Effectiveness and safety of betrixaban extended prophylaxis for venous thromboembolism compared with standard-duration prophylaxis intervention in acute medically ill patients : a systematic literature review and network meta-analysis
Aims: To determine the clinical effectiveness and safety of venous thromboembolism (VTE) prophylaxis using US- and Europe-approved anticoagulants relative to extended-duration VTE prophylaxis with betrixaban. Low molecular weight heparins (LMWHs), unfractionated heparin (UFH), fondaparinux sodium and placebo were each compared to betrixaban, as standard-duration VTE prophylaxis for hospitalized, non-surgical patients with acute medical illness at risk of VTE.
Materials and methods: A systematic literature review was conducted up to June 2019 to identify randomized controlled trials (RCTs) of VTE prophylaxis in hospitalized, non-surgical patients with acute medical illness at risk of VTE. Studies that reported the occurrence of VTE events (including death) and, where possible, major bleeding, from treatment initiation to 20–50 days thereafter were retrieved and extracted. A Bayesian fixed effect network meta-analysis was used to estimate efficacy and safety of betrixaban compared with standard-duration VTE prophylaxis.
Results: Seven RCTs were analyzed which compared betrixaban, LMWHs, UFH, fondaparinux sodium, or placebo. There were significantly higher odds (median odds [95% credible interval]) of VTE with LMWHs (1.38 [1.12–1.70]), UFH (1.60 [1.05–2.46]), and placebo (2.37 [1.55–3.66]) compared with betrixaban. There were significantly higher odds of VTE-related death with placebo (7.76 [2.14–34.40]) compared with betrixaban. No significant differences were observed for the odds of major bleeding with all comparators, VTE-related death with any active standard-duration VTE prophylaxis, or of VTE with fondaparinux sodium, compared with betrixaban.
Limitations and conclusions: In this indirect comparison, betrixaban was shown to be an effective regimen with relative benefits compared with LMWHs and UFH. This indicates that betrixaban could reduce the burden of VTE in at-risk hospitalized patients with acute medical illness who need extended prophylaxis, though without direct comparative evidence, stronger conclusions cannot be drawn
Branch-and-lift algorithm for deterministic global optimization in nonlinear optimal control
This paper presents a branch-and-lift algorithm for solving optimal control problems with smooth nonlinear dynamics and potentially nonconvex objective and constraint functionals to guaranteed global optimality. This algorithm features a direct sequential method and builds upon a generic, spatial branch-and-bound algorithm. A new operation, called lifting, is introduced, which refines the control parameterization via a Gram-Schmidt orthogonalization process, while simultaneously eliminating control subregions that are either infeasible or that provably cannot contain any global optima. Conditions are given under which the image of the control parameterization error in the state space contracts exponentially as the parameterization order is increased, thereby making the lifting operation efficient. A computational technique based on ellipsoidal calculus is also developed that satisfies these conditions. The practical applicability of branch-and-lift is illustrated in a numerical example. © 2013 Springer Science+Business Media New York
Selective Exposure to Berita Harian Online and Utusan Malaysia Online: The Roles of Surveillance Motivation, Website Usability and Website Attractiveness
News media allows audiences to be selective in determining both their news sources
and type of news stories they read. This study examined factors influencing selective
exposure to the online editions of two mainstream Malaysian newspapers, Berita
Harian and Utusan Malaysia. Using selective exposure theory as the theoretical
lens, this study compared both newspapers in terms of their audiences’ level of
surveillance motivation, and how audiences rate the newspapers’ websites with respect
to usability and attractiveness. This study used a within-subject experimental research
design that exposed 51 subjects to both Berita Harian and Utusan Malaysia online
newspapers. The results of the experiment indicate that Berita Harian and Utusan
Malaysia online were significantly different in terms of website usability; however,
no significant differences were found in terms of surveillance motivation or website
attractiveness between the two newspapers. Further analysis indicate that the only
significant predictor of selective exposure was website usability. This study highlights
the importance of website usability for online newspapers wanting to harness audience
selectivity
Voto econômico retrospectivo e sofisticação política na eleição presidencial de 2002
Este artigo aborda o tema do voto econômico a partir da perspectiva da desigualdade de sofisticação política entre os eleitores. Diversos estudos mostram que os eleitores brasileiros tendem a se basear na economia para votar e avaliar os presidentes. Essas evidências algumas vezes servem de suporte para a afirmação de que, mesmo não utilizando os partidos, os rótulos ideológicos e outras questões politicamente relevantes como referências para suas escolhas, o eleitorado de massa no Brasil seria capaz de tomar decisões criteriosas na política. Utilizando dados de pesquisa de painel coletados ao longo de 2002 em Caxias do Sul e Juiz de Fora, o artigo testa até que ponto o efeito da avaliação retrospectiva da economia (tanto do país quanto pessoal) sobre o voto para presidente depende do nível de sofisticação política do respondente. Os resultados mostram que a avaliação da economia tende a ser uma opinião instável ao longo do período analisado, e tem impacto sobre a escolha do candidato a presidente apenas entre os eleitores mais politicamente sofisticados. As implicações dos achados são discutidas e aponta-se para a relevância de se levar em conta o impacto da desigualdade de sofisticação política entre os eleitores no entendimento da dinâmica das eleições e da opinião pública no Brasil
