    PANEL RESEARCH ON POVERTY IN IRELAND. ESRI Working Paper No. 14, October 1989

    This paper attempts to describe the major research project on Poverty and related issues on which we at the ESRI have been working for the past four years. This project forms part of the EC-funded co-ordinated Research Project on Poverty which Professor Deleeck, the co-ordinator, has discussed in the previous paper. I trust that my description of the Irish experiences. both positive and negative, with this type of inquiry will help to elucidate the contribution which such studies can make to our understanding of poverty in Europe today and suggest guidelines for the conduct of such research in the future. I hope that my comments will complement those of Professor Deleeck by sketching the overall approach to the analysis of poverty adopted by one of the countries involved

    Using dense Sentinel-2 time series to explore combined fire and drought impacts in eucalypt forests

    Following one of the driest years on record, millions of hectares of forests in southeast Australia were burned in the 2019-20200 "Black Summer" wildfires. In addition to the areas burned, drought related canopy collapse, dieback and tree mortality was widely observed. In this paper, we present a method to map canopy damage due to drought and fire across a large area. Sentinel-2 satellite imagery was used in a monthly time series to highlight areas of forest where the Normalized Burn Ratio index was significantly below a pre-disturbance "stable" period. The stable period was defined as the 3 years prior to 2019 and the disturbance thresholds are based on bioregion specific standard deviations below pre-disturbance means. The novel methods enabled drought impacted forests to be identified, including those which were subsequently burned by wildfire. Across the 20 Mha of forests studied, 9.9 Mha (49%) fell below the disturbance threshold. Of that, 5.8 Mha was disturbed by fire and a further 4.1 Mha by drought outside of the fire extent. Within the fire extent, almost 0.9 Mha was identified as being significantly drought affected prior to being burned. An analysis of spectral recovery following substantial rainfall from February 2020 onward indicates that most of the areas impacted by both drought and fire have similar rates of recovery to those impacted only by fire. There are some areas, however, where the combined effects of the "double disturbance "appears to be hindering recovery. The methods presented here are easily transferrable and demonstrate an approach for monitoring forest disturbance at higher temporal and spatial scales than those typically used

    Implications of gender-affirming endocrine care for sports participation

    Many transgender (trans) individuals utilize gender-affirming hormone therapy (GAHT) to promote changes in secondary sex characteristics to affirm their gender. Participation rates of trans people in sport are exceedingly low, yet given high rates of depression and increased cardiovascular risk, the potential benefits of sports participation are great. In this review, we provide an overview of the evidence surrounding the effects of GAHT on multiple performance-related phenotypes, as well as current limitations. Whilst data is clear that there are differences between males and females, there is a lack of quality evidence assessing the impact of GAHT on athletic performance. Twelve months of GAHT leads to testosterone concentrations that align with reference ranges of the affirmed gender. Feminizing GAHT in trans women increases fat mass and decreases lean mass, with opposite effects observed in trans men with masculinizing GAHT. In trans men, an increase in muscle strength and athletic performance is observed. In trans women, muscle strength is shown to decrease or not change following 12 months of GAHT. Haemoglobin, a measure of oxygen transport, changes to that of the affirmed gender within 6 months of GAHT, with very limited data to suggest possible reductions in maximal oxygen uptake as a result of feminizing GAHT. Current limitations of this field include a lack of long-term studies, adequate group comparisons and adjustment for confounding factors (e.g. height and lean body mass), and small sample sizes. There also remains limited data on endurance, cardiac or respiratory function, with further longitudinal studies on GAHT needed to address current limitations and provide more robust data to inform inclusive and fair sporting programmes, policies and guidelines

    Yang's gravitational theory

    Yang's pure space equations (C.N. Yang, Phys. Rev. Lett. v.33, p.445 (1974)) generalize Einstein's gravitational equations, while coming from gauge theory. We study these equations from a number of vantage points: summarizing the work done previously, comparing them with the Einstein equations and investigating their properties.     A scalable, fully automated process for construction of sequence-ready human exome targeted capture libraries

    Genome targeting methods enable cost-effective capture of specific subsets of the genome for sequencing. We present here an automated, highly scalable method for carrying out the Solution Hybrid Selection capture approach that provides a dramatic increase in scale and throughput of sequence-ready libraries produced. Significant process improvements and a series of in-process quality control checkpoints are also added. These process improvements can also be used in a manual version of the protocol

    Uncovering the effects of gender affirming hormone therapy on skeletal muscle and epigenetics: protocol for a prospective matched cohort study in transgender individuals (the GAME study)

