    Bioprosthetic Aortic Valve degeneration: a review from a basic science perspective

    Introduction: The increase in the prevalence of aortic stenosis due to an aging population has led to an increasing number of surgical aortic valve replacements. Over the past 20 years, there has been a major shift in preference from mechanical to bioprosthetic valves. However, despite efforts, there is still no "ideal" bioprosthesis. It is crucial to understand the structure, biology, and function of native heart valves to design more intelligent, strong, durable, and physiological heart valve tissues. Methods: A comprehensive review of the literature was performed to identify articles reporting the basic mechanisms of bioprosthetic valve dysfunction and the biology of native valve cells. Searches were run in PubMed,MEDLINE (R) (the Medical Literature Analysis and Retrieval System Online), and Google Scholar. Terms for subject heading and keywords search included "biological heart valve dysfunction", "bioprosthesis dysfunction", "bioprosthesis degeneration", and "tissue heart valves". Results: All the relevant findings are summarized in the appropriate subsections. Structural dysfunction is a logical and expected consequence of the chemical, mechanical, and immunological processes that occur during fixation, manufacture, and implantation. Conclusion: Biological prosthesis valve dysfunction is a clinically significant process. It has become a major issue considering the growing rate of bioprosthesis implantation and improved long-term patient survival. Understanding bioprosthetic aortic valve degeneration from a basic science perspective is a key point to improve technologic advances and specifications that lead to a new generation of bioprostheses.info:eu-repo/semantics/publishedVersio

    Platelet count drop after rapid deployment aortic valve implantation

The implantation of the Perceval valve has been associated with a more intense drop of platelet count compared to other bio-prostheses. This study analyses and compares the platelets progression associated with the Perceval and Intuity valves. Methods: The data was collected retrospectively for patients submitted to isolated aortic valve replacement with the Perceval valve (80 patients) and the Intuity valve (141 patients) in our institution between March 2014 and December 2018. The groups were further divided into those who receive platelet transfusion and those who did not. Results: The minimum values of platelet count were 54% and 67% of the preoperative platelet count in the patients treated with a Perceval and an Intuity valves, respectively (p<0.001). In the patients transfused with platelets, the minimum values were 52% and 79% of the preoperative platelet count, respectively (p<0.01). Recovery of the count was faster in the patients treated with an Intuity valve. Abnormal bleeding and transfusion of packed red blood cells were not significantly different between groups (without platelet transfusion: p=0.71 and p=0.99, respectively; with platelet transfusion: p=0.58 and p=0.99, respectively). Conclusion: Compared to the Intuity valve, the Perceval valve is associated with a transient, but significant, drop in platelet count. This drop was not associated to an increased risk of bleeding. Platelet transfusion, in this setting, should be judicious and not only ruled by absolute values.info:eu-repo/semantics/publishedVersio

    O Balneário da Amizade é considerado o manancial que pertence à bacia hidrográfica do alto curso do córrego do Limoeiro localizado entre os municípios de Presidente Prudente e Álvares Machado. O recurso hídrico é utilizado em casos emergenciais para abastecimento da rede de distribuição de água da Companhia do Saneamento Básico do Estado de São Paulo, sendo destinado à prática de esportes, lazer e turismo. Tendo em vista as características do local, objetivou-se levantar os aspectos ambientais da área de estudo e analisar a qualidade da água, visando à comparação dos resultados com a resolução do Conselho Nacional do Meio Ambiente. A metodologia baseou-se na norma Standard Methods for the Examination of Water &amp; Wastewater para as análises de coliformes termotolerantes e Escherichia coli dos cinco pontos de coleta e na caracterização da área de estudo. Levando em consideração o resultado do levantamento de aspectos ambientais, verificou-se que esses fatores interferiram na qualidade da água. Para avaliar a balneabilidade, considerou-se o ponto permitido para acesso aos banhistas. Ao comparar os resultados dos parâmetros microbiológicos, notou-se que a presença de 83.660 coliformes termotolerantes restringe o uso para contato primário no ponto 2, sendo considerado impróprio aos banhistas, conforme descreve a Resolução CONAMA nº 274/2000.&nbsp

    Bowel symptoms associated with coronavirus disease 19 in hospitalized patients with moderate to severe illness

