    Promoting recovery and addressing stigma: mental health awareness through community development in a low-income area

    One in four Scots will experience mental ill-health at some point in their lives, with increased prevalence in low-income communities. The associated stigma increases the negative impact on an individual's life. This paper outlines the development of a local anti-stigma programme that can be replicated in other areas, and presents key findings from the evaluation. This innovative model, which promotes recovery and addresses stigma, draws on a broad coalition of community support, and enables service users to lead its design and delivery. The paper argues that local initiatives, when complemented by a national programme, can achieve a positive additional impact on attitudes and behaviours; that multiple and flexible approaches are needed for different target groups; that the personal narrative about recovery has a particularly strong impact on participants; and that it may be desirable to tackle stigma in the context of addressing positive mental health. Refinements to the model are discussed

    Participatory action research with asylum seekers and refugees experiencing stigma and discrimination : the experience from Scotland

    Using evidence from a participatory action research process with over 100 asylum seekers and refugees in Scotland, this study explores participants’ views on mental health problems, stigma and discrimination. The study found migration can have adverse effects on mental health and well-being, linked to people’s social circumstances such as racism and the asylum process and that this is exacerbated by stigma and discrimination. It suggests the importance of a socio-cultural context for understanding and addressing stigma, influenced by both social and cultural causal factors, including fear, past trauma, isolation, racism and the stress of the asylum process coupled with negative cultural beliefs about mental health problems. The paper considers the international relevance of this approach and the value of a model grounded in principles of community development and grassroots action

    Moving Forward : Implementing The United Nations Guidelines For The Alternative Care Of Children

    The subject of constant and serious concern expressed by the Committee on the Rights of the Child over its two decades of work to monitor and promote the implementation of the Convention on the Rights of the Child. This concern is not only evident from the Committee’s findings when reviewing individual States’ compliance with the treaty’s provisions, but was also manifested clearly and in global terms when it decided to devote its annual Day of General Discussion to that issue in 2005. The Committee’s preoccupations are based on a variety of factors. These include: • the large number of children coming into alternative care in many countries, too often essentially due to their family’s material poverty, the conditions under which that care is provided, and the low priority that may be afforded to responding appropriately to these children who, lacking the primary protection normally assured by parents, are particularly vulnerable. The reasons for which children find themselves in alternative care are wide-ranging, and addressing these diverse situations – preventively or reactively – similarly requires a panoply of measures to be in place. While the Convention sets out basic State obligations in that regard, it does not provide significant guidance on meeting them. This is why, from the very outset of the initiative in 2004, the Committee gave whole-hearted support to the idea of developing the Guidelines for the Alternative Care of Children that would gain the approval of the international community at the highest level. The acceptance of the Guidelines by the UN General Assembly in 2009 signalled all governments’ general agreement that the ‘orientations for policy and practice’ they set out are both well-founded and desirable. Since that time, the Committee has been making full use of the principles and objectives established in the Guidelines when examining the reports of States Parties to the Convention and in formulating its observations and recommendations to them. As with all internationally agreed standards and principles, however, the real test lies in determining how they can be made a reality throughout the world for those that they target – in this case, children who are without, or are at risk of losing, parental care. Identifying those measures means, first of all, understanding the implications of the ‘policy orientations’ proposed in the Guidelines, and then devising the most effective and ‘do-able’ ways of meeting their requirements. Importantly, moreover, the Guidelines are by no means addressed to States alone: they are to be taken into account by everyone, at every level, who is involved in some manner with issues and programmes concerning alternative care provision for children. This is where the Moving Forward handbook steps in. As its title suggests, it seeks precisely to assist all concerned to advance along the road to implementation, by explaining the key thrusts of the Guidelines, outlining the kind of policy responses required, and describing ‘promising’ examples of efforts already made to apply them in diverse communities, countries, regions and cultures. I congratulate all the organisations and individuals that have contributed to bringing the Moving Forward project to fruition. This handbook is clearly an important tool for informing and inspiring practitioners, organisations and governments across the globe who are seeking to provide the best possible rights-based solutions and care for their children

