    Serendipitous stellar occultation search is so far the only way to detect the existence of very small, very dim, remote objects in the solar system. To date, however, there are only very few reported detections for trans-Neptunian objects (TNOs) in optical bands. In the X-ray band, with the RXTE/PCA data of Sco X-1 taken from June 2007 to October 2009, we found one possible X-ray occultation event. We discuss the veracity and properties of this event, and suggest upper limits to the size distribution of TNOs at hectometer size and of Main-Belt Asteroids (MBAs) at decameter size.Comment: 8 pages, 5 figures, to appear in MNRAS (accepted on Sep. 10, 2010

    Routine chest roentgenography on admission to intensive care unit after heart operations: Is it of any value?

    AbstractThe need for routine immediate postoperative chest roentgenography after heart operations has recently been questioned. In this study we investigated the impact of routine postoperative chest roentgenography on treatment instituted in the cardiovascular intensive care unit immediately after heart operations done via median sternotomy. A total of 404 random patients admitted to the cardiovascular intensive care unit underwent clinical (positioning of endotracheal tube, nasogastric tube, and pulmonary artery catheter) and laboratory (oxygenation) assessment by a cardiovascular intensive care unit physician according to a strict protocol. After clinical assessment, chest roentgenography was done for all admitted patients and the findings reviewed by the same physician. Pathologic conditions noted were recorded on the study form together with any required treatment. Eighteen patients (4.5%) out of 404 required intervention because of abnormalities detected by the chest x-ray film but not predicted by the initial physical and laboratory assessment. None of the pathologic conditions detected was life threatening. We conclude that chest roentgenography done on admission to the cardiovascular intensive care unit should be done only if clinical and laboratory assessment indicate the possibility of underlying pathologic conditions that can only be confirmed or diagnosed by chest roentgenography.     Noise Performance Comparison of 1.5 um Correlated Photon Pair Generation in Different Fibers

    In this paper, the noise performances of 1.5 um correlated photon pair generations based on spontaneous four wave-mixing in three types of fibers, i.e., dispersion shifted fiber, highly nonlinear fiber, and highly nonlinear microstructure fiber are investigated experimentally. Result of the comparison shows that highly nonlinear microstructure fiber has the lowest Raman noise photon generation rate among the three types of fibers while correlated photon pair generation rate is the same. Theoretical analysis shows that the noise performance is determined by the nonlinear index and Raman response of the material in fiber core. The Raman response raises with increasing doping level, however, the nonlinear index is almost unchanged with it. As a result, highly nonlinear microstructure fiber with pure silica core has the best noise performance and has great potential in practical sources of correlated photon pairs and heralded single photons.Comment: 10 pages, 6 figure

    Oral drug delivery strategies for development of poorly water soluble drugs in paediatric patient population

    Selecting the appropriate formulation and solubility-enabling technology for poorly water soluble drugs is an essential element in the development of formulations for paediatric patients. Different methodologies and structured strategies are available to select a suitable approach and guide formulation scientists for development of adult formulations. However, there is paucity of available literature for selection of technology and overcoming the challenges in paediatric formulation development. The need for flexible dosing, and the limited knowledge of the safety of many formulation excipients in paediatric subjects, impose significant constraints and in some instances require adaptation of the approaches taken to formulating these drugs for the adult population. Selection of the best drug delivery system for paediatrics requires an efficient, systematic approach that considers a drug's physical and chemical properties and the targeted patient population's requirements. This review is a step towards development of a strategy for the design of solubility enhancing paediatric formulations of highly insoluble drugs. The aim of this review is to provide an overview of different approaches and strategies to consider in order to assist development of paediatric formulation for poorly water-soluble drugs with the provision of examples of some marketed products. In addition, it provides recommendations to overcome the range of challenges posed by these strategies and adaptations of the adult approach/product presentation required to enable paediatric drug development and administration

    Biomarkers and low risk in heart failure. Data from COACH and TRIUMPH

    Aim Traditionally, risk stratification in heart failure (HF) emphasizes assessment of high risk. We aimed to determine if biomarkers could identify patients with HF at low risk for death or HF rehospitalization. Methods and results This analysis was a substudy of The Coordinating Study Evaluating Outcomes of Advising and Counselling in Heart Failure (COACH) trial. Enrolment of HF patients occurred before discharge. We defined low risk as the absence of death and/or HF rehospitalizations at 180 days. We tested a diverse group of 29 biomarkers on top of a clinical risk model, with and without N-terminal pro-B-type natriuretic peptide (NT-proBNP), and defined the low risk biomarker cut-off at the 10th percentile associated with high positive predictive value. The best performing biomarkers together with NT-proBNP and cardiac troponin I (cTnI) were re-evaluated in a validation cohort of 285 HF patients. Of 592 eligible COACH patients, the mean (± SD) age was 71 (± 11) years and median (IQR) NT-proBNP was 2521 (1301-5634) pg/mL. Logistic regression analysis showed that only galectin-3, fully adjusted, was significantly associated with the absence of events at 180 days (OR 8.1, 95% confidence interval 1.06-50.0, P = 0.039). Galectin-3, showed incremental value when added to the clinical risk model without NT-proBNP (increase in area under the curve from 0.712 to 0.745, P = 0.04). However, no biomarker showed significant improvement by net reclassification improvement on top of the clinical risk model, with or without NT-proBNP. We confirmed our results regarding galectin-3, NT-proBNP, and cTnI in the independent validation cohort. Conclusion We describe the value of various biomarkers to define low risk, and demonstrate that galectin-3 identifies HF patients at (very) low risk for 30-day and 180-day mortality and HF rehospitalizations after an episode of acute HF. Such patients might be safely discharged
