    Re-evaluation of waist circumference in metabolic syndrome: a comparison between Japanese men and women

    We re-evaluated the criteria for waist circumference to predict the accumulation of the components of metabolic syndrome. We used data for 3,185 Japanese, aged 20-79 years. Metabolic syndrome has recently been redefined by a new criterion in Japan, in which waist circumference cutoff points, i.e. 85 cm for men and 90 cm for women, are employed. Among the 3,185 Japanese considered in the present study, 335 men (26.8%) and 69 women (3.6%) were diagnosed as having metabolic syndrome. A cutoff point as a predictor for 2 or more components of metabolic syndrome was evaluated by sensitivity/specificity and a receiver operating characteristic (ROC) curve. The optimal point was estimated as being approximately 85 cm of waist circumference in men and 75 cm in women. We therefore recommend a cutoff value, 75 cm of waist circumference, for the criterion of metabolic syndrome in women

    L-Arginine treatment may prevent tubulointerstitial nephropathy caused by germanium dioxide.BackgroundLong-term oral ingestion of germanium dioxide (GeO2) causes progressive renal failure derived from tubulointerstitial nephropathy in humans and animals. The characteristic of GeO2-induced nephropathy is the renal tissue injury persisting for a long time, even after cessation of GeO2 ingestion. However, a treatment that can suppress the long-lasting renal tissue injury has not yet been established.MethodsUsing the methods of immunohistochemistry and reverse transcription-polymerase chain reaction, we examined the expression of ED1-positive cells (macrophages/monocytes), transforming growth factor (TGF)-β1 mRNA and protein and collagen type IV mRNA and protein in the kidneys of rats with GeO2-induced nephropathy. Concomitantly, the effects of L-arginine treatment on their expression was explored in the kidneys of rats with GeO2-induced nephropathy.ResultsChronic administration of GeO2 caused tubulointerstitial nephropathy characterized by leukocyte invasion into the enlarged tubulointerstitial space in rats. The expression of ED1-positive cells, TGF-β1 protein and collagen type IV protein was markedly increased in the tubulointerstitium of the renal cortex from rats with GeO2-induced nephropathy. Similarly, TGF-β1 and collagen type IV mRNA were significantly enhanced in the renal cortex of rats with GeO2-induced nephropathy. A small number of tubulointerstitial cells expressing TGF-β1 protein were also observed in the renal cortex of rats with GeO2-induced nephropathy. However, L-arginine treatment led to a parallel decrease in the expression of ED1-positive cells, TGF-β1 mRNA and collagen type IV mRNA and protein in rats with GeO2-induced nephropathy.ConclusionsIn general, collagen synthesis is driven by TGF-β1 in the fibrotic process associated with a variety of renal disorders. TGF-β1 is secreted by TGF-β1 producing cells such as macrophages, fibroblasts and myofibroblasts. Thus, the present study indicates that the expression of collagen type IV may be mediated by TGF-β1 released from invading macrophages and, to a lesser extent, released from tubulointerstitial cells, presumably fibroblasts and/or myofibroblasts in GeO2-induced nephropathy. L-Arginine treatment inhibits collagen type IV synthesis possibly by suppressing macrophage invasion and the resultant TGF-β1 expression in this nephropathy. L-Arginine treatment may be beneficial in the prevention of tubulointerstitial fibrosis, which is considered to be the terminal stage of GeO2-induced nephropathy

    Rab7-Mediated Endocytosis Establishes Patterning of Wnt Activity through Inactivation of Dkk Antagonism

    Endocytosis has been proposed to modulate cell signaling activities. However, the role of endocytosis in embryogenesis, which requires coordination of multiple signaling inputs, has remained less understood. We previously showed that mouse embryos lacking a small guanosine triphosphate (GTP)-binding protein Rab7 implicated in endocytic flow are defective in gastrulation. Here, we investigate how subcellular defects associated with Rab7 deficiency are related to the observed developmental defects. Rab7-deficient embryos fail to organize mesodermal tissues due to defects in Wnt-β-catenin signaling. Visceral endoderm (VE)-specific ablation of Rab7 results in patterning defects similar to systemic Rab7 deletion. Rab7 mutants accumulate the Wnt antagonist Dkk1 in the extracellular space and in intracellular compartments throughout the VE epithelium. These data indicate that Rab7-dependent endocytosis regulates the concentration and availability of extracellular Dkk1, thereby relieving the epiblast of antagonism. This intercellular mechanism therefore organizes distinct spatiotemporal patterns of canonical Wnt activity during the peri-gastrulation stages of embryonic development

    Current Status of Coronary Artery Surgery in Japan

    The incidence of coronary artery disease in Japan is relatively low compared to that in western countries. The epidemiologic status of coronary artery diseases in our country was discussed. The selective cine coronary arteriography in 230 patients with chest pain, as well as clinical diagnosis and electrocardiographic findings were reviewed in order to study the surgical pictures of coronary artery disease among Japanese. A current review or aortocoronary bypass in Japan is introduced together with our own experiences. In western countries coronary artery surgery has rapidly become one of the most common heart operations. In Japan, on the contrary, our survey indicates that coronary artery surgery is less than 1%. Therefore, we would like to speculate the present status of coronary artery surgery in Japan in order to compare the situation between these countries

    Selective coronary arteriography is the method of choice for visualizing the anatomical structures and pathological changes of the coronary artery tree in living subjects. It is helpful particularly to clarify the diagnosis of patients who have angina pectoris, because it gives clinically valuable information that is not available from any other source. The technique of selective catheterization and injection of each coronary artery in turn, combined with cineangiography, was introducted by Sones and Shirey and it is the most widely used method in the United States of America today. However, selective coronary arteriography may be performed by various techniques. The purpose of this paper is to relate our experience with the spot film roentgenographic technique in conjunction with the viceoangiocardiographic method
