    Management guidelines for assisted reproduction in the HIV infected couple

    Human immunodeficiency virus (HIV) is a chronic disease that mostly affects individuals of reproductive age. Many of these individuals express the desire to have their own biological children. The two major ethical concerns remain the welfare of the offspring and the avoidance of seroconversion of the uninfected partner. Currently the life expectancy of an HIV infected individual is estimated at 20 years from the time of diagnosis. Today HIV treatments and practices can limit the risk of viral transmission to the offspring and uninfected partner. A multidisciplinary team approach and pre-conceptual counselling always remains mandatory in the management of these couples. Treatment should be individualised depending on which partner is affected and the underlying cause of infertility. Three scenarios are possible with regard to HIV status: positive female partner and negative male partner, negative female partner and positive male partner and both partners positive. These options will be discussed further. Health care facilities should thus, if it is economically and technically feasible, aim to offer fertility services to HIV infected individuals and discordant couples who are willing to use risk reducing treatment modalities.Keywords: Assisted Reproduction; HIV serodiscordant and Concordant couple

    Semen profiles of male partners in females presenting with endometriosis-associated subfertility

    Objective: To establish fertile and subfertile groups of males in couples presenting with endometriosis-related infertility in the female. Design: Retrospective study of semen profiles on partners of women undergoing surgery for endometriosis-related infertility. Setting: University-affiliated assisted reproduction center. Patient(s): One hundred seventeen partners of females undergoing surgery for endometriosis related infertility. Main Outcome Measure(s): Fertility potential according to Tygerberg strict criteria for sperm evaluation. Result(s): A total of 65.8% of the semen analysis were reported normal according to the Tygerberg strict criteria and 34.2% were reported subfertile. Of the total, 11.96% of the patients studied had a severe defect (azoospermia, double, and triple defects). Conclusion(s): Labeling of the male in couples presenting with infertility as fertile, subfertile, or infertile plays an important role in decision making regarding management of the female presenting with endometriosis-related infertility. It is important that in future studies on the effect of surgery on improving pregnancy outcome in patients with endometriosis the semen profile be reported and thresholds used should be mentioned. This will contribute significantly to the future comparison of data on the impact of the male factors in couples presenting with endometriosis-related infertility. © 2009 American Society for Reproductive Medicine.Articl
