Time constraints in digital data storage (Alastair Reynolds: Slow Bullets)

Knowledge, cultural and personal memory is increasingly being associated with digitally stored data. The formation and self-representation of individual identity is similarly happening in a largely computational environment, including social media sites and online social networks. Accordingly, we are presented with profane eschatological promises linked to technological advancement, such as immortality in this world and an eternal conscience, but encompassing all those optimistic interpretations of the digital revolution that concentrate only on the positive aspects. Alastair Reynolds’s 2015 science-fiction novel, Slow Bullets, aims to raise awareness of the time constrains and potential vulnerabilities of digital data storage and transfer. The narrative is reduced to the enclosed space of a single spaceship, thus subjecting the outside world (both as an interconnected ecological system and as the “universe of data”) to the danger of total annihilation. Among others, Reynolds’s novel stresses the importance of implementing and reinforcing a mindset where media may be used and criticised thoughtfully and deliberately

Fiatalkori iliofemoralis mélyvénás thrombosis ritka oka: tünetmentes vena cava inferior elzáródás

INTRODUCTION: Acute lower extremity deep venous thrombosis in young adults is usually related to thrombophilia, immobility, trauma, surgery or malignancy. Therapeutic options have recently included pharmacomechanical thrombus removal although there is no evidence of the indication or of the long-term benefit. Rarely unknown, asymptomatic chronic neonatal inferior vena cava occlusion could also lead to acute ilio-femoral thrombosis. AIM: The aim of the authors was to present the therapeutic possibilities in lower extremity deep venous thrombosis connected with chronic inferior vena cava occlusion. METHOD: In a retrospective single center study data of 21 adults were analyzed. RESULTS: In 4 of the 21 patients chronic inferior vena cava obstruction was identified as an underlying cause of the acute thrombosis. Pharmacomechanical lysis was not offered to them and anticoagulation therapy was introduced. After an average 27.5 +/- 11 month follow-up all the patients were alive and no adverse event occurred. CONCLUSIONS: In patients with acute lower extremity deep venous thrombosis and chronic inferior vena cava occlusion the indication of the endovenous pharmacomechanical lysis therapy is questionable. The authors suggest to consider computed tomography angiography or magnetic resonance angiography in addition to duplex sonography in patients with no obvious risk factors for lower extremity deep venous thrombosis to avoid unnecessary invasive treatment. Interobserver agreement and diagnostic accuracy of shearwave elastography for the staging of hepatitis C virus-associated liver fibrosis

Abstract PURPOSE: Our study aimed to evaluate the technical success rate, interobserver reproducibility, and accuracy of shearwave elastography (SWE) in the staging of hepatitis C virus (HCV)-associated liver fibrosis. METHODS: A total of 10 healthy controls and 49 patients with chronic liver disease were enrolled prospectively. Two examiners performed point shearwave elastography (pSWE) and two-dimensional shearwave elastography (2D-SWE) measurements with an RS85A ultrasound scanner using the S-Shearwave application (Samsung Medison, Hongcheon, Korea). The performance of S-Shearwave in the staging (METAVIR F0-F4) of liver fibrosis was compared with prior transient elastography (TE) with receiver operating characteristic (ROC) curve analysis. RESULTS: The interobserver reproducibility was excellent with pSWE (ICC = 0.92, 95% CI: 0.86-0.95, P < .001). A very good agreement was found between pSWE and TE measurements (ICC =0.85, 95% CI: 0.78-0.89, P < .001). The ROC analysis determined the optimal cut-off values of pSWE for the staging of chronic hepatitis C-associated fibrosis (F2, 1.46 m/s; F3, 1.63 m/s; F4, 1.95 m/s). Both observers achieved excellent diagnostic accuracy (AUROC: 94% vs 97%) in the detection of significant (≥F2) liver fibrosis. CONCLUSION: The interobserver agreement is excellent with S-Shearwave pSWE, and observers can diagnose significant liver fibrosis with a comparable accuracy to TE
