Measurement of the U fission cross section and angular distributions of fragments from fission of U and U in the neutron energy range of 0.3-500 MeV
The U fission cross section and the angular distributions of
fragments from fission of U and U were measured for incident
neutron energies from 0.3 MeV to 500 MeV on the time-of-flight spectrometer of
the neutron complex GNEIS at the NRC "Kurchatov Institute" -- PNPI. Fission
fragments were registered using position-sensitive low-pressure multiwire
counters. In the neutron energy range above 20 MeV, the angular distributions
of U fission fragments were measured for the first time. The fission
cross section of U was measured relative to the fission cross
section of U, which is an accepted international standard. The
obtained data are compared with the results of other experimental works.
Theoretical calculations of the fission cross section and the anisotropy of
angular distribution of fission fragments for the U reaction

Роль молекулярно-генетических исследований в диагностике и профилактике развития профессиональных заболеваний органов дыхания
Morbidity of occupational respiratory diseases depends on workplace environmental and individual genotype as well. The aim of our study was to investigate molecular biomarkers of bronchial asthma (BA) and chronic obstructive pulmonary disease (COPD) in workers exposed to toxic aerosols. The second aim was to develop preventive measures for these diseases. Methods. This was a randomized prospective cohort study that involved workers of food industry. Results. The study involved 159 food industry workers. Lung function abnormalities (29.3%) and hyperresponsiveness to workrelated factors (20.6%) were found in 126 of 159 workers who were at risk of occupational respiratory diseases and were exposed to biological active aerosols. BA and COPD were diagnosed in 17.4% and 11.9% of workers, respectively. Hyposecretory variants (PiMS, PiMZ) of alfa-1-protease inhibitor (API) gene were found in 5.5% of workers. Proportions of workers with deficit of GSTM1 and GSTT1 enzymes were 65% and 60%, respectively. Risk alleles (ID, DD) of the angiotensin converting enzyme (ACE) gene that indicate the susceptibility to cardiovascular diseases and are typical for subjects exposed to dust particles and other hazardous agents were found in 20% of workers. Conclusion. Our results have demonstrated the role of molecular biomarkers of individual susceptibility to occupational factors. Molecular biomarkers could define the risk of occupational respiratory diseases, predict their course and help to choose therapeutic and preventive measures. Identifying the susceptible workers could underlie personalized approach to prevention of respiratory diseases.Развитие профессиональных респираторных заболеваний, в частности бронхиальной астмы (БА) и хронической обструктивной болезни легких (ХОБЛ), зависит не только от условий труда, свойств и дозы экспонированных аэрозолей в зоне дыхания, но и от индивидуальной восприимчивости организма к воздействию чужеродных агентов и способности им противостоять, т. е. от генотипа. Оценкой индивидуального риска развития легочного заболевания профессионального генеза и системы защиты от воздействия производственных аэрозолей служат исследования гипосекреторных мутаций – PiMZ гена α1-ингибитора протеаз (ИП), а также гена детоксикации системы глутатион-трансферазы – GST. Цель. Изучение молекулярно-генетических особенностей формирования БА и ХОБЛ у лиц, работающих в условиях воздействия токсических аэрозолей и определение возможных путей профилактики заболеваний. Материалы и методы. При проведении когортного рандомизированного проспективного исследования с участием работников (n = 159) пищевого производства установлено, что у лиц с профессиональным риском развития заболеваний органов дыхания, имевших контакт с биологически активными аэрозолями (n = 126), выявлены нарушения функции внешнего дыхания (29,3 %) с гиперчувствительностью к производственным факторам (20,6 %). Диагнозы БА и ХОБЛ установлены у 17,4 и 11,9 % работников соответственно. У 5,5 % выявлены гипосекреторные варианты гена α1-ИП (PiMS, PiMZ). Число лиц с отсутствием ферментов GSTM1 и GSTT1 составило 65 и 60 % соответственно. У 20 % работников с респираторными симптомами найдены аллели риска гена ангиотензинпревращающего фермента (ID, DD), свидетельствующие о восприимчивости к сердечно-сосудистой патологии, что характерно для лиц, экспонированных к пылевым частицам и чужеродным