    Probing galaxy evolution through HI 21-cm emission and absorption: current status and prospects with the Square Kilometre Array

    One of the major science goals of the Square Kilometre Array (SKA) is to understand the role played by atomic hydrogen (HI) gas in the evolution of galaxies throughout cosmic time. The hyperfine transition line of the hydrogen atom at 21-cm is one of the best tools to detect and study the properties of HI gas associated with galaxies. In this article, we review our current understanding of HI gas and its relationship with galaxies through observations of the 21-cm line both in emission and absorption. In addition, we provide an overview of the HI science that will be possible with SKA and its pre-cursors and pathfinders, i.e. HI 21-cm emission and absorption studies of galaxies from nearby to high redshifts that will trace various processes governing galaxy evolution.Comment: 31 pages, 7 figures, accepted on 27 May 2022 for publication in the Journal of Astrophysics and Astronomy (to appear in the special issue on "Indian participation in the SKA"), figure 4 has been update

    Discriminating neutrino mass models using Type II seesaw formula

    In this paper we propose a kind of natural selection which can discriminate the three possible neutrino mass models, namely the degenerate, inverted hierarchical and normal hierarchical models, using the framework of Type II seesaw formula. We arrive at a conclusion that the inverted hierarchical model appears to be most favourable whereas the normal hierarchical model follows next to it. The degenerate model is found to be most unfavourable. We use the hypothesis that those neutrino mass models in which Type I seesaw term dominates over the Type II left-handed Higgs triplet term are favoured to survive in nature.Comment: No change in the results, a few references added, some changes in Type[IIB] calculation

    Significant benefits of AIP testing and clinical screening in familial isolated and young-onset pituitary tumors

    Context Germline mutations in the aryl hydrocarbon receptor-interacting protein (AIP) gene are responsible for a subset of familial isolated pituitary adenoma (FIPA) cases and sporadic pituitary neuroendocrine tumors (PitNETs). Objective To compare prospectively diagnosed AIP mutation-positive (AIPmut) PitNET patients with clinically presenting patients and to compare the clinical characteristics of AIPmut and AIPneg PitNET patients. Design 12-year prospective, observational study. Participants & Setting We studied probands and family members of FIPA kindreds and sporadic patients with disease onset ≤18 years or macroadenomas with onset ≤30 years (n = 1477). This was a collaborative study conducted at referral centers for pituitary diseases. Interventions & Outcome AIP testing and clinical screening for pituitary disease. Comparison of characteristics of prospectively diagnosed (n = 22) vs clinically presenting AIPmut PitNET patients (n = 145), and AIPmut (n = 167) vs AIPneg PitNET patients (n = 1310). Results Prospectively diagnosed AIPmut PitNET patients had smaller lesions with less suprasellar extension or cavernous sinus invasion and required fewer treatments with fewer operations and no radiotherapy compared with clinically presenting cases; there were fewer cases with active disease and hypopituitarism at last follow-up. When comparing AIPmut and AIPneg cases, AIPmut patients were more often males, younger, more often had GH excess, pituitary apoplexy, suprasellar extension, and more patients required multimodal therapy, including radiotherapy. AIPmut patients (n = 136) with GH excess were taller than AIPneg counterparts (n = 650). Conclusions Prospectively diagnosed AIPmut patients show better outcomes than clinically presenting cases, demonstrating the benefits of genetic and clinical screening.     The Changing Landscape for Stroke\ua0Prevention in AF: Findings From the GLORIA-AF Registry Phase 2

