Zahnmedizinische Betreuung von Pflegebedürftigen und Menschen mit Behinderungen
Zusammenfassung: 2009 lebten in Deutschland 7,1Millionen Menschen (8% der Gesamtbevölkerung) mit anerkannter Schwerbehinderung und rund 2,34Millionen Pflegebedürftige, wobei es teilweise Überschneidungen innerhalb der Gruppen gab. Obwohl mithilfe strukturierter Prophylaxeprogramme die Zahn- und Mundgesundheit der deutschen Bevölkerung insgesamt verbessert werden konnte, profitieren Pflegebedürftige und Menschen mit Behinderung heute noch nicht im selben Umfang von dieser Entwicklung. Sie weisen häufig infolge eingeschränkter Kooperativität, Immobilität, körperlicher und/oder geistiger Behinderung eine schlechtere Mundhygiene und damit verbunden höhere Kariesaktivität sowie schwerere Parodontalerkrankungen im Vergleich zur Durchschnittsbevölkerung auf. Ziel einer zukünftigen Gesundheitsversorgung sollte es sein, dass Pflegebedürftige und Menschen mit Behinderung eine gleich gute Mundgesundheit wie Personen ohne diese Einschränkungen aufweisen. Alle beteiligten Berufsgruppen, zum Beispiel Pflegekräfte, Leiter von Behinderten- und Pflegeeinrichtungen und der medizinische Dienst sollten ein zahnmedizinisches Problembewusstsein für diese Gruppen haben und eine ihren Aufgaben entsprechende Ausbildung im Bereich der Mundgesundheit vorweisen. Die zahnmedizinische Betreuung sollte die individuellen stomatologischen Defizite aus Behinderung, Gebrechlichkeit und Pflegebedürftigkeit ausgleichen. Um dies sicherzustellen, ist es wichtig, sowohl einen chancengleichen Zugang zur zahnmedizinischen Versorgung als auch ein den individuellen Einschränkungen angepasstes zahnmedizinisches Leistungsangebot vom Gesetzgeber einzuforder
Mundgesundheit, Zahnstatus und Ernährung im Alter
Zusammenfassung: Der Verlust natürlicher Zähne beeinträchtigt die Kaufunktion erheblich und kann durch die Eingliederung von Zahnprothesen nur teilweise ausgeglichen werden. Gleichzeitig erschweren oft Mundtrockenheit und Dysphagie die Nahrungsaufnahme bei alten Menschen. Subjektiv oft nicht wahrgenommen stellen Prothesenträger ihre Nahrungsmittelauswahl um und greifen auf spezielle Zubereitungsarten zurück. Letztendlich zeigt der Zahnstatus einen Einfluss auf die Nahrungsaufnahme. Mit abnehmender Zahnzahl werden weniger Kalorien, Proteine, Fett, nichtstärkehaltige Polysacharide und Vitamine aufgenommen. Oft werden die fehlenden Kalorien durch vermehrte Zucker und Fettaufnahme ausgeglichen. Besonders zahnlose Personen mit geringer Schulbildung greifen zu einer eher fett- und zuckerhaltigen Ernährung. Auch nimmt der tägliche Verzehr von Früchten und Gemüse mit der Anzahl der Seitenzahnkontakte ab. Eine Verbesserung der Kaufunktion durch zahnärztliche Maßnahmen geht nicht automatisch mit einer verbesserten Nahrungsaufnahme einher und sollte daher immer mit einer Ernährungsberatung kombiniert werde
Oral health of seafarers - a review
The research base needs to be expanded to cover all seafarers. Dental professional expertise should be sought in policy and guideline development relevant to oral health. A strategy comprising preventive, screening, and treatment service components should be developed and a certificate of dental health introduced. Funding strategies in a complex environment of transnational stakeholders for the improvement of oral-health services for seafarers are needed. Aspects of military oral health care systems could be an example for civilian operators
Hilft das zahnmedizinische Bonussystem den stationär Pflegebedürftigen?