агентам. Результаты. Полученные данные не только демонстрируют значимость проведения молекулярно-генетических исследований с определением биомаркеров индивидуальной восприимчивости организма к профессиональным факторам, но и позволяют определить степень риска развития респираторных заболеваний, прогнозировать тяжесть их течения и обосновать лечебно профилактические мероприятия. Заключение. При изучении генетической предрасположенности к вредным факторам, присутствующим на рабочем месте, появляется возможность идентификации лиц, особо чувствительных к экспозиции вредных веществ, и установления предельно допустимых уровней содержания поллютантов на рабочем месте, что, в свою очередь, позволит осуществить персонифицированный подход к профилактике легочных заболеваний
ОПЫТ ПРИМЕНЕНИЯ ИНГИБИТОРА ИНТЕРЛЕЙКИНА 1 КАНАКИНУМАБА У ПАЦИЕНТОВ С АУТОВОСПАЛИТЕЛЬНЫМИ ЗАБОЛЕВАНИЯМИ
Interleukin 1 (IL 1) is the main mediator for many auto inflammatory diseases (AID). Cryopyrin-associated periodic syndromes (CAPS) became the first diseases, for which the efficacy of IL 1 inhibitors was shown with a high degree of evidence. Canacinumb (totally human monoclonal antibodies to IL 1 is registered in Russia since 2011 for treating CAPS. At the moment research is being conducted to evaluate the efficacy and tolerance to IL 1 inhibitors in patients with other AIDs. The accumulated experience and scarce randomizedcontrolled studies demonstrate a successful usage of IL 1 inhibitors in colchicine-resistant patients with family Mediterranean fever with the cupping of inflammation attacks, reducing the acute phase activity and also in patients with other monogenic (TRAPS, HIDS and others) and multifactorial pathologies (systemic juvenile arthritis, Stilk disease in adults, gout etc.). Using the IL 1 inhibitors, especially canacinumab, in patients with different AIDs has shown good tolerance and a high efficacy in all patients with no correlation to the age, according to Russian and world research. Thus, canacinumab, thanks to its therapeutic abilities, has broad perspectives in terms of lightening the disease course, improving survival, life quality and the overall prognosis. Основным медиатором воспаления для многих аутовоспалительных заболеваний (АВЗ) является интерлейкин 1β (IL 1β). Криопиринассоциированные периодические синдромы (CAPS) стали первыми заболеваниями, при которых с высокой степенью доказательности была показана эффективность ингибиторов IL 1. Канакинумаб (полностью человеческие моноклональные антитела к IL 1β) с 2011 г. зарегистрирован в Российской Федерации для лечения CAPS. В настоящее время проводятся исследования по оценке эффективности и переносимости ингибиторов IL 1 у пациентов с другими АВЗ. Накопленный практический опыт и немногочисленные рандомизированные контролируемые исследования демонстрируют успешное применение ингибиторов IL 1 у колхицинрезистентных пациентов с семейной средиземноморской лихорадкой с купированием воспалительных атак и снижением острофазовой активности, а также у больных с другими моногенными (TRAPS, HIDS и др.) и мультифакториальными патологиями (системный ювенильный артрит, болезнь Стилла у взрослых, подагра и др.). Применение ингибиторов IL 1, в частности канакинумаба, у пациентов с различными АВЗ, по данным российских и зарубежных исследований, показало хорошую переносимость и высокую эффективность препарата у всех больных независимо от возраста. Таким образом, канакинумаб, благодаря своим терапевтическим возможностям, имеет широкие перспективы в плане облегчения течения болезни, улучшения выживаемости, качества жизни и общего прогноза

CHRONIC HEPATITIS B VIRUS INFECTION IN PREGNANCY: STRATEGIES OF ANTIVIRAL THERAPY
Treatment of chronic hepatitis B during pregnancy is an extremely complicated issue. Despite implementation of immune prophylaxis, a significant proportion of babies born by mothers with high viral load are infected by hepatitis B virus. Cumulative data suggest that antiviral therapy in the 3 trimester of pregnancy is an effective intervention in the event of unsuccessful immune prord phylaxis. To minimize fetal effects of nucleoside and nucleotide analogues, antiviral therapy during pregnancy should be administered to mothers with high risk of disease progression and/or uncontrolled hepatitis B virus infection. The safety data obtained indicate that telbivudine and tenofovir can be used during pregnancy. Nevertheless, antiviral therapy requires a thorough assessment of the risk to benefit ratio