    Background GLORIA-AF (Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation) is a prospective, global registry program describing antithrombotic treatment patterns in patients with newly diagnosed nonvalvular atrial fibrillation at risk of stroke. Phase 2 began when dabigatran, the first non\u2013vitamin K antagonist oral anticoagulant (NOAC), became available. Objectives This study sought to describe phase 2 baseline data and compare these with the pre-NOAC era collected during phase 1. Methods During phase 2, 15,641 consenting patients were enrolled (November 2011 to December 2014); 15,092 were eligible. This pre-specified cross-sectional analysis describes eligible patients\u2019 baseline characteristics. Atrial fibrillation disease characteristics, medical outcomes, and concomitant diseases and medications were collected. Data were analyzed using descriptive statistics. Results Of the total patients, 45.5% were female; median age was 71 (interquartile range: 64, 78) years. Patients were from Europe (47.1%), North America (22.5%), Asia (20.3%), Latin America (6.0%), and the Middle East/Africa (4.0%). Most had high stroke risk (CHA2DS2-VASc [Congestive heart failure, Hypertension, Age  6575 years, Diabetes mellitus, previous Stroke, Vascular disease, Age 65 to 74 years, Sex category] score  652; 86.1%); 13.9% had moderate risk (CHA2DS2-VASc = 1). Overall, 79.9% received oral anticoagulants, of whom 47.6% received NOAC and 32.3% vitamin K antagonists (VKA); 12.1% received antiplatelet agents; 7.8% received no antithrombotic treatment. For comparison, the proportion of phase 1 patients (of N = 1,063 all eligible) prescribed VKA was 32.8%, acetylsalicylic acid 41.7%, and no therapy 20.2%. In Europe in phase 2, treatment with NOAC was more common than VKA (52.3% and 37.8%, respectively); 6.0% of patients received antiplatelet treatment; and 3.8% received no antithrombotic treatment. In North America, 52.1%, 26.2%, and 14.0% of patients received NOAC, VKA, and antiplatelet drugs, respectively; 7.5% received no antithrombotic treatment. NOAC use was less common in Asia (27.7%), where 27.5% of patients received VKA, 25.0% antiplatelet drugs, and 19.8% no antithrombotic treatment. Conclusions The baseline data from GLORIA-AF phase 2 demonstrate that in newly diagnosed nonvalvular atrial fibrillation patients, NOAC have been highly adopted into practice, becoming more frequently prescribed than VKA in Europe and North America. Worldwide, however, a large proportion of patients remain undertreated, particularly in Asia and North America.     Pinch test: a reliable physical sign for management of acute appendicitis

    Introduction: Acute appendicitis is often a surgical emergency. It primarily affects the growing children and needs to be evaluated early. Many patients of acute appendicitis will respond to antibiotics therapy. But a number of such patients will face recurrence and complications thereby pitting their lives in risk. To overcome such accidents acute appendicitis need to be diagnosed early and categorized so as to decide which group will benefit from conservative therapy and who will need emergency operation. The diagnosis of acute appendicitis is based on history and physical examination of the abdomen. Laboratory tests like leukocytosis > 10000/cu.mm and left shift of the Differential leukocyte count are important tests to understand the severity of acute appendicitis. The ALVARADO Score is also widely used for the idea for a better management of acute appendicitis. Pinch test is a single parameter finding to diagnose acute appendicitis. It is a simple examination technique, can be done bedside and more comfortable for the patients. Methods: 100 consecutive patients were taken up for the study. Detailed clinical examination was done. Diagnosis was based on primarily physical findings and history. Investigations were undertaken and Pinch test and its results were documented in all the cases. Alvarado scoring was also done. All patients were operated and co-related to the findings with Pinch test recorded. Result: Out of 100 cases, 59 were females and 41 were males, which showed slight female preponderance of the diseases. The patients were in the age group of 11 to 30 years with a median age of 21 years. The initial sensitivity and specificity of pinch test was 86.36% and 83.33%. In comparison the modified ALVARADO (MASS) score, it was 90.90% and 33.33% respectively. Pinch Test can be a tool to identify the cases of Appendicitis, which need urgent operative treatment. Conclusion: Pinch test can be an additional sign to identify cases of acute appendicitis which will need emergency surgery

    Laser surface coating of Fe-Cr-Mo-Y-B-C bulk metallic glass composition on AISI 4140 steel

    Fe-based bulk metallic glasses exhibit very high hardness, elastic modulus/limit and wear/corrosion resistance. In the present investigation, an attempt has been made to develop an amorphous coating with Fe48Cr15Mo14Y2C15B6 bulk metallic glass on AISI 4140 substrate by laser surface processing. Following coating, the microstructure and phase aggregate were analyzed by scanning electron microscope and X-ray diffraction, respectively. Microhardness and wear resistance were assessed using Vickers microhardness tester and ball-on-plate wear testing machine, respectively. The coating thickness varied directly with incident laser power and interaction time. Despite trials with wide range of process parameters, the present experiments were unable to retain complete amorphous surface microstructure after laser surface coating. Numerical prediction of the thermal profile and related parameters suggest that the cooling rate and thermal gradient experienced by the coated zone were fairly high. Yet failure to retain amorphous/glassy microstructure of an otherwise bulk metallic glassy alloy suggests that compositional changes (solute redistribution) within the coated zone and across the coating-substrate interface are responsible for nucleation and growth of crystalline phases from the melt. However, correlation between coating parameters and surface microstructure and properties allowed determination of the optimum conditions that ensured fine grained uniform microstructure with a significant improvement in hardness and wear resistance