    Understanding and promoting student mental health in Scottish higher education - a mapping exercise

    There has been an increase in the incidence of mental health difficulties among HE students over the past decade. Official statistics show that the proportion of undergraduates declaring a mental health difficulty on entry to HE rose from 5 in every 10,000 in 1994-5 to 30 in every 10,000 in 2004-5. Incidence of severe psychological problems has increased, and student mental health is generally worse than that of the general population (for agematched populations). Anxiety and depression are the most commonly noted difficulties. The research review identified a relationship between mental health and the following factors: finances, accommodation, academic issues, university systems and social factors. Academic issues, and specifically coursework, emerged as particularly related to stress levels and mental health issues

    What has economics got to do with it? The impact of socioeconomic factors on mental health and the case for collective action

    A clear link exists between social and economic inequality and poor mental health. There is a social gradient in mental health, and higher levels of income inequality are linked to higher prevalence of mental illness. Despite this, in the late 20th and early 21st century, psychiatric and psychological perspectives have dominated mental health research and policy, obscuring root socioeconomic contributors. Drawing on contemporary research on the social determinants of mental health, with particular reference to Europe and the U.S., this paper argues that a sharper focus on socioeconomic factors is required in research and policy to address inequalities in mental health. Current attempts to move this direction include: evaluation of the impact of economic policies on mental health, community-based partnerships, increased professional awareness and advocacy on socioeconomic factors. This necessitates greater understanding of the barriers to such actions. This paper argues that advancing ‘upstream’ approaches to population mental health requires an interdisciplinary research vision that supports greater understanding of the role of socioeconomic factors. It also demands collective cross-sectoral action through changes in social and economic policy, as well as economic frameworks that move beyond an exclusive focus on economic growth to embrace collective and societal wellbeing

    Investigating Views of Science and Humanities: Tertiary Educated Adults on Complementary and Alternative Medicines

    This qualitative study explored the understandings of people with a university education in science and those without a university science background about the evidence base relating to Complementary and Alternative Medicine- a controversial multibillion-dollar industry on a global scale. The findings indicated that science- trained and nonscience- trained respondents alike valued scientific rigor and testing in relation to health care but also used subjective kinds of evidence in justifying their views and decisions about CAMS. In addition, both science and humanities graduates engaged with evidence in similar ways as defined by "habits of mind." These findings are discussed in relation to their implications for science education and engagement with scientific ideas, including scientific literacy and the belief systems that people bring to their understanding of science

    Strategies to limit the impact of nematode pressure on sugarcane productivity in the Isis

    Two trials were done in this project. One was a continuation of work started under a previous GRDC/SRDC-funded activity, 'Strategies to improve the integration of legumes into cane based farming systems'. This trial aimed to assess the impact of trash and tillage management options and nematicide application on nematodes and crop performance. The impact of trash and tillage management options and nematicide application on crop performance and plant-parasitic nematode populations in a sugarcane/peanut farming system. Proc. Aust. Soc. Sugar Cane Technol. 37, 192-203. Nematicide application in the plant crop significantly reduced total numbers of plant parasitic nematodes (PPN) but there was no impact on yield. Application of nematicide to the ratoon crop significantly reduced sugar yield. The study confirmed other work demonstrating that implementation of strategies like reduced tillage reduced populations of total PPN, suggesting that the soil was more suppressive to PPN in those treatments. The second trial, a variety trial, demonstrated the limited value of nematicide application in sugarcane farming systems. This study has highlighted that growers shouldn’t view nematicides as a ‘cure all’ for paddocks that have historically had high PPN numbers. Nematicides have high mammalian toxicity, have the potential to contaminate ground water (Kookana et al. 1995) and are costly. The cost of nematicide used in R1 was approx. 320−320 - 350/ha, adding $3.50/t of cane in a 100 t/ha crop. Also, our study demonstrated that a single nematicide treatment at the application rate registered for sugarcane is not very effective in reducing populations of nematode pests. There appears to be some levels of resistance to nematodes within the current suite of varieties available to the southern canelands. For example the soil in plots that were growing Q183 had 560% more root knot nematodes / 200mL soil compared to plots that grew Q245. The authors see great value in investment into a nematode screening program that could rate varieties into groups of susceptibility to both major sugarcane nematode pests. Such a rating could then be built into a decision support ‘tree’ or tool to better enable producers to select varieties on a paddock by paddock basis