    Bowel symptoms, such as diarrhea, have higher prevalence during coronavirus disease 2019 (COVID-19). The correlation between the severity of these symptoms and their prognosis has not been defined yet. Furthermore, higher prevalence of gastrointestinal symptoms after recovery from COVID-19 has been reported. This study aimed to analyze the correlation of potential factors with the severity of diarrhea during COVID-19 and to assess the progression of post-COVID-19 bowel symptoms. This prospective longitudinal cohort included 109 patients with moderate to severe COVID-19 symptoms who were hospitalized from May to July 2021. Patients were interviewed to assess the presence and intensity of bowel symptoms during COVID-19 and 3–6 months after hospital discharge using the Gastrointestinal Symptom Rating Scale questionnaire. Demographic and clinical data were obtained and their correlations with the intensity of bowel symptoms were examined. The presence of severe diarrhea was correlated with the need for ventilatory support and the use of anticoagulants but not with the use of antibiotics. In addition, bowel symptoms, such as loose stools and incomplete evacuation but not diarrhea, constipation, or abdominal pain, persisted for at least 3–6 months after hospital discharge. These results suggest that the severity of diarrhea during COVID-19 may be associated with worsening of the disease and that only loose stools and incomplete evacuation are present after COVID-19. The chronicity of these bowel symptoms should be evaluated to improve the treatment of patients with COVID-19

    POST-DISSECTION THORACO-ABDOMINAL ANEURYSMS: RESULTS OF OPEN AND ENDOVASCULAR REPAIR

    Introduction/Objectives: Repair of post-dissection thoraco-abdominal aortic aneurysms (PD-TAAA) is a complex challenge. Choosing the correct approach to manage these aneurysms is not straightforward as both open and endovascular strategies are valid. Our aim was to analyze and compare the results of PD-TAAA treated by endovascular or open surgery. Methods: A seven-year (January-2013 and May-2020) single-center retrospective cohort study of patients with PD-TAAA treated by endovascular (group-1) or open (group-2) surgery was conducted. Primary outcome was in-hospital mortality. Secondary outcomes were organ lesion, in-hospital infections, length of stay (LOS), endoleaks, branch occlusions, re-interventions and mortality during follow-up. Results: Twenty-one patients (15-men) were treated: 8 in group-1 and 13 in group-2. The mean age was lower in group-2 [68 (SD:11) versus 48 (SD:12), p=0.004]. Three patients had connective tissue disease (CTD). Group-1 patients had a higher ASA score (p<0.001). In group-1, debranching and TEVAR were performed in 2 patients and custom-made fenestrated/ branched-endografts were used in 6. In group-2, there was one thoracic aorta interposition graft and reconstruction involving the visceral arteries ocurred in 12 patients. Seven cases were operated using the Crawford technique with visceral patch, and branched grafts were used in 3 patients with CTD. Intercostal arteries were revascularized in 5 patients. In-hospital mortality was 12% (1 patient) in group-1 and 15% (2 patients) in group-2, LogRank=0.9. The LOS was longer in group-2 (p=0.033), and there was a tendency for a longer stay in intensive care unit in this group. No difference was observed in spinal cord ischemia, acute kidney injury or re-interventions. There were more post-operative infections in group-2 (12% versus 69%, p = 0.017). During follow-up [median 15 months (IQR:55)], there was no mortality after discharge. In group-1, 14% had type-II-endoleaks, without aneurysmal sac dilation. Branch permeability during follow-up was 100% in group-1 and 95% in group-2, LogRank=0.3. Conclusion: Endovascular and open surgery of PD-TAAA allowed treatment of a wide variety of patients in this cohort. Patients treated by the endovascular surgery were older and had higher surgical risk but without repercussions on the outcomes. Open surgery was associated with longer hospital stay and more postoperative complications

    Standardization of in vitro digestibility and DIAAS method based on the static INFOGEST protocol

    Background: The FAO recommends the digestible indispensable amino acid score (DIAAS) as the measure for protein quality, for which the true ileal digestibility needs to be assessed in humans or pigs. However, due to high costs and ethical concerns, the FAO strongly encourages as well the development of validated in vitro methods, which complement the in vivo experiments. Method: Recently, an in vitro workflow, based on the validated static INFOGEST protocol, was developed and compared towards in vivo data. In parallel to the validation with in vivo data, the repeatability and reproducibility of the in vitro protocol were tested in an international ring trial (RT) with the aim to establish an international ISO standard method within the International Dairy Federation (IDF). Five different dairy products (skim milk powder, whole milk powder, whey protein isolate, yoghurt, and cheese) were analyzed in 32 different laboratories from 18 different countries, across 4 continents. Results: in vitro protein digestibilities based on Nitrogen, free R-NH2, and total amino acids as well as DIAAS values were calculated and compared to in vivo data, where available.