Zusammenfassung: Die Mundgesundheit bei Pflegebedürftigen ist mangelhaft. Zufällig ausgewählte Pflegebedürftige (Berlin n=75, Nordrhein-Westfalen n=94, Sachsen n=73) aus 30 stationären Pflegeeinrichtungen wurden u.a. zu ihrem Verhalten der zahnmedizinischen Inanspruchnahme und der Nutzung ihres zahnmedizinischen Bonusheftes (zBH) befragt. Von 242Befragten (Median: 82Jahre, Range: 43-100Jahre, Frauen: 78,5%) besaßen nur 18,6% ein zBH, dabei gab es keine signifikanten geschlechts- und altersspezifischen Unterschiede. Regionale Unterschiede waren signifikant (Berlin 5,3%, Nordrhein-Westfalen 18,1%, Sachsen 32,9%; χ2-Test p<0,01). Die Zahnzahl war bei Bonusheftinhabern größer (Mann-Whitney-Test p=0,01), die Zeitspanne zum letzten Zahnarztbesuch geringer (p<0,01). Von den Bonusheftinhabern gaben 18,6% an, den Zahnarzt länger als 12Monate nicht aufgesucht zu haben (stationär Pflegebedürftige ohne Bonusheft 51,3%). Als Beitrag zur Qualitätssicherung, zur oralen Infektionskontrolle sowie zur Verbesserung der mundbezogenen und allgemeinen Lebensqualität wird die regelmäßige jährliche Durchführung zahnmedizinischer Reihenuntersuchungen mit dem Führen eines zBH für jeden Bewohner vorgeschlage

Collective Cargo Transport and Sorting with Molecular Swarms
Recent work has demonstrated the viability of DNA robotics
and artificial molecular machines for molecular transportation
and cargo sorting with potential applications in manufacturing
responsive molecular devices, programmable therapeutics,
and autonomous chemical synthesis. We extend
previous work on cooperative molecular transportation using
artificial molecular machines, where we similarly functionalize
DNA-conjugated microtubules driven by kinesin motor
proteins. DNA-functionalized microtubules propelled by
surface-adhered kinesin motors enable the self-organization
of molecular swarms, where such swarms load and transport
cargo (microbead) in a simulated chemical environment. We
demonstrate programmable molecular swarms for cargo sorting
and cooperative transport. Cargo loading occurs when
sufficient microtubules are at the same location as the cargo,
and cargo unloading occurs at specific points in the environment
through interaction with localized DNA species. Our
contribution is the design of a chemotaxis molecular controller,
forcing the swarm to tumble (random change direction)
when the system is not following a molecular gradient
corresponding to the cargo type, thus directing it to specific
points for cargo unloading. This work thus contributes to the
open problem of how to best design programmable molecular
machines for various tasks in microscopic environments
Thixotropy in macroscopic suspensions of spheres
An experimental study of the viscosity of a macroscopic suspension, i.e. a
suspension for which Brownian motion can be neglected, under steady shear is
presented. The suspension is prepared with a high packing fraction and is
density-matched in a Newtonian carrier fluid. The viscosity of the suspension
depends on the shear rate and the time of shearing. It is shown for the first
time that a macroscopic suspension shows thixotropic viscosity, i.e.
shear-thinning with a long relaxation time as a unique function of shear. The
relaxation times show a systematic decrease with increasing shear rate. These
relaxation times are larger when decreasing the shear rates, compared to those
observed after increasing the shear. The time scales involved are about 10000
times larger than the viscous time scale and about 1000 times smaller than the
thermodynamic time scale. The structure of the suspension at the outer cylinder
of a viscometer is monitored with a camera, showing the formation of a
hexagonal structure. The temporal decrease of the viscosity under shear
coincides with the formation of this hexagonal pattern
Discrete exterior calculus (DEC) for the surface Navier-Stokes equation
We consider a numerical approach for the incompressible surface Navier-Stokes
equation. The approach is based on the covariant form and uses discrete
exterior calculus (DEC) in space and a semi-implicit discretization in time.
The discretization is described in detail and related to finite difference
schemes on staggered grids in flat space for which we demonstrate second order
convergence. We compare computational results with a vorticity-stream function
approach for surfaces with genus 0 and demonstrate the interplay between
topology, geometry and flow properties. Our discretization also allows to
handle harmonic vector fields, which we demonstrate on a torus.Comment: 21 pages, 9 figure
Regulation and quantification of cellular mitochondrial morphology and content
Mitochondria play a key role in signal transduction, redox homeostasis and cell survival, which extends far beyond their classical functioning in ATP production and energy metabolism. In living cells, mitochondrial content (“mitochondrial mass”) depends on the cell-controlled balance between mitochondrial biogenesis and degradation. These processes are intricately linked to changes in net mitochondrial morphology and spatiotemporal positioning (“mitochondrial dynamics”), which are governed by mitochondrial fusion, fission and motility. It is becoming increasingly clear that mitochondrial mass and dynamics, as well as its ultrastructure and volume, are mechanistically linked to mitochondrial function and the cell. This means that proper quantification of mitochondrial morphology and content is of prime importance in understanding mitochondrial and cellular physiology in health and disease. This review first presents how cellular mitochondrial content is regulated at the level of mitochondrial biogenesis, degradation and dynamics. Next we discuss how mitochondrial dynamics and content can be analyzed with a special emphasis on quantitative live-cell microscopy strategies.acceptedVersio