    Omani senior secondary school students' knowledge of and attitudes to antibiotic resistance

    Antibiotic resistance is a worldwide problem that is increasing largely due to the misuse of antibiotics in human health and agriculture. This situation is further exacerbated by a dearth of new antibiotic development, the focus of pharmaceutical companies having shifted to more lucrative treatments for chronic conditions such as elevated blood pressure. To conserve the efficacy of the current crop of antibiotics, it is vital that they are used appropriately by individuals. Effective education may be a means to achieve such appropriate use. This paper reports on a large-scale, mixed methods study, which employed a survey and oral questionnaires, undertaken with senior secondary Omani students. The study explored students' understanding of antibiotic resistance as well as their attitudes to the issue of antibiotic resistance. The study findings indicated that, although some students had a reasonably clear understanding of antibiotic resistance, many had serious misconceptions that could result in misuse of antibiotics. The article concludes with suggestions for amending secondary school pedagogy in Oman to address the misconceptions

    Integration or isolation? Refugees’ social connections and wellbeing

Drawing on an innovative participatory mapping approach, this study examined the social connections of isolated single refugee men from Iran and Afghanistan (highlighted as particularly marginalized) and the implications for their mental health and wellbeing. Findings indicated very low levels of contact with family, local friends or local services, difficulties establishing trust and few opportunities for reciprocal relationships. The article makes an important contribution to the field of refugee integration in a number of ways. It suggests that the role of trust should be made explicit within the Indicators of Integration framework and be included as a ‘Facilitator’ of integration. It challenges Putnam’s simple binary distinction between bonding and bridging relationships and suggests a new conceptualization based on a continuum between bonds and bridges. It offers theoretical innovation by bringing together the concept of reciprocity with Hobfoll’s resource-conservation model to offer new insights into the way domains of the Framework interact. Its important contribution is in critiquing Putnam’s reliance on the idealization of community solidarity and suggesting conceptualizations of integration must be informed by the impact of intersecting but differentiated communities. Two key priorities emerge for policy and practice: enabling asylum seekers and refugees to develop sufficient close bonding relationships and finding more effective ways of building knowledge and trust of relevant resources and services.https://doi.org/10.1093/jrs/fez04034pubpub

    Developing family-based care : complexities in implementing the UN guidelines for the alternative care of children

    In response to immense challenges facing children in out-of-home care in all parts of the world, there is a growing international trend towards the development of family-based placements for children in out-of-home care, away from large-scale institutions. This development of family based care within a range of care options is recommended within the international Guidelines for the Alternative Care of Children (the Guidelines), which were welcomed unanimously by the United Nations General Assembly in 2009. This paper offers an overview of these guidelines’ key principles, and considers the complexities that arise in efforts towards their implementation. Drawing on the literature, supported by research that informed Moving forward (the implementation handbook on the Guidelines) and illustrated by practice examples from across global regions, the authors examine three fundamental challenges in States’ efforts to implement the Guidelines’ ‘suitability’ principle, namely: de-institutionalising the care system; financing suitable family-based care and supporting the suitability of kinship care. The paper critically reflects on de-institutionalised systems and practices, and the cross-cultural assumptions about suitable foster and kinship care that emerge in efforts towards de-institutionalisation; it aims to spark new thinking on strategic ways in which alternative care is planned and delivered, to impact on future practice
