trial. Patients met the standard indication criteria for intravenous thrombolysis other than a time last-known-well >4.5 hours (eg, wake-up stroke). Patients were randomly assigned (1:1) to receive alteplase at 0.6 mg/kg or standard medical treatment if magnetic resonance imaging showed acute ischemic lesion on diffusion-weighted imaging and no marked corresponding hyperintensity on fluid-attenuated inversion recovery. The primary outcome was a favorable outcome (90-day modified Rankin Scale score of 0–1). Results—Following the early stop and positive results of the WAKE-UP trial (Efficacy and Safety of MRI-Based Thrombolysis in Wake-Up Stroke), this trial was prematurely terminated with 131 of the anticipated 300 patients (55 women; mean age, 74.4±12.2 years). Favorable outcome was comparable between the alteplase group (32/68, 47.1%) and the control group (28/58, 48.3%; relative risk [RR], 0.97 [95% CI, 0.68–1.41]; P=0.892). Symptomatic intracranial hemorrhage within 22 to 36 hours occurred in 1/71 and 0/60 (RR, infinity [95% CI, 0.06 to infinity]; P>0.999), respectively. Death at 90 days occurred in 2/71 and 2/60 (RR, 0.85 [95% CI, 0.06–12.58]; P>0.999), respectively. Conclusions—No difference in favorable outcome was seen between alteplase and control groups among patients with ischemic stroke with unknown time of onset. The safety of alteplase at 0.6 mg/kg was comparable to that of standard treatment. Early study termination precludes any definitive conclusions

    Efficacy of ME1036 against meticillin-resistant Staphylococcus aureus and vancomycin-insensitive S. aureus in a model of haematogenous pulmonary infection.

    ME1036, a novel parenteral carbapenem, was developed for the treatment of meticillin-resistant Staphylococcus aureus (MRSA) and vancomycin-intermediate S. aureus (VISA). A model of haematogenous pulmonary infection was induced in mice by tail vein injection of MRSA strain NUMR101 or VISA Mu50 enmeshed in agar beads. After 24h of infection, mice were treated twice daily for 7 days with 200mg/kg/day vancomycin (VCM) or ME1036. Mice infected with VISA were also pre-treated with cyclophosphamide to induce an immunocompromised state. The number of viable bacteria in the lungs was counted 12h after the final drug treatment. VCM decreased the number of viable MRSA in the lungs in comparison with the control, although the difference was not significant (mean+/-standard error of the mean log(10) colony-forming units (CFU)/lung=6.876+/-0.54 vs. 8.25+/-0.41, respectively). In contrast, treatment with ME1036 resulted in a significant decrease in the number of viable MRSA (log(10)CFU/lung=2.69+/-0.44 (n=6); P3 log(10) reduction versus control against both MRSA strains (>5 log for the VCM-susceptible strain and 3.4 log for the VISA), whereas VCM produced <1.3 log for both strains

    Cross sections for nuclide production in proton- and deuteron-induced reactions on 93

    Isotopic production cross sections were measured for proton- and deuteron-induced reactions on 93Nb by means of the inverse kinematics method at RIKEN Radioactive Isotope Beam Factory. The measured production cross sections of residual nuclei in the reaction 93Nb + p at 113 MeV/u were compared with previous data measured by the conventional activation method in the proton energy range between 46 and 249 MeV. The present inverse kinematics data of four reaction products (90Mo, 90Nb, 88Y, and 86Y) were in good agreement with the data of activation measurement. Also, the model calculations with PHITS describing the intra-nuclear cascade and evaporation processes generally well reproduced the measured isotopic production cross sections

    Spallation reaction study for fission products in nuclear waste: Cross section measurements for 137

    Spallation reactions for the long-lived fission products 137Cs, 90Sr and 107Pd have been studied for the purpose of nuclear waste transmutation. The cross sections on the proton- and deuteron-induced spallation were obtained in inverse kinematics at the RIKEN Radioactive Isotope Beam Factory. Both the target and energy dependences of cross sections have been investigated systematically. and the cross-section differences between the proton and deuteron are found to be larger for lighter fragments. The experimental data are compared with the SPACS semi-empirical parameterization and the PHITS calculations including both the intra-nuclear cascade and evaporation processes

    Assessing the effectiveness of a water safety program for junior high school students in Japan1

    AbstractWater safety programs to protect children from water-related accidents and increase their awareness of the risk factor crucial to maintaining safety. This study examined the behavior of 48 junior high school students after an intervention program on water safety in a river using the Rohrmann’s Risk Communication Model and the Analysis, Design, Development, and Evaluation (ADDIE) model. A self-administered questionnaire was designed to collect responses about the children’s perception of danger at three-time points: pre-post-post (5 months) after the program’s implementation. We hypothesized that water safety education can increase children’s awareness of appropriate behavior around water and raise their awareness of danger. The evaluation indicators consisted of an increase in the awareness of the water area risk perception, general risk acceptance, and risk-taking behavior. The results suggest that practical water safety program may have a positive effect on the students’ awareness of water safety and help prevent water accidents. This study is unique in that it was conducted in a natural body of water
