The marginal values of lifesavers and lifeguards to beach users in Australia and the United States
We estimate the marginal benefits of increasing lifesavers and lifeguards for beach users in Australia and the United States. Visits, income, education, age, distance from a patrol, and willingness to swim on an unpatrolled beach explain willingness to pay but rivalry does not; snob and bandwagon effects prevail. By comparing benefits with costs, the levels of lifeguards and lifesavers in Australia were found to be underprovided, consistent with shared good theory. Increasing services provides greater net benefits to users but replacing volunteer lifesavers with paid lifeguards may not because volunteering brings broader social benefits
The malaria testing and treatment landscape in Kenya: results from a nationally representative survey among the public and private sector in 2016
Abstract Background Since 2004, Kenya’s national malaria treatment guidelines have stipulated artemisinin-based combination therapy (ACT) as first-line treatment for uncomplicated malaria, and since 2014, confirmatory diagnosis of malaria in all cases before treatment has been recommended. A number of strategies to support national guidelines have been implemented in the public and private sectors in recent years. A nationally-representative malaria outlet survey, implemented across four epidemiological zones, was conducted between June and August 2016 to provide practical evidence to inform strategies and policies in Kenya towards achieving national malaria control goals. Results A total of 17,852 outlets were screened and 2271 outlets were eligible and interviewed. 78.3% of all screened public health facilities stocked both malaria diagnostic testing and quality-assured ACT (QAACT). Sulfadoxine–pyrimethamine (SP) for intermittent preventive treatment in pregnancy was available in 70% of public health facilities in endemic areas where it is recommended for treatment. SP was rarely found in the public sector outside of the endemic areas (< 0.5%). The anti-malaria stocking private sector had lower levels of QAACT (46.7%) and malaria blood testing (20.8%) availability but accounted for majority of anti-malarial distribution (70.6% of the national market share). More than 40% of anti-malarials were distributed by unregistered pharmacies (37.3%) and general retailers (7.1%). QAACT accounted for 58.2% of the total anti-malarial market share, while market share for non-QAACT was 15.8% and for SP, 24.8%. In endemic areas, 74.9% of anti-malarials distributed were QAACT. Elsewhere, QAACT market share was 49.4% in the endemic-prone areas, 33.2% in seasonal-transmission areas and 37.9% in low-risk areas. Conclusion Although public sector availability of QAACT and malaria diagnosis is relatively high, there is a gap in availability of both testing and treatment that must be addressed. The private sector in Kenya, where the majority of anti-malarials are distributed, is also critical for achieving universal coverage with appropriate malaria case management. There is need for a renewed commitment and effective strategies to ensure access to affordable QAACT and confirmatory testing in the private sector, and should consider how to address malaria case management among informal providers responsible for a substantial proportion of the anti-malarial market share
The illusion of choice: an exploratory study looking at the top 10 food companies in Australia and their brand connections
© 2018 The Authors. Objective: To identify the brands owned by each of the 10 top grossing food companies operating in Australia and visually represent them on an infographic. Methods: Desktop research was conducted to determine Australia's 10 largest food companies based on revenue. Brand ownership for each of the companies was traced through financial records and company publications. This information was then visually documented in the form of an infographic ‘food web’ to clearly illustrate company and brand ownership. Results: Fonterra, Coca-Cola Amatil, Lion, Murray Goulburn, George Weston Foods, Wilmar, Nestle, Mondelez, Parmalat and Asahi were determined as the top 10 food companies operating in Australia. The food web illustrated that brand ownership ranged from 75 (Nestle) to four (Fonterra) brands per company. Conclusions: The food web illustrates the dominance of each of these major companies within Australia and shows how their diverse brand ownership limits consumer choice. Implications for public health: This study expands on current knowledge and further defines the breadth of market influence that the top 10 food companies have within the Australian food context, and how they use their brand power to create an illusion of choice for consumers. The food web will assist in promoting transparency of brand ownership in the Australian food market, therefore allowing consumers to make an informed decision about the food they purchase, and will allow community and other organisations to make an informed decision about which companies they form partnerships with

This paper is a historical review, documenting the evolution of Home Economics as a subject in Irish primary and post-primary education from the 1800s to the twenty-first century. In the 1800s and early twentieth-century domestic subjects, including cookery, was widely taught to females in both primary and post-primary schools. The philosophical underpinning of the subject was to enhance the quality of life for families. The subject remained a popular choice for young women up until the establishment of the Irish Free State which, thereafter, witnessed many changes in the teaching of cookery and domestic science in primary and post-primary schools. The core ideology of the subject has remained relevant and it aims to provide students with knowledge, practical skills, understanding and attitudes for everyday life as individuals and as family members. This reflects the richness of the subject from the past and the relevance of the subject in addressing issues of a twenty-first century society
The nuanced nature of work quality : evidence from rural Newfoundland and Ireland
This article explores the relationship between job and work quality and argues that while it is important to examine job quality, to understand workers’ experiences fully, the focus should be on the broader concept of work quality, which places the job against its wider socio-economic context. Based on the experiences of 88 rural workers gathered via interviews in Newfoundland and Ireland, it appears that the same or similar jobs can be regarded very differently depending upon the context in which they are embedded, as people at different locations and/or stages of life have an individual set of aspirations, expectations and life experiences. The study found that the factors that affect work quality are moulded by broader aspects of life – family, friends, community, lifestyle and past experiences – that shape an individual

Effective suckling in relation to naked maternal-infant body contact in the first hour of life: an observation study
Background
Best practice guidelines to promote breastfeeding suggest that (i) mothers hold their babies in naked body contact immediately after birth, (ii) babies remain undisturbed for at least one hour and (iii) breastfeeding assistance be offered during this period. Few studies have closely observed the implementation of these guidelines in practice. We sought to evaluate these practices on suckling achievement within the first hour after birth.
Methods
Observations of seventy-eight mother-baby dyads recorded newborn feeding behaviours, the help received by mothers and birthing room practices each minute, for sixty minutes.
Results
Duration of naked body contact between mothers and their newborn babies varied widely from 1 to 60 minutes, as did commencement of suckling (range = 10 to 60 minutes). Naked maternal-infant body contact immediately after birth, uninterrupted for at least thirty minutes did not predict effective suckling within the first hour of birth. Newborns were four times more likely to sustain deep rhythmical suckling when their chin made contact with their mother’s breast as they approached the nipple (OR 3.8; CI 1.03 - 14) and if their mothers had given birth previously (OR 6.7; CI 1.35 - 33). Infants who had any naso-oropharyngeal suctioning administered at birth were six times less likely to suckle effectively (OR .176; CI .04 - .9).
Conclusion
Effective suckling within the first hour of life was associated with a collection of practices including infants positioned so their chin can instinctively nudge the underside of their mother’s breast as they approach to grasp the nipple and attach to suckle. The best type of assistance provided in the birthing room that enables newborns to sustain an effective latch was paying attention to newborn feeding behaviours and not administering naso-oropharyngeal suction routinely