    INTRODUCTION: Gender affirming hormone therapy (GAHT) is increasingly used by transgender individuals and leads to shifts in sex hormone levels. Skeletal muscle is highly responsive to hormone activity, with limited data on the effects of GAHT on different human tissues. Here, we present the protocol for the GAME study (the effects of Gender Affirming hormone therapy on skeletal Muscle training and Epigenetics), which aims to uncover the effects of GAHT on skeletal muscle 'omic' profiles (methylomics, transcriptomics, proteomics, metabolomics) and markers of skeletal muscle health and fitness. METHODS AND ANALYSIS: This study is a prospective age-matched cohort study in transgender adults commencing GAHT (n=80) and age-matched individuals not commencing GAHT (n=80), conducted at Austin Health and Victoria University in Victoria, Australia. Assessments will take place prior to beginning GAHT and 6 and 12 months into therapies in adults commencing GAHT. Age-matched individuals will be assessed at the same time points. Assessments will be divided over three examination days, involving (1) aerobic fitness tests, (2) muscle strength assessments and (3) collection of blood and muscle samples, as well as body composition measurements. Standardised diets, fitness watches and questionnaires will be used to control for key confounders in analyses. Primary outcomes are changes in aerobic fitness and muscle strength, as well as changes in skeletal muscle DNA methylation and gene expression profiles. Secondary outcomes include changes in skeletal muscle characteristics, proteomics, body composition and blood markers. Linear mixed models will be used to assess changes in outcomes, while accounting for repeated measures within participants and adjusting for known confounders. ETHICS AND DISSEMINATION: The Austin Health Human Research Ethics Committee (HREC) and Victoria University HREC granted approval for this study (HREC/77146/Austin-2021).     Comparison of the Airtraq® and Truview® laryngoscopes to the Macintosh laryngoscope for use by Advanced Paramedics in easy and simulated difficult intubation in manikins

    <p>Abstract</p> <p>Background</p> <p>Paramedics are frequently required to perform tracheal intubation, a potentially life-saving manoeuvre in severely ill patients, in the prehospital setting. However, direct laryngoscopy is often more difficult in this environment, and failed tracheal intubation constitutes an important cause of morbidity. Novel indirect laryngoscopes, such as the Airtraq<sup>® </sup>and Truview<sup>® </sup>laryngoscopes may reduce this risk.</p> <p>Methods</p> <p>We compared the efficacy of these devices to the Macintosh laryngoscope when used by 21 Paramedics proficient in direct laryngoscopy, in a randomized, controlled, manikin study. Following brief didactic instruction with the Airtraq<sup>® </sup>and Truview<sup>® </sup>laryngoscopes, each participant took turns performing laryngoscopy and intubation with each device, in an easy intubation scenario and following placement of a hard cervical collar, in a SimMan<sup>® </sup>manikin.</p> <p>Results</p> <p>The Airtraq<sup>® </sup>reduced the number of optimization manoeuvres and reduced the potential for dental trauma when compared to the Macintosh, in both the normal and simulated difficult intubation scenarios. In contrast, the Truview<sup>® </sup>increased the duration of intubation attempts, and required a greater number of optimization manoeuvres, compared to both the Macintosh and Airtraq<sup>® </sup>devices.</p> <p>Conclusion</p> <p>The Airtraq<sup>® </sup>laryngoscope performed more favourably than the Macintosh and Truview<sup>® </sup>devices when used by Paramedics in this manikin study. Further studies are required to extend these findings to the clinical setting.</p

    Developing the practice context to enable more effective pain management with older people: an action research approach

    Background\ud \ud This paper, which draws upon an Emancipatory Action Research (EAR) approach, unearths how the complexities of context influence the realities of nursing practice. While the intention of the project was to identify and change factors in the practice context that inhibit effective person-centred pain management practices with older people (65 years or older), reflective critical engagement with the findings identified that enhancing pain management practices with older people was dependent on cultural change in the unit as a whole. \ud \ud Methods\ud \ud An EAR approach was utilised. The project was undertaken in a surgical unit that conducted complex abdominal surgery. Eighty-five percent (n = 48) of nursing staff participated in the two-year project (05/NIR02/107). Data were obtained through the use of facilitated critical reflection with nursing staff. \ud \ud Results\ud \ud Three key themes (psychological safety, leadership, oppression) and four subthemes (power, horizontal violence, distorted perceptions, autonomy) were found to influence the way in which effective nursing practice was realised. Within the theme of 'context,' effective leadership and the creation of a psychologically safe environment were key elements in the enhancement of all aspects of nursing practice. \ud \ud Conclusions\ud \ud Whilst other research has identified the importance of 'practice context' and models and frameworks are emerging to address this issue, the theme of 'psychological safety' has been given little attention in the knowledge translation/implementation literature. Within the principles of EAR, facilitated reflective sessions were found to create 'psychologically safe spaces' that supported practitioners to develop effective person-centred nursing practices in complex clinical environments
