    A Protocol for a Pan-Canadian Prospective Observational Study on Active Surveillance or Surgery for Very Low Risk Papillary Thyroid Cancer

    BackgroundThe traditional management of papillary thyroid cancer (PTC) is thyroidectomy (total or partial removal of the thyroid). Active surveillance (AS) may be considered as an alternative option for small, low risk PTC. AS involves close follow-up (including regularly scheduled clinical and radiological assessments), with the intention of intervening with surgery for disease progression or patient preference.MethodsThis is a protocol for a prospective, observational, long-term follow-up multi-centre Canadian cohort study. Consenting eligible adults with small, low risk PTC (< 2cm in maximal diameter, confined to the thyroid, and not immediately adjacent to critical structures in the neck) are offered the choice of AS or surgery for management of PTC. Patient participants are free to choose either option (AS or surgery) and the disease management course is thus not assigned by the investigators. Surgery is provided as usual care by a surgeon in an institution of the patient’s choice. Our primary objective is to determine the rate of ‘failure’ of disease management in respective AS and surgical arms as defined by: i) AS arm – surgery for progression of PTC, and ii) surgical arm - surgery or other treatment for disease persistence or progression after completing initial treatment. Secondary outcomes include long-term thyroid oncologic and treatment outcomes, as well as patient-reported outcomes.DiscussionThe results from this study will provide long-term clinical and patient reported outcome evidence regarding active surveillance or immediate surgery for management of small, low risk PTC. This will inform future clinical trials in disease management of small, low risk papillary thyroid cancer.Registration detailsThis prospective observational cohort study is registered on clinicaltrials.gov (NCT04624477), but it should not be considered a clinical trial as there is no assigned intervention and patients are free to choose either AS or surgery

    International audienceLes auteurs de cette communication désirent proposer au CR23 un format spécial d’une heure pour présenter un atelier sur Jean-Guy Vaillancourt et son œuvre. Quatre auteurs animeront cet atelier : René Audet, Philippe Boudes, Corinne Gendron et Florence Rudolf. D’autres auteurs pourraient s’ajouter à l’équipe d’ici la tenue du Congrès. Jean-Guy Vaillancourt fut un pionnier de la sociologie de l’environnement au Québec et dans la francophonie. Il fut également un fondateur du CR23. Sa disparition à l’été 2015 suscite donc parmi la communauté des sociologues le besoin de revenir sur sa vie et son œuvre. L’atelier que nous proposons vise donc à aborder – sur le mode à la fois de l’analyse et du témoignage – les principaux axes de la pensée sociologique et environnementale de Vaillancourt, parmi lesquels : La genèse et l’évolution de la pensée de Vaillancourt ; l’objet et les méthodes de l’écosociologie ; les mobilisations écologistes d’hier à aujourd’hui ; la réforme environnementale et le développement durable ; des objets de recherche : énergie, changement climatique et gestion des déchets. L’atelier sera conçu de manière à maximiser les échanges et les interventions des participants et contribuera à interroger la dynamique actuelle de la sociologie de l’environnement francophone

    Effects of dietary vitamin supplementation and semen collection frequency on hormonal profile during ejaculation in the boar

    International audienceTo evaluate the effect of dietary and management factors on boar hormonal status during ejaculation, 39 boars were canulated to determine the profiles of luteinizing hormone (LH), follicle-stimulating hormone (FSH), 17 beta-estradiol (EA and testosterone (T) in blood plasma and seminal fluid. Prior to canulation, 18 boars were fed a basal diet (control), whereas the remainder (n = 21) were fed a basal diet supplemented with extra vitamins (supplemented). Within each dietary treatment, two regimens of semen collection were used over the 3 mo preceding the hormonal evaluation: three times per 2 wk (3/2) or three times per wk (3/1). Plasma E(2) was lower (P < 0.01) before ejaculation (232.5 +/- 22.6 pg/mL) than at the onset of ejaculation (255.2 +/- 27.1 ng/mL). Plasma T increased from 5.14 +/- 0.72, before ejaculation to 5.87 +/- 0.86 ng/mL at the onset of ejaculation in supplemented boars, whereas it decreased from 5.15 +/- 0.65 to 4.87 +/- 0.70 ng/mL in controls (diet by time, P < 0.05). At the onset of ejaculation, plasma FSH was higher in 3/2 boars (0.436 +/- 0.06 ng/mL) than in 3/1 boars (0.266 +/- 0.04 ng/mL; P < 0.05). During ejaculation, plasma LH increased linearly (P < 0.01) from 0.59 +/- 0.07 to 0.97 +/- 0.10 ng/mL, and plasma E(2) and T concentrations were correlated (r = 0.62, P < 0.01). Plasma FSH before and during ejaculation was negatively correlated with sperm production (r = -0.60, P < 0.01) and testicular weight (r = -0.50, P < 0.01). In conclusion, dietary and management factors had few impacts on hormonal profiles during ejaculation, but homeostasis of some hormones was related to some criteria of reproductive performance in boars
